授权式教育对肿瘤患者PICC自我管理能力的效果研究 预览

摘要 目的探讨授权式教育在肿瘤化疗间歇期患者参与PICC安全管理中的应用效果,为授权式教育在患者的PICC健康教育中的应用提供科学的理论支持和依据。方法采用方便抽样的方法 ,选取2014年11月—2015年7月在大连大学附属中山医院首次置入PICC并带管出院符合纳入排除标准的肿瘤患者102例,按照置管先后顺序分别划分到对照组和实验组。对照组给予常规健康教育,实验组在此基础上实行授权式教育。干预3个月后,对比患者组间、其自身前后PICC导管自我管理能力及每个维度得分,统计两组患者导管不良事件、导管并发症发生率和非计划性拔管率,从而对授权式教育的干预方式进行效果评价。结果 (1)干预后两组患者PICC导管自我管理能力总分及各维度得分均有提高。实验组患者干预前后导管自我管理能力得分差值d高于对照组,差异有统计学意义(P〈0.05);(2)两组患者导管不良事件、导管并发症总发生率和非计划性拔管率比较,差异有统计学意义(P〈0.05);其中静脉炎发生率和导管堵塞、导管脱出发生率实验组低于对照组,差异有统计学意义(P〈0.05);导管感染、血栓的发生率以及导管破损发生率实验组低于对照组,但差异无统计学意义(P〉0.05)。结论授权式教育与常规健康教育均可提高患者的自我管理能力,减少导管并发症和不良事件以及非计划拔管的发生,但授权式教育的效果优于常规健康教育方式,故建议在推广授权式教育的同时辅以常规健康教育方式,实现健康教育的多样化,从而满足患者的健康需求,为患者提供更优质的服务,创造安全的护理环境。 Object ive To study the application effect of empowering education in the PICC safety management of patients during the tumor chemotherapy interim period and provide scientific theory basis for the application in the PICC health ed-ucation. Methods 102 cases of tumor patients with first placement of PICC and discharged with tube in Affilated Zhongshan Hospital Dalian University in Dalian from November 2014 to July 2015 were selected and divided into two groups according the tube placement admission, the control group adopted the routine health education, while the test group adopted the em-powering education, after 3-month intervention, the PICC catheter self -management ability scores and every dimension score of patients were compared and the catheter adverse events, incidence rate of catheter complications and unplanned tube drawing rate of the two groups were counted thus evaluating the effect of empowering education intervention method.Results After intervention, the PICC catheter self-management total score and various dimensional scores were improved, and before and after intervention, the self-management ability D-value in the experimental group was higher than that in the control group, and the difference was statistically significant (P 〈 0 .0 5 ) , and the differences in the catheter adverse events, total inciden ce rate of catheter complications and unplanned tube drawing rate between the two groups were statistically significant (P 〈 0 .0 5 ) ,and the incidence rate of phlebitis, catheter obstruction and catheter pulling in the test group was lower than that in the control group, and the difference was statistically significant(P〈 0.05), and the incidence rate of catheter infection, thrombus and catheter defect in the test group was lower than that in the control group, and the difference was statistically significant (P〉0.05) .
