肺癌循环核酸的研究进展 预览

摘要 原发性支气管肺癌(简称肺癌)起源于支气管黏膜或腺体,是目前世界上最常见的恶性肿瘤之一。近年来肺癌发病率在中国呈明显上升趋势,在城市肿瘤死亡排序已上升为第1位。肺癌是一种预后极差的疾病,尽管采用手术、放疗和化疗等多学科治疗,其5年生存率仍低于15%,其中重要的原因是缺少有效的早期诊断方法。传统的痰脱落细胞学和胸部X线的检查已经不能适应肺癌的早期发现,CT、高分辨CT(HRCT)、正电子发射计算机断层显像(PET)等影像学手段又价格昂贵,不适合太规模高危人群的筛查与监测。循环核酸的发现为肺癌的诊断和监测提供了新的靶分子,对肺癌的早期诊断及康复干预可能有重要价值。正常人斑浆中存在有微量的DNA或RNA,主要来源于自身的淋巴细胞,而恶性肿瘤患者尤其是转移癌患者血浆中DNA或RNA含量常明显增高。关于外周血循环核酸的来源。目前的研究倾向两种观点:(1)原发灶肿瘤细胞凋亡或坏死后释放;(2)肿瘤转移过程中循环肿瘤细胞裂解释放。
