目的分析颅脑损伤后患者的中性粒细胞、凝血功能水平及其与预后的评估。方法选取2018年2月-2019年2月本院诊治颅脑损伤60例患者资料,设为研究组,同期选取健康体检者30例,设为对照组,均测定患者中性粒细胞、凝血指标水平,分析两组凝血功能指标水平以及研究组不同预后中性粒细胞、凝血指标情况。结果研究组凝血酶相关时间(thrombin time,TT)(16.52±0.48)s、凝血酶原相关时间(prothrombin time,PT)(14.52±1.54)s、激活部分的凝血酶原时间(activated partial thromboplastin time,APTT)(36.23±6.52)s、国际标准比值(International Normalized Ratio,INR)(1.19±0.61)及血浆纤维蛋白原(fibrinogen,FBG)(2.32±0.26)g/L,与对照组比较,差异有统计学意义(P <0.05);研究组患者中,预后良好组PT(12.15±1.35)s、APTT(32.23±4.56)s及INR(1.10±0.22),均比不良组少,差异有统计学意义(P <0.05);预后良好组中性粒细胞(neutrophil,N)(0.73±0.10)、中性粒细胞/淋巴细胞比(neutrolphiltolymphocyteratio,NLR)(3.06±1.05),均比不良组少,差异有统计学意义(P <0.05)。结论临床可对颅脑损伤患者检测中性粒细胞、凝血功能水平,以评估患者病情及预后情况,具一定临床应用价值。
Objective To analyze the levels of neutrophils in and coagulation function and evaluate the prognosis effects. Methods The clinical data of60 patients with craniocerebral injury diagnosed and treated in the hospital from February 2018 to February 2019 were selected as the study group.Another 30 healthy physical examinees in the hospital during the same period were selected as the control group. The levels of neutrophils and coagulation indexes were determined. The coagulation function of the two groups, neutrophils and coagulation indexes of different prognosis in the study group were analyzed. Results The TT, PT, APTT, INR and FBG in the study group were(16.52±0.48)s,(14.52±1.54)s,(36.23±6.52)s,(1.19±0.61) and(2.32±0.26)g/L respectively, with statistically significant difference compared with those in the control group(P < 0.05). In the study group, the PT, APTT and INR of patients with good prognosis group were(12.15±1.35)s,(3.23±4.56)s and(1.10±0.22), all shown to be less than those in patients with poor prognosis group, the difference was statistically significant(P < 0.05). The N and NLR in patients with good prognosis group were(0.73±0.10) and(3.06±1.05), both less than those in patients with poor prognosis group, and the difference was statistically significant(P < 0.05). Conclusion For patients with craniocerebral injury, clinical detection of neutrophils and coagulation function is with certain clinical value, which can help evaluate their conditions and prognosis.
