冠心病患者血清MMP-8和MMP-9水平与冠脉易损斑块关系的研究 预览 被引量:7

Relationship among serum MMP-8,MMP-9 levels and coronary vulnerable plaques in patients with coronary heart disease

摘要 目的:研究冠心病(CHD)患者血清基质金属蛋白酶(MMP)-8和MMP-9水平与经虚拟组织学血管内超声(VH-IVUS)明确的冠脉易损斑块间的关系,分析易损斑块的危险因素,为其诊治提供理论依据。方法:依据VH-IVUS检查是否存在易损斑块,120例CHD患者被分为非易损斑块组(66例)和易损斑块组(54例)。对比分析两组间一般资料、高敏C反应蛋白(hsCRP)、空腹血糖、血脂、血肌酐、血尿酸、MMP-8和MMP-9水平。并依次采用单因素和多因素Logistic回归分析冠脉易损斑块的危险因素。结果:与非易损斑块组相比,易损斑块组hsCRP[(3.33±0.76)mg/L比(8.65±2.19)mg/L]、LDL-C[(2.92±0.51)mmol/L比(3.37±0.57)mmol/L]、MMP-8[(5.30±1.61)ng/ml比(8.98±2.80)ng/ml]和MMP-9[(82.63±6.64)ng/ml比(116.29±21.99)ng/ml]水平显著升高,HDL-C[(1.18±0.22)mmol/L比(1.01±0.19)mmol/L]水平显著降低,P〈0.05或〈0.01。多因素Logistic回归分析显示MMP-8、MMP-9、hsCRP和LDL-C是冠脉易损斑块的独立危险因素(OR=1.379~3.135,P〈0.05或〈0.01)。结论:MMP-8、MMP-9水平可作为预测和识别易损斑块的辅助检查指标,且有助于制定有针对性的治疗方案。 Objective:To study relationship among serum levels of matrix metalloproteinase(MMP)-8,MMP-9 and coronary vulnerable plaques confirmed by virtual histology intravascular ultrasound(VH-IVUS)in patients with coronary heart disease(CHD),analyze risk factors of vulnerable plaques,and provide theoretical basis for diagnosis and treatment of coronary vulnerable plaques.Methods:According to VH-IVUS examined results of virtual histology,a total of 120 CHD patients were divided into non-vulnerable plaque group(n=66)and vulnerable plaque group(n=54).General data,levels of high sensitive C reactive protein(hsCRP),fasting blood glucose,blood lipids,creatinine,serum uric acid,MMP-8 and MMP-9 were measured,compared and analyzed between two groups.Single factor and multi-factor Logistic regression analyses were used in order to analyze risk factors for coronary vulnerable plaques.Results:Compared with non-vulnerable plaque group,there were significant rise in levels of hsCRP[(3.33±0.76)mg/L vs.(8.65±2.19)mg/L],LDL-C [(2.92±0.51)mmol/L vs.(3.37±0.57)mmol/L],MMP-8[(5.30±1.61)ng/ml vs.(8.98±2.80)ng/ml]and MMP-9[(82.63±6.64)ng/ml vs.(116.29±21.99)ng/ml],and significant reduction in HDL-C level[(1.18±0.22)mmol/L vs.(1.01±0.19)mmol/L]in vulnerable plaque group,P〈0.05 or 0.01.Multi-factor Logistic regression analysis indicated that MMP-8,MMP-9,hsCRP and LDL-C were independent risk factors for coronary vulnerable plaques(OR=1.379~3.135,P〈0.05or0.01).Conclusion:MMP-8 and MMP-9 levels can be used as auxiliary indexes to predict and identify vulnerable plaques,which contribute to developing targeted therapeutic protocol.
