2018年武汉市血吸虫病消除达标考核地区疫情评估 预览

Evaluation on the endemic status of schistosomiasis in areas to the elimination criteria in Wuhan city in 2018

摘要 目的对武汉市3个区的10个街(场)进行血吸虫病消除达标考核与疫情评估。方法按照《血吸虫病控制和消除(GB15976-2015)》,对武汉市3个传播阻断区的10个街(场)开展人群、家畜、螺情、档案资料考核以及现场钉螺调查。结果10个考核街2013~2017年5年内血检51226人,血检阳性117人次,2013~2017年血阳率分别为0.183%、0.155%、0.543%、0.117%、0.149%,共粪检117人次,粪检阳性0例。5年血检阳性的49例病人经流调发现均为历史遗留病人;5年累计存栏家畜21294头,全部粪检未发现感染性家畜;2013~2017年未发现阳性螺,活螺平均密度分别为0.054、0.031、0.005、0.007、0.005只/0.11m^2;现场抽样考核17个流行村,历史累计有螺面积3739.43hm^2,现有钉螺面积138.53hm^2,查螺5300框,捕获钉螺38只,活螺0只,未发现感染性钉螺;达到传播阻断后,3个区的10个街至少连续10年未发现本地感染的血吸虫病病人、病畜与感染性钉螺。结论2018年武汉市东西湖、新洲、江岸3区的10个街已达血吸虫病消除标准,为下一步全区达标提供了充分依据,当前疫情处于流行历史最低水平,应继续落实监测巩固措施,严防反弹风险。 Objective To assess the endemic status of schistosomiasis in the 10 communities under 3 administrative districts in Wuhan city for evaluation of schistosomiasis elimination as the defined criteria. Methods By the Criteria for Control and Elimination of Schistosomiasis(GB 15976-2015), we conducted a survey on the documented data from and field study in population, livestock and snail status in the 10 communities under 3 administrative districts in Wuhan city, where schistosomiasis transmission was achieved. Results Between 2013 and 2017, a total of 51 226 residents received indirect hemagglutination (IHA ) test, and positive results were found in 117. The positive rate was 0.183%, 0.155%, 0.543%, 0.117% and 0.149%, respectively from 2013 to 2017. Stool test was performed in 117 subjects, and revealed no positive antibody. Forty-nine patients with positive findings were historical infection in the past 5 years. A total of 21 294 livestock were raised in the 5 years, and subjected to fecal test, in which no infection was identified. Infected snails were free between 2013 and 2017. The average density for living snails was 0.054, 0.031, 0.005, 0.007, 0.005 heads/0.11 m^2, respectively from 2013 to 2017. Areas with snails were 3 739.43 hm^2 in history, and 138.53 hm^2 at present. Field sampling survey was conducted in 17 villages in 5 300 frames. Thirty-eight dead snails were obtained, yet no living snail was caught, and no infected snails were found. After reaching the criteria of transmission interruption, no local infection in population, livestock and snails was reported in 10 consecutive years in the 10 communities under the 3 administrative districts. Conclusion Transmission interruption has been achieved in the 10 communities under administrative districts of Dongxihu, Xinzhou and Jiangan. This may be the evidence for evaluation of the control target at schistosomiasis.
