冠心病合并糖尿病患者血清炎症因子水平变化及其临床意义 预览

摘要 目的:探讨冠心病合并糖尿病患者可溶性细胞间粘附分子(ICAM-1)、几丁质酶-3样蛋白-l(YKL-40)、脂蛋白相关磷脂酶A2(Lp-PLA2)水平变化及临床意义。方法:入选冠心病患者120例,分为单纯冠心病组84例和冠心病合并糖尿病组36例,另选同期有胸痛症状冠状动脉造影正常的患者 20例为对照组。采用Gensini评分系统评定冠状动脉血管病变狭窄程度,用酶联免疫吸附试验(ELISA)法分别检测血清ICAM-1、YKL-40、LP-PLA2水平。对各组进行比较分析,并分别与Gensini积分进行相关性分析。结果:与对照组相比,单纯冠心病组和冠心病合并糖尿病组ICAM-1、YKL-40、LP-PLA2性均明显升髙,差异有统计学意义(P<0. 05或P<0.01);冠心病合并糖尿病组ICAM-I、YKL-40、LP-PLA2活性高于单纯冠心病组,差异有统计学意义(P均<0. 01);非参数Spearman相关分析显示单纯冠心病组 ICAM-l、YKL-40 水平与 Gensini 积分无明显相关性(r= 0. 184,r= 0. 180,P 均>0.05),LP-PLA2 水平与 Gensini积分呈正相关(r= 0. 180,P<0.01)。冠心病合并糖尿病组ICAM-1、YKL-40、LP-PLA2水平与 Gensini 积分无明显相关性(r=0.054,r=0. 031,r=-0. 045,P 均>0.05)。结论:炎症因子 ICAM-1、YKL- 40、LP-PLA2可能在冠心病合并糖尿病的发病机制中起重要作用,但与冠状动脉病变程度之间的关系值得进一步研究。 Objective: Aim To investigate the change of inflammatory factors including serum ICAM- 1, YKL-40, Lp-PLA2 in patients with coronary heart disease combined diabetes and its clinical significance. Methods: 120 patients with coronary heart disease were divided into 2 groups: 84 cases of simple coroanry heart disease group;36 cases of coronary heart disease combined diabetes group;20 patients with chest pain and normal angiography served as a control group. Gensini score was used to assessed the severity degree of cor-onary artery stenosis. Serum levels of ICAM-1、YKL-40、Lp-PLA2 of different groups were measured by enzyme- linked immunosorbent assay and the comparative analysis on them was canied out.Correlation between ICAM-I, YKL-40,Lp-PLA2 and Gensini score were analyzed. Results: Compared to normal control group, the levels of the serum ICAM-I、YKL-40、LP-PLA2 were significantly increased in the simple coronary heart diseases group and coronary heart disease combined diabetes group(P<0. 05,P<0. 01 );In coronary heart disease combined diabetes group,the levels of the serum ICAM-1、YKL-40、LP-PLA2 were higher than those in simple coronay heart disease group(P<0. 01).The levels of the serum ICAM-IAYKL-40 were not and LP-PLA2 was pastiovely related the gensini score(r = 0. 184,r=0. 180,P>0. 05) and LP-PLA2 was pastiovely related the gensini score (r=0. 180,P<0. 01) in simple coronary heart disease group.The levels of the serum ICAM-I、YKL-40、LP- PLA2 were not related the gensini score(r=0. 054,r = 0. 031, r =-0. 045,P>0. 05) in coronary heart disease with diabetes group. Conclusions: The inflammatory factors including serum ICAM-1 、YKL-40、LP-PLA2 may play an imporant role in the pathogenesis of the coronary heart disease in patients with heart disease combined diabetes.
