白内障超声乳化联合人工晶状体植入术在治疗闭角型青光眼合并白内障患者中的应用被引量：37

摘要目的探讨白内障超声乳化联合人工晶状体植入术治疗闭角型青光眼合并白内障患者的临床应用价值。方法选择2011年6月至2013年6月我院收治的闭角型青光眼合并白内障患者,采用白内障超声乳化联合人工晶状体植入术进行治疗,比较患者治疗前后最佳矫正视力和眼压的变化情况,并观察患者治疗后并发症的发生情况。结果与治疗前相比,患者治疗后最佳矫正视力【0.1者明显减少,仅为12.50%,最佳矫正视力0.3~0.4者明显提高,达到38.39%,差异均有统计学意义(均为P【0.05)。与治疗前相比,治疗后急性闭角型青光眼患者眼压由(43.93±10.22)mmHg(1 kPa=7.5 mmHg)降低至(12.45±4.91)mmHg,慢性闭角型青光眼患者眼压由(35.67±11.78)mmHg降低至(13.83±5.50)mmHg,差异也均有统计学意义(均为P【0.05)。术后急性闭角型青光眼患者6眼发生角膜水肿,占10.00%(6/60),慢性闭角型青光眼患者6眼发生角膜水肿,占11.53%(6/52);急性闭角型青光眼患者5眼发生前房纤维素渗出,占8.33%(5/60),慢性闭角型青光眼患者3眼发生前房纤维素渗出,占5.77%(3/52),两组比较差异均无统计学意义(均为P】0.05)。结论白内障超声乳化联合人工晶状体植入术治疗闭角型青光眼合并白内障,能够有效提高患者治疗效果及生活质量,是临床较为理想的治疗方案之一。 Objective To discuss the clinical application value of phacoemulsification combined with intraocular lens(IOL)implantation for patients with angle-closure glaucoma and cataract.Methods The angle-closure glaucoma and cataract patients in our hospital were selected from June 2011 to June 2013 and all the patients were treated with phacoemulsification combined with IOL implantation.The preoperative and postoperative best corrected visual acuity and intraocular pressure of the patients were compared and analyzed and the postoperative complications were investigated.Results Compared with pre-operation,the rate of postoperative best corrected visual acuity less than 0.1 reduced significantly only 12.50%, and the rate of best corrected visual acuity from 0.3 to 0.4 improved significantly up to 38. 39%there were statistical differences(all P<0.05).Compared with pre-operation,the postoperative intraocular pressure in acute angle-closure glaucoma patients was decreased from(43.93±10.22)mmHg(1 kPa=7.5 mmHg)to(12.45±4.91)mmHg,and which in chronic angle-closure glaucoma patients was decreased from(35.67±11.78)mmHg to(13.83±5.50)mmHg,there were statistical differences(all P<0.05).The postoperative corneal edema appeared in 6 eyes(10.00%)with acute angle-closure glaucoma and 6 eyes(11.53%)with chronic angle-closure glaucoma,the postoperative anterior chamber fibrin exudation appeared in 5 eyes(8.33%)with angle-closure glaucoma and 3 eyes(5.77%)with chronic angle-closure glaucoma,there was no statistical difference(all P>0.05).Conclusion Phacoemulsification combined with IOL implantation for patients with angle-closure glaucoma and cataracts can improve clinical effect and quality of life,which is one of the ideal programs for clinical treatment.
