冠心病患者脂蛋白相关磷脂酶A2活性水平及其临床意义 预览

Relationship between lipoprotein-related phospholipase A2 activity and its clinical significance in patients with coronary heart disease

摘要 目的探讨影响冠心病患者中脂蛋白相关磷脂酶(lipoprotein associated phospholipase A2,Lp-PLA2)活性水平及危险因素。方法采取病例研究,选取2017年1月1日至2017年5月21日于胜利油田中心医院总共400例样本,冠心病患者199例,对照组201例,其中男263例,女137例;比较不同组间Lp-PLA2的水平,并采用多因素回归模型分析Lp-PLA2水平的影响因素和冠心病的危险因素。结果冠心病组中的Lp-PLA2水平均明显高于对照组,差异均具有统计学意义( P <0.05)。Lp-PLA2水平与年龄、体重指数(BMI)、2型糖尿病、甘油三酯(TG)、低密度脂蛋白(LDL-C)、吸烟呈正相关关系( P <0.05);而与高密度脂蛋白(HDL-C)呈负相关关系( P <0.05)。吸烟、2型糖尿病、HDL-C水平降低和Lp-PLA2水平升高是冠心病的独立危险因素。结论推测由于空气污染,PM10、SO2、NO2、CO、O3等的含量过高,造成心脑血管疾病的患病风险加大,Lp-PLA2是冠心病的独立危险因素,对于冠心病患者的风险预测及预后评估有重要作用。 Objective To study the correlationship among plasma lipoprotein associated phospholipase A2 (Lp-PLA2) levels of coronary heart disease (CHD) patients in the oil pollution.To evaluate their relationship and prognostic value.Methods Case control study.A total of 400 consecutively admitted patients from January 2017 to June 2017.All patients received coronary angiography and then divided into two groups.CAD group,n =199,and Control group,n = 201,the patients with coronary stenosis <50%,263 males and 137 females.To compared with different group the levels of Lp-PLA2.The influencing and the risk factors of Lp-PLA2 in coronary heart disease were analyzed by multivariate regression model.Results The level of Lp-PlA2 in coronary heart disease group was significantly higher than that in control group ( P <0.05).The level of Lp-PLA2 had a positive correlation with age,body mass index (BMI),type 2 diabetes mellitus,total cholesterol (TC),low density lipoprotein (LDL-C) and smoking ( P <0.05),then high density lipoprotein (HDL-C) was negatively correlated ( P <0.05).Smoking,type 2 diabetes,decreased HDL-C levels and elevated Lp-PLA2 levels were independent risk factors for coronary heart disease.Conclusion Air pollution in the PM10,SO 2,NO 2,CO,O 3,etc.,leads to increased prevalence of cardiovascular and cerebrovascular diseases,Lp-PLA2 is an independent risk factor for coronary heart disease.So it has an important role in risk prediction and prognosis evaluation of patients with coronary heart disease.
