河北省2016年脊髓灰质炎实验室运转情况分析 被引量:1

Analysis on the running status of the polio laboratory in Hebei Province in 2016

摘要 目的分析河北省2016年脊髓灰质炎(脊灰)实验室运转情况,为维持河北省无脊灰状态提供实验室依据。方法对河北省各市通过急性弛缓性麻痹(Acute flaccid paralysis,AFP)病例监测信息报告管理系统报告的AFP病例及河北省脊灰实验室监测数据进行分析,对WHO要求的各项监测指标进行评价。结果 2016年河北省AFP病例监测信息报告管理系统共报告AFP病例365例,实验室收到362例AFP病例粪便标本719份,5例分离到脊灰病毒(Poliovirus,PV),分离率为1.38%;49例分离到非脊灰肠道病毒(NPEV),分离率为13.54%。分离的PV分离成单血清型共计10株,VP1区编码区核苷酸均发生了不同程度变异,但都以低个数变异为主,均为疫苗相似株(Sabin Like,SL)。2016年接受WHO一次病毒分离和两次型内鉴定(Intra-typic differentiation,ITD)盲样标本能力考核,实验室各项监测指标均远远超过WHO的认证标准。按WHO要求,细胞支原体检测结果均为阴性,没有受到支原体污染,细胞敏感性的评价两种细胞都对PV敏感、有效。结论河北省脊灰实验室运转正常,没有发现脊灰野病毒(Wild poliovirus,WPV)和疫苗衍生脊灰病毒(Vaccine-derived poliovirus,VDPV)及VDPV病例,为河北省维持无脊灰状态提供了准确、可靠的病毒学依据。 Objective To analyze the running status of polio laboratory in Hebei Province in 2016 and provide laboratory basis for maintaining polio-free status in Hebei Province. Methods Cases of acute flaccid paralysis(AFP) reported from AFP cases monitoring information management system in each city of Hebei Province and the monitoring data from Hebei polio laboratory were analyzed and the surveillance indicators required by WHO were evaluated. Results A total of 365 AFP cases were reported from Hebei AFP cases monitoring information management system and 362 AFP cases and 719 feces specimens were collected by the laboratory,among which 5 were isolated polio virus(PV) with the isolated rate of 1. 38% while 49 were isolated non-polio-enterovirus(NPEV) with the isolated rate of 13. 54%. A total of 10 isolated PV were separated into single serotype. Different degrees of variations of nucleotide occurred in both VP1 region and coding region which were mainly low number variations and all of them were Sabin Like(SL). In 2016,Hebei polio laboratory passed the proficiency tests of blind samples of virus isolation once and intra-typic differentiation twice. The monitoring indicators of the laboratory were far beyond the certification standards set by WHO. According to the standards set by WHO,the results of mycoplasma detection of cells,which were not contaminated by mycoplasma,were all negative. In the evaluation of cellular sensitivity,both of the two kinds of cells were sensitive and effective to PV. Conclusion Hebei polio laboratory is running well. No Wild poliovirus(WPV),Vaccine-derived poliovirus or VDPV case is detected,which provides accurate and reliable virological evidence for maintaining the polio-free status in Hebei Province.
