目的:调查急危重症护士工作环境和道德困境的现状,并分析护理工作环境对道德困境的影响。方法:采用便利抽样法选取长沙市4所三级甲等医院的急危重症护士457名作为研究对象,对其进行问卷调查。结果 :急危重症护士工作环境总分为(90.95±13.24)分,道德困境总分为(53.98±35.89)分;护理工作环境与道德困境呈负相关(P<0.01);控制人口学变量后,护理工作环境中的护士参与医院事务及充足的人力和物力维度可独立解释道德困境29.1%的变异。结论 :急危重症护士道德困境现状不容乐观,护理工作环境对道德困境有一定的预测性,护理管理者可通过改善工作环境来缓解急危重症护士道德困境水平。
Objective: To investigate the working environment and moral distress of emergency and critical care nurses, and to analyze the relationship of working environment and moral distress. Methods: A convenience sample of 457 emergency and critical care nurses from 4 tertiary hospitals in Changsha were investigated by Practice Environment Scale and Moral Distress Scale. Results: The total score of working environment was 90.95±13.24, and the total score of moral distress was 53.98±35.89. Working environment was significantly negatively correlated with moral distress(P<0.01). Nurses’ participation in hospital affairs and sufficient manpower and material resources in nursing work environment can independently explain 29.1% variation of moral distress after controlling demographic variables. Conclusion: The moral distress of emergency and critical care nurses is not optimistic, the working environment can predict moral distress partially, and the moral distress level could be relieved by improving the working environment.
