加用异丙托溴铵对哮喘患儿肺功能、炎症因子及VEGF表达的影响 预览

Effects of ipratropium bromide on pulmonary function,inflammatory factors and VEGF expression in asthmatic children

摘要 目的研究加用异丙托溴铵治疗小儿哮喘的临床疗效,及其对患儿肺功能、炎症因子和血管内皮生长因子(VEGF)表达的影响。方法选取我院2017年3月至2018年3月收治的90例哮喘患儿,采用随机数字法将其分为对照组和观察组,每组45例。两组患儿均接受常规吸氧、抗感染、抗病毒等治疗。此外,对照组患儿接受沙丁胺醇及氨茶碱治疗;观察组患儿在对照组治疗的基础上,加用异丙托溴铵治疗。比较两组患儿的治疗总有效率,观察两组患儿治疗前后的肺功能、炎症因子及VEGF表达水平的变化。结果观察组患儿的治疗总有效率明显高于对照组(91.11%vs.73.33%,P<0.05);治疗后,观察组患儿的第1秒用力呼气容积(FEV1)、1秒率(FEV1/FVC)和呼气高峰流量(PEFR)分别为(93.45±15.68) L、94.42%±15.80%、(2.89±0.63) L/s,均明显高于对照组[(78.24±12.94) L、85.57%±14.74%、(2.43±0.57) L/s](P<0.05);治疗后,观察组患儿的白介素-13(IL-13)、转化生长因子β1(TGF-β1)及VEGF表达水平分别为(18.25±5.59)、(98.87±10.56)、(194.42±20.49) ng/L,均明显低于对照组[(33.24±7.94)、(132.20±14.62)、(264.57±32.74) ng/L](P<0.05)。结论加用异丙托溴铵治疗小儿哮喘临床疗效显著,可明显降低患儿血清的IL-13、TGF-β1及VEGF表达水平,改善患儿的肺功能,抑制气道炎症反应及气道重构。 Objective To study the clinical efficacy of ipratropium bromide in the treatment of asthma in children and its influence on pulmonary function,inflammatory factors and vascular endothelial growth factor(VEGF) expression in children.Methods A total of 90 asthmatic children treated in our hospital from March 2017 to March 2018 were selected.They were randomly divided into control group and observation group,45 cases in each group.Children in the two groups received conventional oxygen inhalation,anti-infection and anti-virus treatment.In addition,children in control group received salbutamol and aminophylline,and children in observation group was treated with ipratropium on the basis of treatment for control group.The total effective rate was compared between the two groups.Changes in lung function,inflammatory factors and VEGF expression before and after treatment were observed.Results The total effective rate of observation group was higher than that of control group(91.11% vs.73.33%,P<0.05).After treatment,the FEV1 and FEV1/FVC and peak expiratory flow(PEFR) in observation group was(93.45±15.68) L,94.42%±15.80%,and(2.89±0.63) L/s,respectively,which were higher than those in control group [(78.24±12.94) L,85.57%±14.74%,and(2.43±0.57) L/s](P<0.05).After treatment,the value of interleukin-13(IL-13),TGF-β1 and VEGF expression in observation group was(18.25±5.59),(98.87±10.56) and(194.42±20.49) ng/L,respectively,which were lower than those in control group [(33.24±7.94),(132.20±14.62) and(264.57±32.74) ng/L](P<0.05).Conclusion The addition of ipratropium bromide in the treatment of children with asthma has a significant clinical effect,which can significantly reduce the serum levels of IL-13,TGF-β1 and VEGF expression,improve the lung function of children,and inhibit the airway inflammation and air tract reconstruction.
