目的分析肝硬化门静脉血栓(PVT)形成的独立危险因素,并建立实用性强、适用性广的无创预测模型以期为肝硬化PVT形成防治提供参考依据。方法回顾性分析西南医科大学附属医院2018年9月至2020年4月收治的肝硬化符合纳入排除标准者353例,60... 目的分析肝硬化门静脉血栓(PVT)形成的独立危险因素,并建立实用性强、适用性广的无创预测模型以期为肝硬化PVT形成防治提供参考依据。方法回顾性分析西南医科大学附属医院2018年9月至2020年4月收治的肝硬化符合纳入排除标准者353例,60例合并PVT患者为PVT组,随机选择同期120例无PVT患者为对照组,对可能影响PVT形成的因素进行单因素分析及多因素Logistic回归分析,由此建立肝硬化PVT形成的无创预测模型。结果肝炎病毒及酒精是肝硬化的前两位病因,分别占63.89%、16.28%,其中乙型病毒性肝炎占57.78%,是肝硬化的最主要病因。多因素Logistic回归分析显示,血红蛋白(Hb)、D-二聚体、脾静脉内径是PVT形成的独立危险因素(P<0.01)。PVT形成的预测模型为logit P_((PVT))=-2.76+0.34×D_(2)+2.72×脾静脉内径-0.02×Hb。此外,PVT组上消化道出血发生率显著高于对照组(70.00%vs 37.50%,P<0.01),两组余临床表现及并发症发生率差异无统计学意义(P>0.05)。结论乙型病毒性肝炎仍是肝硬化的主要病因,其次为酒精。PVT患者上消化道出血风险明显增加,不利于胃食管静脉曲张出血防治。Hb降低、D-二聚体升高、脾静脉内径增宽是肝硬化门静脉血栓形成的独立危险因素,建议对肝硬化患者动态随访上述指标,必要时行进一步诊治。 展开更多

Objective:To investigate the antibacterial activity of patchouli alcohol(PA)against 127 bacteria strains,including the common bacteria and drug-resistant bacteria strains both in the in vitro and in vivo tests.Methods:For the in vitro trial,the antibacterial property of PA against 107 Gram-positive and 20 Gram-negative bacteria strains was screened by agar double dilution method.For the in vivo trial,specific pathogen free Kunming strain of both male and female white mice,were used to test the protective ability of PA after being injected with the median lethal dose of the tested strains.Results:PA possessed antibacterial activity against all the tested 127 strains.In the in vitro test,PA could inhibit both Gram-negative bacteria(25-768μg/m L)and Gram-positive bacteria(1.5-200μg/m L).Particularly,PA was active against some drug-resistant bacteria like methicillin-resistant Staphylococcus aureus(MRSA).PA also exhibited in vivo anti-MRSA activity in mice via intraperitoneal injection.PA could protect mice entirely infected with MRSA at 100 and 200 mg/kg,while 80% mice injected with MRSA could be protected at a low dose of 50 mg/kg.Conclusion:PA might be a potential antibacterial drug from natural sources and might be worthy to explore its mechanism and application in further study. 展开更多

Background:To examine the association between alcohol drinking and short sleep duration among Chinese adults.Methods:Based on inclusion and exclusion criteria,28,167 records of 15,074 adults longitudinal data were obtained from the China Health and Nutrition Survey for the period from 2004 to 2011.A multilevel logistic regression model was employed to explore the association between alcohol drinking and sleep duration.Results:Compared with nondrinking participants,a high drinking frequency was positively correlated with a short sleep duration in both the males(odds ratio(OR)=1.33,95%confidence interval(CI)=1.20–1.48,P<0.001)and females(OR=1.60,95%CI=1.18–2.18,P=0.003)before the covariates were adjusted.After adjusting for age,residence area,education level,marital status,smoking,coffee consumption,tea consumption,and activity level,this association remained significant in the males(OR=1.31,95%CI=1.17–1.46,P<0.001)but not in the females(OR=1.16,95%CI=0.85–1.59,P=0.340).Conclusion:The present results suggest that a high drinking frequency was positively correlated with a short sleep duration in male population.Comprehensive measures therefore need to be implemented to improve sleep duration,especially to increase awareness of the dangers of drinking alcohol. 展开更多

Non-biodegradable and toxic photocatalysts pose as secondary threats to the environmental security.In this work,non-toxic biocomposites were prepared using natural bamboo cellulose fibers(CF),polyvinyl alcohols(PVAs),BiVO_(4),and carbon dots(CDs)from biomaterials.Thereafter,their optical properties,chemical characterizations,and photocatalytic performances were evaluated.The novel BiVO_(4)(-)CF@CDs(-)PVA composites were prepared via the combined implementation of in situ and impregnation methods.The optical properties revealed that the CDs and fibers increased the range and intensity of light absorption.The PVAs were used as the shield and dispersant to enhance stability because they have numerous chemical groups.In view of the various possible paths of electron migration,the enhanced photocatalytic activity of BiVO_(4)(-)CF@CDs(-)PVA composites for tetracycline degradation was observed under visible light illumination.The origins of the structure,morphology,and optical mechanism in the enhancement of the photocatalytic ability of BiVO_(4)(-)CF@CDs(-)PVA composites were discussed and demonstrated. 展开更多

背景:目前在基础研究方面,股骨头坏死的模型建立进展较缓慢,并且大多数研究并未阐述各种动物模型的优缺点及优选方法。目的:探索目前最适合的股骨头坏死动物造模方法及思路,综述各模型的优缺点,并提出目前所存在的问题及解决方法,为以... 背景:目前在基础研究方面,股骨头坏死的模型建立进展较缓慢,并且大多数研究并未阐述各种动物模型的优缺点及优选方法。目的:探索目前最适合的股骨头坏死动物造模方法及思路,综述各模型的优缺点,并提出目前所存在的问题及解决方法,为以后的疾病研究提供参考及观点依据。方法:应用计算机检索2009至2020年CNKI、PubMed数据库、万方数据知识服务平台,英文检索词“osteonecrosis of the femoral head,animal model,osteoarthritis,collapse of the femoral head,trauma,corticosteroid,alcohol,liquid nitrogen”;中文检索词“股骨头坏死,动物模型,创伤,激素,酒精,液氮”,最终共纳入55篇文献。结果与结论:①股骨头坏死的常见原因包括酒精、激素以及机械创伤;②其中大鼠无呕吐反射中枢,在酒精型造模中较为常用;③激素引起股骨头坏死的机制尚不明确,主要有脂肪代谢紊乱、血管内凝血和骨质疏松3种假说;动物实验中激素造模的发病机制与人类相似,也可以反映后期股骨头坏死的病理变化;④创伤后股骨头发生软骨下骨折、局部塌陷导致血液循环障碍,引起股骨头缺血坏死;但在动物实验中,创伤型动物的发病机制与人类不相同;⑤但是目前各类动物模型中都存在问题:在激素模型的造模中,激素种类的选取与用量未有统一的标准,模型坏死的部位具有不确定性;犬冷冻模型近几年较受欢迎,但缺乏成熟且精确的操作方法,冷冻范围的不确定对造模后期有一定影响;大部分创伤模型与人类的发病机制不同。 展开更多

Exploring highly efficient bifunctional photocatalysts for simultaneous H2 evolution and organic chemical production in pure water represents a green route for sustainable solar energy storage and conversion.Herein,a facile strategy was explored for preparing a hierarchical porous heterostructure of Fe_(4)Ni_(5)S_(8)@ZnIn_(2)S_(4)(FNS@ZIS)by the in situ growth of ZIS nanosheets on Prussian blue analogue(PBA)-derived bimetallic FNS sulfides.A series of FNS@ZIS hierarchical structures were facilely prepared by adjusting the loading amount(n%)of FNS(n=19,26,and 32 for FNS@ZIS-1-3).These structures can efficiently drive the solar co-production of H_(2) and organic chemicals.The optimal co-production was achieved with FNS@ZIS-2,affording a H_(2) evolution rate of 10465μmol·g^(-1)·h^(-1),along with high selectivity for the oxidation of benzyl alcohol to benzaldehyde(>99.9%).The performance was 22 and 31 times higher than that of FNS and ZIS,respectively,and even superior to the state-of-the-art results achieved using various sacrificial agents.Further mechanistic study indicated that the unique hierarchical core/shell architecture can facilitate interfacial charge separation,afford bimetallic synergy,abundant active sites and excellent photostability.This work highlights a simple and efficient method for preparing porous multimetallic hierarchical structures for the solar co-production of organic chemicals and H_(2) fuel. 展开更多

Natural Compound Formula RM88 Significantly Reduces Blood Alcohol Concentration in Humans 认领

Abuse of alcohol causes substantial personal, psychological and medical health issues. In addition, there are significant national economic costs from lost productivity. However, there are limited pharmaceutical drugs for the treatment of alcohol overuse. Historically, many cultures have used herbs and other natural compounds to reduce problematic alcohol induced behaviour but the evidence is anecdotal. This study investigated if a natural compound formula (RM88) that was developed could reduce blood alcohol concentration (BAC) in a controlled case series. Thirteen subjects (5 males, 8 females, age range 18 to 85 years) completed 16 paired sessions of alcohol only versus RM88 with alcohol. Subjects consumed one to three standard drinks of beer, wine or spirits (14.7 to 29.4 gm alcohol). Measurements were made by a fuel cell breathalyzer for a period of 90 minutes. Summated BAC showed a reduction in 94% (15/16) of paired test sessions (BAC reduction range 23% - 79%, mean 50.9% ± 16.5%, p = 0.0005). Data normalized to 20 gm alcohol (two standard drinks) showed a significance of p = 0.00026. One subject on prednisone and hydroxychloroquine drugs had increased BAC from RM88. The average reduction</span><span style="font-family:Verdana;">s</span><span style="font-family:Verdana;"> of BAC for the beverages w</span><span style="font-family:Verdana;">ere</span><span style="font-family:Verdana;"> spirits 34% (n = 3), beer 36% (n = 3), and wine 52% (n = 10). Both the elderly men and women who drink alcohol are at a greater risk of social isolation and economic deprivation and the women in particular are in the danger of rapid development of dependence a phenomenon termed “telescoping” dependence. This study seeks to determine the prevalence of alcohol use disorders as well as their associated factors among the elderly in Jos South LGA, Plateau State, Nigeria. <strong>Methods:</strong> A community-based cross-sectional study of elderly (≥60 years) men and women. A total of 400 out of a total of 422 elderly community dwellers identified were interviewed with the aid of a composite questionnaire incorporating the socio-demography, Alcohol Smoking and Substance Involvement Screening Test (ASSIST), Mini International Neuropsychiatric Interview (MINI) and the 12-item General Health Questionnaire (GHQ-12). <strong>Results:</strong> The lifetime and current prevalence of alcohol use were 69.8% and 45.5% respectively. The prevalence of AUDs was (n = 52, 13.0%). AUDs was significantly associated with being male (OR = 2.57, 95%CI: 1.35 - 3.77, p < 0.01). Participants with AUDs were more likely to report difficulty ambulating (<em>χ</em><sup>2</sup> = 0.34, 0.56), cardiovascular (<em>χ</em><sup>2</sup> = 0.42, p = 0.52), respiratory (<em>χ</em><sup>2</sup> = 0.55, p = 0.46), and digestive (<em>χ</em><sup>2</sup> = 0.17, p = 0.68) problems, though not statistically significant. Similarly, no significant associations were seen with probable psychiatric co-morbidity (<em>χ</em><sup>2</sup> = 2.35, p = 0.13) and chronic pain (<em>χ</em><sup>2</sup> = 0.52, p = 0.47). <strong>Conclusions:</strong> Alcohol use and use disorders were common in this elderly cohort. This study was set out to evaluate the influence of levels of education on the risks of cardiovascular disease in an urban community in Awka, southeast Nigeria. <strong>Methodology:</strong> In this cross sectional study of 294 subjects conducted in Awka in 2017, biodata, anthropometric and demographic data were obtained. A questionnaire was used to extract information on smoking, alcohol use, tobacco snuff use, diabetes mellitus status, time spent at work > 10 hours daily and night sleep duration < 4 hours. Blood pressure and body mass index were measured. Data were analyzed and the association between levels of education and the potential cardiovascular risks determined. <strong>Results: </strong>The prevalence of smoking was 5.4%, tobacco snuff use 5.4%, alcohol 42.2%, thickened arterial wall 27.2%, meat protein 91.8%, diabetes mellitus 9.5%, time spent at work > 10 hours daily 29.3%, obesity 33.3%, hypertension 58.5%, night sleep < 4 hours 5.4%. Levels of education has association with tobacco use (p = 0.009), alcohol use (p = 0.013), thickened arterial wall (p < 0.001), diabetes mellitus (p < 0.001), time spent at work > 10 hours daily (p < 0.001), obesity (p = 0.020), hypertension (p < 0.001), meat consumption (p = 0.047). However, smoking and night sleep < 4 hours have no significant association with levels of education, p = 0.415, and p = 0.132. <strong>Conclusion:</strong> There was variability in the prevalence of cardiovascular risks at different levels of education. The prevalence of smoking and night sleep duration < 4 hours did not vary with levels of education. We reported recently that novel alcohol-based formulations containing plant-derived compounds, including epigallocatechin-3-gallate-palmitate (EGCG-P), a green tea polyphenol ester, provide > 99.99% inactivation of bacterial spores within 60 sec. Based on recently published data from our group and others, we hypothesize that a combination of EGCG-P and alcohol formulated with other plant-derived ingredients would achieve high sporicidal efficacy against a wide spectrum of bacterial spores and can provide novel hand hygiene methods against bacterial spores without toxicity. The objectives of the current study were to optimize </span><span style="font-size:12px;font-family:Verdana;">two novel formulations with combinations of glycerol, citric acid, and EGCG-P</span><span style="font-size:12px;font-family:Verdana;"> to increase sporicidal activity and explore the rapid inactivation mechanisms and suitability for sporicidal products with broad-spectrum activities against aerobic and anaerobic bacterial spores. This study investigates poly(vinyl alcohol) (PVA) membranes as controlled release micro-matrices, which can be useful in therapeutic applications for optimizing the administration of drugs. Currently, the use of hydrogels is limited by protein size. This study investigates the delivery of PspA, a large protein of approximately 38 kD. Pneumococcal surface protein A (PspA) has been shown to provide protective immunity against pneumococcal infection and is considered as a pneumococcal vaccine. The protein release experiments demonstrated that from an initial pH 7.4, approximately 60% of PspA diffuse into a neutral environment with an initial burst and a declining rate reaching equilibrium. The results indicate that the protein was successfully incorporated and released from the membrane over time. The hydrogel and protein interaction is temporary, and the membrane system is ideal for protein drug delivery. The data confirm that the protein did not aggregate and was active after release. The protein release is promising and a step forward to develop microneedles to facilitate high molecular weight protein delivery as well as vaccine delivery. 展开更多
