Objective To investigate the T lymphocyte subsets changes in elderly patients with acute exacerbation chronic obstructive pulmonary disease （AECOPD） and effect of immune intervention. Methods 66 cases of elderly patients with AECOPD included in the observation group,40 healthy elderly people were included in the control group, the T lymphocyte subsets levels were compared. 66 cases of elderly patients with AECOPD were divided into conventional treatment group and immune intervention group（which treated by thymosin α1 based on the conventional treatment）, after treatment, the T lymphocyte subsets levels were compared. Results The levels of serum CD3＋ , CD4＋ levels, CD4＋/ CD8＋ in elderly patients with AECOPD were lower than the control group, the difference was of statistically significant （P 〈0.05）; After treatment, the CD3＋, CD4＋ and CD4＋/CD8＋ in immune intervention group were significantly increased, the difference was of statistically significant （ P〈0.05） ; and the CD3＋, CD4＋, CD8＋ and CD4＋/CD8＋ in conventional treatment group did not change significantly （ P 〉 0.05）. Conclusions Elderly patients with AECOPD are of immune dysfunction, thymosin α1 therapy can enhance the patient＇s immune function.
