内皮型一氧化氮合酶基因G894T多态性位点与新发高血压患者左心室质量指数的相关性研究 预览

摘要 目的:探讨内皮型一氧化氮合酶基因(e NOS)单核苷酸多态性位点G894T(rs1799983)与原发性高血压患者左心室质量指数的相关性。方法:新发的轻中度原发性高血压患者121例,均来自北京市昌平地区的常住汉族人群,予以超声心动图检查,利用Taq Man MGB等位基因分型试剂盒检测患者e NOS基因多态性G894T的分布频率,分析该基因型与患者左心室质量指数的相关性。结果:G894T位点(GT+TT)基因型患者的舒张末期室间隔厚度[(9.64±1.21)vs.(9.10±1.15)mm,P=0.028]和左心室质量指数显著高于GG基因型患者[(87.51±22.23)vs.(74.36±15.98)mm,P=0.004],利用多元逐步回顾分析,校正年龄、性别、体质指数、吸烟、饮酒、心率及血压、血糖及血脂水平后,G894T位点仍与左心室质量指数具有相关性,该位点和心率一起可以解释左心室质量指数31.7%的变异(R2=0.317,P=0.002)。结论:e NOS基因G894T单核苷酸多态性位点可能与高血压患者左心室肥厚的易感性有关。 Objective:To investigate the relationship between the G894T polymorphism of eNOS gene and the left ventricular mass index of patients with hypertension. Methods: 121 patients with mild to moderate hypertension were recruited in this study. The left ventricular mass index (LVMI) predicted from sex and body surface area was calculated according to the equation. Genotyping was performed to identify the G894T polymor- phism of the eNOS gene with the TagMan MGB technology. Results : The interventricular septal thickness and left ,~ent~cular mass inx in patients with (GT ± TT) genotype were significantly higher than those in patients with GG genotype [ IVST ( 9. 64 ± 1.21 ) vs. ( 9. 10 ± 1.15 ), P =0 028 ; LVMI : ( 87.51 ± 22. 23 ) vs. ( 74. 36 ±15.98), P =0.004]. In the stepwise regression analysis, after adjusting the age, sex, BMI, smoking, drinking, heart rate, blood pressure, glucose and lipid, the heart rate and the G894T polymorphism were asso- ciating with the LVMI of patients with hypertension ( R2 = 0. 317, P = 0. 002 ). Conclusion: The G894T poly- morphism in eNOS gene might be one of the determinant factors of left ventricular hypertrophy in patients with essential hypertension.
