Objective: Discussing the possible mechanism of Qingdan Hewei Decoction in the treatment of bile reflux gastritis with gallbladder heat invading stomach. Methods: One hundred and thirty-two cases who met the inclusion criteria were divided into treatment group and control group by random number generation method. The treatment group was given free decoction granules of Qingdan Hewei Decoction, and the control group was given Compound Bismuth Aluminate Granules.The expression levels of plasma Ghrelin, COX-1, COX-2 and PGE2 were observed in the treatment group and control group before and after intervention by ELISA method. Results: After treatment, the levels of the Ghrelin, COX-1 and PGE2 in plasma of the two groups were significantly increased(P<0.01), and the level of the treatment group was significantly higher than that of control group(P<0.01). The level of plasma COX-2 in the two groups were significantly decreased(P<0.01), and the level of the treatment group was significantly lower than that in the control group(P<0.01). Conclusion: The mechanism of Qingdan Hewei Decoction in the treatment of bile reflux gastritis with gallbladder heat invading stomach may be through regulating ghrelin/COXPG system. On the one hand, Ghrelin promotes gastrointestinal motility to relieve bile reflux, on the other hand, Ghrelin regulates PGE2 to repair gastric mucosal lesions.
