广州市医疗机构放射工作人员职业健康管理现况调查

Cross-sectional study on the occupational health management for radiation workers in medical institutions in Guangzhou

摘要 目的探讨广州市医疗机构放射工作人员职业健康管理现况及影响因素。方法 2017年7月,采用分层整群抽样方法,对来自省部级、市级、区级、社区和民营机构共528名放射工作人员进行问卷调查。结果 528名调查对象各项职业健康管理水平为41.67%~95.64%。多因素logistic回归分析显示,知晓相应政策是享有放射保健津贴、放射保健休假、知晓单位有健康疗养、定期教育培训、岗前职业健康检查、个人剂量监测和持有《放射工作人员证》的促进因素(OR为2.790~58.366)。除岗前教育培训和在岗期间职业健康检查外,市级医疗机构的放射工作人员职业健康管理水平均高于省部级医疗机构的放射工作人员(OR为2.235~14.542)。放射诊断人员持有《放射工作人员证》、定期进行职业健康检查和知晓单位有健康疗养较其他类别高(OR为2.068~3.857);放射工龄是享有放射保健津贴(OR=1.024)、持有《放射工作人员证》(OR=1.107)以及岗前教育培训(OR=1.092)的促进因素。结论广州市医疗机构放射工作人员职业健康管理发展不平衡,应重点关注放射工作人员权益保障,并加强对省部级医疗机构、非放射诊断类以及工龄短放射工作人员的监管力度。 [Objective]To explore the situation of occupational health management and its influencing factors for radiation workers in medical institutions in Guangzhou city.[Methods]In July 2017,stratified cluster sampling method was used to investigate 528 radiation workers with self-designed questionnaire from provincial and ministerial,municipal,district-level,community and private medical institutions.[Results]There were 528 valid questionnaires.The level of occupational health management was 41.67%-95.64%.Multivariate logistic regression analysis showed that awareness of relevant policies was the promotion factor for radiation health care allowance,radiation health care vacation,awareness that unit had health recuperation,regular training,pre-job occupational health examination,personal dose monitoring and holding the radiation worker certificate(OR was 2.790-58.366).In addition to pre-job training and in-service occupational health examination,the occupational health management level of radiation workers in municipal medical institutions was higher than that in provincial and ministerial medical institutions(OR was 2.235-14.542).The rate was higher among diagnostic radiologists than other types in holding the radiation w orkers certificate,regular occupational health examination and awareness that unit had health recuperati on(OR was 2.068-3.857).Radiation age was the promotion factor for keeping radiation health care allowance(OR=1.024),holding radiation worker certificate(OR=1.107)and pre-job training(OR=1.092).[Conclusion]The development of occupational health management for radiation workers in medical institutions in Guangzhou is unbalance.We should pay more attention to protect the rights and interests of radiation workers,and strengthen the supervision to the provincial and ministerial medical institutions,nonradiological diagnostic categories and short-age radiation workers.
