高血压合并冠心病患者PCI后外周血TLR2、TLR4、TNF-α、IL-1β的表达及意义 预览 被引量:1

摘要 目的检测高血压合并冠心病患者在经皮冠状动脉介入术(PCI)后外周血中Toll样受体2(TLR2)、Toll样受体4(TLR4)、肿瘤坏死因子-α(TNF-α)和白介素-1β(IL-1β)的表达水平,并探究其临床意义。方法选择2016年3月~2017年8月于孝感市中心医院心血管内科行PCI的50例高血压合并冠心病患者(高血压合并冠心病组)和40例单纯冠心病患者(单纯冠心病组),另选择40例身体健康者作为对照组。冠心病患者按照冠状动脉造影结果,行选择性PCI治疗。在PCI治疗前和治疗后7d,实时定量PCR(RT-qPCR)检测外周血TLR2和TLR4mRNA水平,酶联免疫吸附试验(ELISA)检测外周血TNF-α和IL-1β蛋白表达水平。结果PCI前,高血压合并冠心病组外周血TLR2、TLR4mRNA水平均显著高于对照组和单纯冠心病组(P<0.05);PCI后,高血压合并冠心病组外周血TLR2、TLR4mRNA水平均显著降低(P<0.05),但TLR2mRNA水平仍显著高于对照组(P<0.05)。PCI前,高血压合并冠心病组外周血TNF-α、IL-1β蛋白水平均显著高于对照组和单纯冠心病组(P<0.05);PCI后,单纯冠心病组、高血压合并冠心病组外周血TNF-α和IL-1β蛋白水平显著降低(P<0.05),但仍高于对照组(P<0.05)。PCI前后,外周血TLR2、TLR4、TNF-α和IL-1β表达水平均显著呈正相关关系(P<0.05)。结论冠心病患者外周血TLR2、TLR4、TNF-α和IL-1β表达量增加,合并高血压可增加其水平,PCI可降低冠心病患者外周血TLR2、TLR4、TNF-α和IL-1β水平。 Abstract]Objective To detect the expressions of tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α)and interleukin-1β(IL-1β),and discuss the clinical significance in patients with hypertension complicated by coronary heart disease(CHD)after percutaneous coronary intervention(PCI).Methods The patients with hypertension complicated by CHD(hypertension+CHD group,n=50),patients with only CHD(CHD group,n=40)and healthy controls(control group,n=40)were chosen from Department of Cardiology of Central Hospital of Xiaogan City from Mar.2016 to Aug.2017.All patients were given optional PCI according to results of coronary angiography(CAG).The levels of TLR2 mRNA and TLR4 mRNA were detected by using RT-qPCR before and 7 d after PCI.The protein expressions of TNF-αand IL-1βwere detected by using ELISA.Results Before PCI,levels of TLR2 mRNA and TLR4 mRNA were significantly higher in hypertension+CHD group than those in control group and CHD group(P<0.05).After PCI,levels of TLR2 mRNA and TLR4 mRNA decreased significantly in hypertension+CHD group(P<0.05),but TLR2 mRNA level was still higher than those in control group(P<0.05).Before PCI,protein expressions of TNF-αand IL-1βwere significantly higher in hypertension+CHD group than those in control group and CHD group(P<0.05).After PCI,protein expressions of TNF-αand IL-1βdecreased significantly in CHD group and hypertension+CHD group(P<0.05),but were still higher than those in control group(P<0.05).Before and after PCI,expressions of TLR2,TLR4,TNF-αand IL-1βall showed significant positive correlations(P<0.05).Conclusion The expressions of TLR2,TLR4,TNF-αand IL-1βall increase in CHD patients,and will be higher when they are complicated by hypertension.PCI can reduce the levels of TLR2,TLR4,TNF-αand IL-1β.
