机器人辅助腹腔镜治疗TURP术后前列腺癌的效果分析

摘要 目的:探讨达芬奇(DaVinci)机器人辅助腹腔镜治疗经尿道前列腺电切术(TURP)术后前列腺癌患者的效果。方法:分析2016年5—10月我院收治的6例前列腺癌患者,患者既往均行过TURP术,本次入院行机器人辅助腹腔镜前列腺癌根治术,两次手术间隔时间平均为9.6个月,大多在3个月以内,观察手术时间,术中失血量,引流管及导尿管留置时间,术后住院时间,术后并发症。结果:6例手术均顺利完成,无中转手术病例。手术时间130~250m in,平均180m in;术中出血50~200ml,平均140ml,无1例术中输血;术后住院时6~11d,平均术后住院时间8d;术后3~7d拔除引流管,平均4d;术后14-21天拔除尿管,平均17d,1例患者拔除尿管后有轻度尿失禁。结论:TURP术后行DaVinci机器人辅助前列腺癌根治性切除术疗效满意,TURP术增加了手术操作难度,但在机器人辅助腹腔镜技术熟练的条件下是安全可行的。 Objective To evaluate the feasibility of patients who underwent robot-assisted laparoscopic radical prostatectomy(RALP)with previous TURP.Methods We retrospectively analyzed the clinical data and the surgical methods of six prostate cancer patients with previous TURP who were performed daVinci robot-assisted radical prostatectomy from May.2016 to Oct.2016.The average time interval of two operations was nearly 10 months,mainly for three months.The observation items included the operative time,the blood loss,the duration of drainage tube and catheter,the average postoperative hospital stay,the postoperative complications.Results All the six cases were operated successfully.None returned to traditional laparoscopy and open operation.The average operative time was 180(range,130-250)min,The average blood loss was 140(range,50-200)ml,and no blood transfusion case was found.the average postoperative hospital stay was 8(range,6-11)days.The drainage tube was removed on the 3rd to 7th postoperative day and the average indwelling time of catheter was 17(range,14-21)days.No one in the urine leakage and 1 case of patients with mild urinary incontinence.Conclusions DaVinci robot-assisted laparoscopic radical prostatectomy is a safe and effective minimal invasive surgical method and applicable to the treatment of prostate cancer with previous TURP,despite of which increased the operation difficulty.
