早产儿应用乳铁蛋白及人促红素临床疗效观察 预览

摘要 目的探讨在早产儿出生后早期应用乳清乳铁蛋白(LF)及重组人促红细胞生成素(rHuEPO)治疗,观察早产儿在营养、贫血、免疫、感染及神经系统发育等方面的临床疗效。方法将2014年3月至2018年3月南宁市第一人民医院216例不同胎龄早产儿资料纳入临床研究,根据给药方式将其分为乳清乳铁蛋白联合重组人促红细胞生成素组(n=110)、乳清乳铁蛋白组(n=106)。乳清乳铁蛋白连续服用至矫正月龄3月龄,重组人促红细胞生成素连续给药至矫正胎龄40周。生后第8天、第2周末、第1月龄、第2月龄、第3月龄、第4月龄、第6月龄测定早产儿血红蛋白、红细胞压积、网织红细胞计数、血清铁蛋白、血清铁、IgG,统计患儿体重增长及感染率、输血病人次,矫正胎龄40周时行新生儿行为神经测定(NBNA)检查,6月龄行婴幼儿期智能发育评估,判定临床疗效。结果两组早产儿生后第8天查血红蛋白(Hb)、红细胞压积(Hct)、网织红细胞计数(Ret)、血清铁蛋白(SF)、血清铁(SI)水平比较差异均无统计学意义(均P〉0.05)。两组Hb、Hct水平都是在生后1个月降至低值;Ret、SF、SI在2周末降至低值,1月龄时较2周末升高。2月龄、3月龄时Hb、Hct、Ret、SF、SI又都逐渐升高,在4月龄、6月龄时两组比较差异均无统计学意义(均P〉0.05);两组间Ret、SF在治疗2周末时比较无统计学差异(均P〉0.05),LF+rHuEPO组在治疗后1、2、3月龄时血红蛋白、红细胞压积、血清铁水平均高于LF组,差异均有统计学意义(tHb值分别为6.991、2.726、6.766;tHct值分别为3.548、6.414、3.239,tSI值分别为5.91、3.009、9.312,均P〈0.05)。两组治疗过程中给予输血治疗早产儿62例,其中LF组37例(34.90%),LF+rHuEPO组共25例(22.70%),两组输血率比较差异有统计学意义(χ^2=3.912,P〈0.05)。在生后前两周两组早产儿体重增长无差异(� Objective To explore the treatment of lactoferrin(LF)and recombinant human erythropoietin(rHuEPO)in early neonatal period in premature infants and to observe the clinical efficacy on nutrition,anemia,immunity,infection and nervous system development of preterm infants.Methods From March 2014 to March 2018,data of 216 preterm infants with different gestational age in Nanning First People's Hospital were included in clinical study.According to method of drug administration,infants were divided into LF combining rHuEPO group(n=110)and LF group(n=106).LF was continuously used until corrected age of 3 months.Recombinant human erythropoietin was administered continuously until corrected age of 40 weeks.At 8 days after birth,at the end of second week after birth,at 1 month old,2 months old,3 months old,4 months old and 6 months old,hemoglobin(Hb),hematocrit(Hct),reticulocyte count(Ret),serum ferritin(SF)and serum iron(SI),IgG of preterm infants were detected,weight growth,infection rate,number of patients receiving blood transfusion were recorded.Neonatal behavioral neurological assessment(NBNA)examination was performed at corrected age of 40 weeks.Intelligent development assessment for infants was implemented at 6 months of age.Clinical curative effect was determined.Results Differences in Hb,Hct,Ret,SF and SI levels in two groups were not statistically significant(all P〉0.05).Levels of Hb and Hct in both groups decreased to low level at 1 month after birth.Values of Ret,SF and SI dropped to low level at the end of second week,which were higher at age of 1 month.Hb,Hct,Ret,SF and SI gradually increased at 2 months and 3 months old,and the differences in two groups were not statistically significant at 4 months and 6 months old(all P〉0.05).There were no statistically significant differences in Ret and SF between two groups at the end of second week after treatment(all P〉0.05),and Hb,Hct and SI levels in LF combining rHuEPO group were higher than those in LF gro
