糖脂代谢异常与缺血性脑卒中的相关性分析 预览

摘要 目的探讨糖脂代谢与缺血性脑卒中(ischemic stroke,IS)的关系,为IS的防治提供理论依据。方法选取受试对象共314例,依据颅脑CT或磁共振成像(magnetic resonance imaging,MRI)将受试对象分为IS组(试验组)149例和对照组165例,记录年龄、糖尿病史、高血压病史,检测空腹血糖(fasting blood glucose,FBG)、三酰甘油(triacylglycerol,TG)、总胆固醇(total cholesterol,TC)、高密度脂蛋胆固醇(high-density lipoprotein cholesterol,HDL-C)、低密度脂蛋胆固醇(low-density lipoprotein cholesterol,LDL-C),检测糖化血红蛋白A1c(glycated hemoglobin A1c,HbA1c)水平,对比分析上述相关指标在2组中的差异,应用Logistic回归分析方法分析各指标与IS发生的关系。结果试验组中HbA1c和TG水平均高于对照组(P〈0.05),FBG、TC和LDL-C有增高趋势,HDL-C有减低趋势(P〉0.05)。Logistic回归分析结果提示,糖尿病、HbA1c和TG与IS发生关系密切。结论糖尿病、HbA1c及TG与IS发生的关系更密切,在关注IS患者危险众多危险因素的同时更应注意糖尿病、HbA1c及TG水平。 Objective To explore the relationship between glycolipid metabolism and ischemic stroke(IS) in order to provide theoretical basis for prevention and treatment of IS. Methods A total of 314 patients with IS were enrolled in the study. According to the examination results of CT or magnetic resonance imaging(MRI),these patients were divided into ischemic stroke group(IS group,n = 119) and control group(n = 195). The patient's age,history of diabetes,history of hypertension,the levels of fasting blood glucose(FBG),triglyceride(TG),total cholesterol(TC),high density lipid cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C) and glycated hemoglobin A1 c(HbA1c) were observed and compared between two groups. Moreover the correlation between these indexes and pathogenesis of IS was analyzed by using Logisticregression analysis. Results The levels of HbA1c and TG in IS group were significantly higher than those in control group(P〈0. 05),and the levels of FBG,TC and LDL-C had a increase trend,and HDL-C showed a decrease trend(P〈0. 05) in IS group. Logistic regression analysis showed that there was a close correlation between these indexes and pathogenesis of IS. Conclusion There is a close correlation between diabetes,HbA1c,TG and pathogenesis of IS,therefor,it is quite important to pay much attention to diabetes,HbA1c,TG in patients with IS.
