The Effect of Sensory Stimulation versus Motor Control Cueing of the Foot on Balance in Elderly Females: Single Group Pretest-Posttest Study 预览

Study Design: A Quasi Experimental Single Group Pre-Test Post-Test. Objectives: The aim of this study was to determine the effects of sensory stimulation and motor control cueing of the foot on balance in females over the age of 65. Background: Balance deficits in the elderly population have shown to have detrimental consequences. Evidence suggests that sensory stimulation and motor control of the foot improve postural sway, however, studies have yet to compare the effects of both interventions on balance. Method and Measures: A total of 40 participants were contacted to complete the study with 32 participants meeting inclusion criteria for data collection and completing the research study. Two data collection days were utilized to perform pre and post-test outcome measures using the Functional Reach Test and Overall Sway Index (collected using the Biodex Medical System Inc., Biodex Biosway Portable Balance System). The interventions were 2-minute sensory stimulation to the sole of each foot on day one, and the “short foot” motor control cue on day two. Four related sample T-tests were conducted with the interventions as the dependent variable and the time of observation (pre and post) as the independent variable. Results: The results revealed sensory stimulation significantly improved balance as indicated by a decline in postural sway from pretest (5.92 ± 3.05) to posttest (5.00 ± 2.92) (p = 0.045) and a significant increase in Functional Reach from pretest (8.01 ± 2.55) to posttest (8.70 ± 2.66) (p Conclusion: Based on the results, researchers concluded that sensory stimulation is a viable intervention to utilize in order to improve balance in the elderly population. Further research to identify the appropriate prescription of sensory stimulation to maximize effects should be completed. 展开更多

Ad libitum water consumption prevents exercise-associated hyponatremia and protects against dehydration in soldiers performing a 40-km route-march

Background: It remains unclear if ad libitum water drinking, as a hydration strategy, prevents exercise-associated hyponatremia(EAH) during prolonged exercise. The aim of this study was to determine the incidence of EAH within the broader context of fluid regulation among soldiers performing a 40-km route-march ingesting water ad libitum.Methods: Twenty-eight healthy male soldiers participated in this observational trial. Pre-and post-exercise body mass, blood and urine samples were collected. Blood samples were assessed for serum sodium([Na+]), glucose, creatinine, urea nitrogen(BUN), plasma osmolality, creatine kinase(CK), and plasma arginine vasopressin(AVP) concentrations. Plasma volume(PV) was calculated using hematocrit and hemoglobin. Urine samples were analyzed for osmolality and (Na+)Water intake was assessed by weighing bottles before, during and after the march. The mean relative humidity was 55.7%(21.9–94.3%) and the mean dry bulb temperature was 27.1°C(19.5°C-37.0°C) during the exercise.Results: Twenty-five soldiers(72±10 kg)(Mean±SD) completed the march in 09:11±00:43(hr:min). Participants consumed 736±259 ml/h of water and lost 2.8±0.9 kg(4.0%±1.4%, P<0.05) of body mass. Significant(pre-march vs. post-march;P<0.05) decreases in serum [Na+](141 mmol/L vs. 136 mmol/L), plasma osmolality(303 m Osmol/kg H2O vs. 298 m Osmol/kg H2O), and serum creatinine(111 μmol/L vs. 101 μmol/L) and urine [Na+](168 mmol/L vs. 142 mmol/L), as well as significant increases in plasma AVP(2 pg/ml vs. 11 pg/ml), plasma CK(1423 U/L vs. 3894 U/L) and urine osmolality(1035 m Osmol/kg H2O vs. 1097 m Osmol/kg H2O) were found. The soldier(72 kg) with the lowest postexercise sodium level completed the march in 08:38. He drank 800 ml/h, lost 2% body mass, and demonstrated(prepost) increases in plasma osmolality(294–314 m Osmol/kg H2O), BUN(20–30 mg/dl), AVP(2–16 pg/ml) and PV(41%). This study analyzes the horizontal and vertical migration processes of Cd contents based on the investigations conducted during May and August 1990. It reveals the dynamic balance process of Cd substance in a marine bay and shows that in the waters close to the source input, the content of Cd in the surface waters is the highest, and also the vertical dilution amount is the highest. This is the beginning of the dynamic vertical balance process. Along with distance from the source input, the sedimentation is an ongoing process, which results in a decrease in the content of Cd in the surface waters, and a reduction in the vertical dilution amount. This process continues to the balance point. In waters far away from the source input, the content of Cd is homogeneous in the waters, and the vertical dilution amount decreases to the lowest value, i.e., the balance point. Cross-efficiency evaluation is recognized as an effective way of efficiency assessment for a set of decision making units (DMUs) in the framework of data envelopment analysis (DEA). It has been generally suggested that secondary goals be introduced for cross-efficiency evaluation owing to the non-uniqueness of optimal solutions in self-evaluation. This paper develops a variety of secondary goals in the spirit of promoting balance in the output efficiencies of the DMU under evaluation. The proposed models attempt to make each output contribute as equally as possible to the self-evaluated efficiency. In this way, the weight flexibility can for one thing be reduced by the introduced secondary goals with selections from alternate optimal solutions, in addition to counting on the dilution of flexibility in the subsequent peer-evaluation. The proposed approach might be applicable to evaluation problems in which multiple outputs are considered important and balance is encouraged to put all dimensions into sufficient use. The effectiveness of the proposed approach and its comparisons with some relevant secondary goals are illustrated empirically using numerical examples. 展开更多

Influences of the diameter of the balance hole on the flow characteristics in the hub cavity of the centrifugal pump 预览

The diameter of the impeller balance hole in a centrifugal pump is closely related to axial force.Thus,to select the appropriate diameter of a balance hole is crucial for balancing the axial force of the centrifugal pump.After the validation of the CFD method and the grids independence study,the characteristics of flow in the hub cavity have been analyzed in 4 cases with different diameters of balance holes.The leakage rate of the liquid in the hub cavity of the centrifugal pump is calculated and compared with the test result.According to the research,when no balance hole exists,the distribution of liquid velocity in the core area of the hub cavity exhibits axial symmetry,and the circumferential velocity component is approximately 0.4 times of the impeller's rotational speed.When the balance hole exists,the distribution of liquid velocity in the hub cavity does not exhibit axial symmetry,and the circumferential velocity component is larger than 0.4 times of the impeller's rotational speed.The radial leakage velocity on the turbulent boundary layer of the pump cover will increase with the increasing diameter of the balance hole. 展开更多

审计师群体、群际互动与审计质量——基于客户重要性的经验证据 预览

长期以来,会计师事务所内分化出若干审计师群体的现象普遍存在。基于我国近年来发生的47起事务所合并事件,我们从客户重要性视角考察审计师群际互动对审计质量的影响。我们根据合并前会计师事务所的名称将合并前审计师区分为不同的审计... 长期以来,会计师事务所内分化出若干审计师群体的现象普遍存在。基于我国近年来发生的47起事务所合并事件,我们从客户重要性视角考察审计师群际互动对审计质量的影响。我们根据合并前会计师事务所的名称将合并前审计师区分为不同的审计师群体,再进一步根据合并前审计师的姓名将合并后审计师区分为相应的审计师群体。研究发现,相对于合并前,合并后客户重要性降幅越大,则审计师越倾向于出具严厉审计意见。在区分强势和弱势审计师群体后发现,合并前审计师群体间的平衡性越好,合并后客户重要性的降幅与审计意见严厉程度之间的正相关关系越弱。进一步地,在区分强势审计师群体的地域性和声誉后发现,合并前强势审计师群体的地域性越强以及声誉越差,则合并后客户重要性的降幅与严厉审计意见的可能性越不趋向于正相关。这些实证结果说明,审计质量不仅受到审计组织和审计师个体特征的影响,还取决于审计师群体间的互动。 展开更多

目的探讨基于现实生活的任务导向性训练对学龄期脑瘫儿童平衡和移动能力的影响。方法对湘雅博爱康复医院在2017年9月至2018年9月收治的脑瘫儿童中随机抽取45例采用数字随机法分为两组(实验组22例,对照组23例),对照组行常规康复治疗,实... 目的探讨基于现实生活的任务导向性训练对学龄期脑瘫儿童平衡和移动能力的影响。方法对湘雅博爱康复医院在2017年9月至2018年9月收治的脑瘫儿童中随机抽取45例采用数字随机法分为两组(实验组22例,对照组23例),对照组行常规康复治疗,实验组行常规康复和基于现实生活的任务导向性训练,采用Berg平衡量表(BBS)、粗大运动功能评定量表(GMFM)、儿童残疾评定量表(PEDI)来比较治疗前后(120d)的疗效差异。结果治疗120d后,对照组和实验组BBS、GMFM-E区、PEDI-移动能力部分评分均较治疗前增高(对照组:t值分别为15.906、18.250、23.332,实验组:t值分别为25.592、29.693、37.932,均P<0.001),实验组各项评分指标均优于对照组(F值分别为6.552、1.927、2.227,均P<0.05)。结论基于现实生活的任务导向性训练可有效提高学龄期脑瘫儿童在现实生活中的平衡和移动能力。 展开更多

Cloud dominates influence factors of atmospheric radiation, while aerosol–cloud interactions are of vital importance in its spatiotemporal distribution. In this study, a two-moment(mass and number) cloud microphysics scheme, which significantly improved the treatment of the coupled processes of aerosols and clouds, was incorporated into version 1.1 of the IAP/LASG global Finite-volume Atmospheric Model(FAMIL1.1). For illustrative purposes, the characteristics of the energy balance and cloud radiative forcing(CRF) in an AMIP-type simulation with prescribed aerosols were compared with those in observational/reanalysis data. Even within the constraints of the prescribed aerosol mass, the model simulated global mean energy balance at the top of the atmosphere(TOA) and at the Earth’s surface, as well as their seasonal variation, are in good agreement with the observational data. The maximum deviation terms lie in the surface downwelling longwave radiation and surface latent heat flux, which are 3.5 W m-2(1%) and 3 W m-2(3.5%), individually. The spatial correlations of the annual TOA net radiation flux and the net CRF between simulation and observation were around 0.97 and 0.90, respectively. A major weakness is that FAMIL1.1 predicts more liquid water content and less ice water content over most oceans. Detailed comparisons are presented for a number of regions, with a focus on the Asian monsoon region(AMR). The results indicate that FAMIL1.1 well reproduces the summer–winter contrast for both the geographical distribution of the longwave CRF and shortwave CRF over the AMR. Finally, the model bias and possible solutions, as well as further works to develop FAMIL1.1 are discussed. 展开更多

This study evaluated the effects of dietary fat levels on the growth, nutrient digestibility, nitrogen balance and pelt characteristics of the blue fox(Alopex lagopus). Age-matched blue foxes(12 weeks old) with similar body weights were randomly divided into six dietary groups over 98 days, during the growing-furring period. Blue foxes in the six groups were fed with experimental diets containing 10%(groupⅠ), 12%(groupⅡ), 14%(group Ⅲ), 16%(group Ⅳ), 18%(groupⅤ) and 20%(group Ⅵ) fat during the growing period(from 13 weeks to 18 weeks of age);foxes were fed with diets containing 12%(groupⅠ), 14%(groupⅡ), 16%(group Ⅲ), 18%(group Ⅳ), 20%(groupⅤ) and 22%(group Ⅵ) fat during the furring period(from 18 weeks to 26 weeks of age). The results indicated that 16%-18% dietary fat, during the growing period significantly improved nutrient utilization, and as a main energy source, significantly reduced the feed/gain ratio. A dietary fat level of 18%, during the furring period not only improved the growth and development and decreased the feed/gain ratio, but also increased nutrient digestibility and fur quality. These data, combined with data from a conic regression model, showed that dietary fat levels between 16.29% and 18.40%, during the growing period and between 17.72% and 18.72%, during the furring period enhanced the growth performance and fur quality of blue foxes. 展开更多

Methods: A total of 42 patients with secretory otitis media treated in our hospital from September 2016 to January 2018 were selected as the observation group and 42 healthy people as the control group. The levels of proinflammatory effect [including transforming growth factor beta 1 (TGF-β1), transforming growth factor-beta 2 (TGF-β2)], fluid balance [including aquaporin-1 (AQP-1) and aquaporin-4 (AQP-4)], vascular permeability [including hyaluronic acid (HA), fibronectin (Fn) and platelet activating factor (PAF)] and oxidative stress-related indexes [including malondialdehyde (MDA) and superoxide dismutase (SOD)] were observed and compared between the two groups. Results: The levels of AQP-1, AQP-4 and Fn in the observation group were significantly lower than those in the control group. The expression levels were (4.52±0.39) g/L, (23.06±7.21) g/L and (120.59±13.07) mg/L, respectively. The levels of TGF-β1, TGF-β2, HA, PAF, MDA and SOD were significantly higher than those of the control group and the expression levels were (11.99±4.23) 毺g/L, (4.22±1.66) 毺g/L, (70.54±6.99) 毺g/L, (123.83±20.58) ng/mL, (6.30±0.44) nmol/mL and (15.85±0.78) NU/mL, respectively. The difference was statistically significant. Conclusions: Inflammatory reaction occurs in patients with otitis media with secretory otitis media. It is easy to break the liquid balance in the middle ear cavity and to increase vascular permeability and oxidative stress. The relevant indicators should be strengthened in clinical practice, so as to provide evidence for early diagnosis and treatment of the disease. 展开更多

Hydrothermal precipitates associated with active vents in the eastern Manus Basin, an actively opening back-arc basin in the Bismarck Sea, Papua New Guinea, are among the most Cu-rich on the modern seafloor. The volcanic rocks associated with this mineralization may be insufficiently enriched in Cu to account for the Cu content of the sulfides by simple leaching. The PACMANUS hydrothermal field lies in the eastern portion of the eastern Manus Basin. Mass balance modeling of the PACMANUS hydrothermal system indicates that simple leaching of a stationary reaction zone (0.144 km^3) by hydrothermal fluids cannot yield the Cu found in associated sulfide deposits because unacceptably high leaching, transportation and precipitation efficiencies are required to derive the Cu in sulfides by leaching processes. With 100% leaching, transport and precipitating efficiency, 0.166 km^3 of volcanic rocks would need to be leached to account for the Cu budget of hydrothermal sulfide deposits. The key requirement for forming metal-rich magmatic fluids is a large amount of metals available to enter the exsolved vapor phase. Magmas generated in the eastern Manus Basin inherently have high fO2 because of metasomatism of the mantle source by oxidized materials from the subducted slab, leading to copper enrichment in the magma chamber. Moreover, the presence of Cu in gas-rich melt inclusi on bubbles in Pual Ridge andesite is evidence that degassing and partitioning of Cu into the magmatic volatile phase has occurred in the eastern Manus Basin. Numerical mass balance modeling indicates that approximately 0.236 Mt Cu was potentially transferred to the hydrothermal system per cubic kilometer magma. 目的探讨躯干核心肌群训练的时间强度对老年脑卒中患者平衡能力的疗效。方法采用病例对照的研究方法,选取2013年4月~2013年10月在华中科技大学同济医学院附属协和医院康复医学科住院治疗的45例老年脑卒中所致偏瘫患者,按照随机数表法随... 目的探讨躯干核心肌群训练的时间强度对老年脑卒中患者平衡能力的疗效。方法采用病例对照的研究方法,选取2013年4月~2013年10月在华中科技大学同济医学院附属协和医院康复医学科住院治疗的45例老年脑卒中所致偏瘫患者,按照随机数表法随机分为A、B、C3组,每组15例,均给予常规治疗,并在此基础上联合躯干核心肌群训练,A组1次/d,B组2次/d,C组3次/d,每次30min,每周5d,共训练3个月。在治疗前及治疗后1、2和3个月时分别采用Berg平衡量表对3组患者平衡功能进行评定。结果治疗前3组患者平衡能力评分比较,差异无统计学意义(P>0.05);治疗1、2和3个月后,3组患者的平衡量表评分都明显提高,组内比较差异均有统计学意义(P<0.05);1和2个月后组间比较,平衡能力评分C组高于B组,B组高于A组,差异均有统计学意义(P<0.05),但3个月后,3组患者平衡能力评分,组间比较差异均无统计学意义(P>0.05)。结论增加治疗剂量,提高躯干核心肌群强化训练的时间强度可以加快老年脑卒中患者早期平衡功能的恢复,但随着治疗时间延长,远期疗效并无差异。 展开更多

运用泰尔指数法和综合评价法,分析了新医改以来上海市不同层级医疗资源空间配置均衡性及变化状况。结果表明上海市不同层级各类医疗资源持续增长,医疗资源的供给更加充分。总体上,医疗资源的配置依然呈现“中心城区强、郊区弱”的不均... 运用泰尔指数法和综合评价法,分析了新医改以来上海市不同层级医疗资源空间配置均衡性及变化状况。结果表明上海市不同层级各类医疗资源持续增长,医疗资源的供给更加充分。总体上,医疗资源的配置依然呈现“中心城区强、郊区弱”的不均衡状况,其中社区层面医疗资源配置的均衡性要高于医院层面,但由于社区层面总体上占有的医疗资源非常有限,所以该均衡处于低水平状态。基于上述分析,为进一步提高上海市不同层级医疗资源的配置均衡性,要适度的对医院有所规束,加大向基层倾斜力度,加快推进医疗资源在层级分布的均衡性;限制医备竞赛,适度增加人力资源供给;通过机制优化和技术创新,促进医疗资源的高效利用和流转。 展开更多
