Analysis on Epidemiological Characteristics of Age Distribution of Measles in China During 2003--2006

摘要 目的了解全国(未包括香港、澳门特别行政区和台湾地区)2003-2006年麻疹发病年龄特征,为制订消除麻疹策略提供依据。方法对麻疹病例年龄构成和年龄别发病率进行描述流行病学分析。结果全国2003~2006年麻疹主要发生在小年龄组儿童中,但与2003、2004年相比,2005、2006年〈15岁儿童麻疹病例构成比从近85%降至70%左右;〈8月龄和8~11月龄儿童麻疹病例构成比超过10%。各省(自治区、直辖市,下同)麻疹病例具有不同年龄构成模式,2006年〈15岁麻疹病例构成比〈50%的4个省,50%~69%的11个省,≥70%的16个省。2006年≤2岁儿童麻疹发病率大幅度上升,〈1岁儿童发病率〉150/10万,25~30岁人群出现麻疹发病高峰。结论全国2005、2006年麻疹发病年龄特征发生较大变化,〈1岁和≥15岁人群麻疹发病增多。应提高2剂次麻疹减毒活疫苗(MV)接种率和满8月龄婴儿MV及时接种率,做好2~7岁儿童第2剂次MV接种,建立和完善查验预防接种证制度,适时在重点地区和重点人群中开展MV强化免疫。 Objective To know epidemiological characteristics of age distribution of measles in China during 2003-2006 and provide evidence for specific strategies and measures of measles prevention and control. Methods Analyzing epidemiological characteristics of age distribution of measles with descriptive epidemiology. Results Most of measles cases were found among the children aged 0-- 14 years old during 2003-- 2006. The proportions of measles cases aged 0 - 14 years old decreased from 85% in 2003 and 2004 to 70% in 2005 and 2006,which showed decreasing trend. The proportions of measles cases aged 〈 8 months and 8-11 months reached over 10% respectively. Different provinces have different age distribution model. In 2006, the proportions of measles cases aged 0--14 years old are less than 50% in 4 provinces,50%--69% in 11 provinces and ≥70% in 16 provinces. Measles incidence of 0--2 years old children increased remarkably,and the incidence of 〈1 year old children reached over 150/100000. The measles incidence of persons aged 25--30 years old was also high. Conclusions The epidemiological characteristics of age distribution of measles in China have been changed in 2005 and 2006. Measles cases of 〈1 and ≥15 years old increased. Every province should improve the coverage rates of 2 doses and timely coverage rate of the first dose of measles vaccine. Measles vaccination of the second dose should be completed in 2007. All provinces should set up completed system of school immunization entry requirement. Provinces with high incidence rate of measles should conduct measles supplementary immunization activities (SIAs) at suitable time.
