Objective To research the operative technique and the result of local facial muscle flap transfer for the dynamic treatment of cheek-tramatic longstanding facial paralysis. Methods Dissectted two matched facial muscle flaps including SMAS along the injury insicion line, exchanged the position of the matched flaps along the injury line axis and suture; the incision was closed by ＂W＂ plasty. Results All 12 cases with long-standing truma facial paralysis accepted our treatment above got the static symmetry of oral commissure and partly dynamic appearance without false. The result was satisfied with 6 months＂ follow-up. Conclusion Local facial muscle flap transfer can repair the oralnasal deformity of longstanding trauma facial paralysis and reconstruct the oral appearance partly.
