目的探讨数字评分法(NRS)指导痔手术后镇痛药给药时机对患者的影响。方法选取2018年4-8月收治于某医院肛肠外科痔患者121例,按随机收治病区分为A、B两组,A组为对照组共纳入病例59例,B组为实验组共纳入病例62例。实验组患者使用NRS确定镇痛药给药时机,对照组患者按现行习惯实施给药。比较两组患者术后48 h内的疼痛体会、首次给药时间差、48 h镇痛药物使用总量及副作用的差异。结果实验组患者术后4个时间节点的疼痛感均低于对照组(P<0.05),实验组患者实施首次镇痛给药时间为(331±41)min,对照组为(359±15)min,两组差异有统计学意义(P<0.05)。术后48 h帕瑞昔布钠总使用量,A组为(4.97±0.22)次,B组为(5.03±0.73)次,两组差异无统计学意义(P>0.05)。两组患者在术后48 h内的不良反应率无统计学差异(P>0.05)。结论NRS评分量表指导痔手术后镇痛药的给药时机,可以减轻患者痛苦,改善患者手术体验。
Objective To investigate the effect of numerical rating scale( NRS) guided timing of analgesic drug administration on analgesia after surgery. Methods A total of 121 patients with hemorrhoid from April 2018 to August 2018 were selected and divided into observation group and control group randomly,59 cases were enrolled in the control group,and 62 cases were included in the experimental group. The experimental group used NRS to determine the timing of administration of analgesics,while the control group was administered according to current practice. The pain experience within 48 hours of post-surgery,the first administration time,total amount of analgesic drugs used within 48 hours and the side effects were compared between the two groups.Results The pain of experimental group was lower than that in the control group at the 4 moments after surgery( P<0.05). The first analgesic administration time in the experimental group was( 331 ± 41) minutes,and the control group was( 359 ± 15) min,there was a significant difference between the two groups( P< 0.05). The total dosage of parecoxib within 48 hours after surgery was( 4.97±0.22) times in the control group and( 5.03±0.73) times in the experimental group,there was no significant difference between the two groups( P>0.05). There were no significant differences in the adverse reaction rate between the two groups within 48 hours after surgery( P>0.05).Conclusions The NRS rating scale guides the timing of administration of analgesics after surgery could alleviate the patient’s pain and improve the patient’s surgical experience.
