血清胃蛋白酶原检测在慢性浅表性胃炎诊断中的临床意义 预览

目的:探讨血清胃蛋白酶原(Pepsinogen,PG)Ⅰ、Ⅱ测定在慢性浅表性胃炎诊断中的临床意义。方法:选取经胃镜诊断为慢性浅表性胃炎患者62例作为试验组,对照组选取同期健康体检者62例;采用ELISA法检测试验组和对照组血清PGⅠ、Ⅱ水平。采用S... 目的:探讨血清胃蛋白酶原(Pepsinogen,PG)Ⅰ、Ⅱ测定在慢性浅表性胃炎诊断中的临床意义。方法:选取经胃镜诊断为慢性浅表性胃炎患者62例作为试验组,对照组选取同期健康体检者62例;采用ELISA法检测试验组和对照组血清PGⅠ、Ⅱ水平。采用SPSS18.0统计软件对数据进行分析。结果:试验组PGⅠ、Ⅱ水平分别为(109.7±22.8)μg/L、(16.7±4.3)μg/L,对照组分别为(94.7±18.4)μg/L、(14.9±3.8)μg/L。试验组胃蛋白酶原Ⅰ水平高于健康对照组,差异有统计学意义(P<0.05),而PG-Ⅱ的水平和PG-I与PG-Ⅱ比值差异无统计学意义(P>0.05);幽门螺杆菌(Hpylori,Hp)抗体阳性患者PG-I与PG-Ⅱ水平高于阴性患者,而PG-I/PG-Ⅱ则低于阴性患者;慢性浅表性胃炎合并溃疡患者PG-I与PG-Ⅱ水平显著高于单纯浅表性胃炎,差异有统计学意义(P<0.05)。结论:血清胃蛋白酶原Ⅰ、Ⅱ检测对慢性浅表性胃炎有一定的辅助诊断价值。 展开更多

Awareness of suspended sediment load (SSL) and its continuous monitoring plays an important role in soil erosion studies and watershed management.Despite the common use of the conventional model of the sediment rating curve (SRC) and the methods proposed to correct it,the results of this model are still not sufficiently accurate.In this study,in order to increase the efficiency of SRC model,a multi-objective optimization approach is proposed using the Non-dominated Sorting Genetic Algorithm Ⅱ (NSGA-Ⅱ) algorithm.The instantaneous flow discharge and SSL data from the Ramian hydrometric station on the Ghorichay River,Iran are used as a case study.In the first part of the study,using self-organizing map (SOM),an unsupervised artificial neural network,the data were clustered and classified as two homogeneous groups as 70% and 30% for use in calibration and evaluation of SRC models,respectively.In the second part of the study,two different groups of SRC model comprised of conventional SRC models and optimized models (single and multi-objective optimization algorithms) were extracted from calibration data set and their performance was evaluated.The comparative analysis of the results revealed that the optimal SRC model achieved through NSGA-Ⅱ algorithm was superior to the SRC models in the daily SSL estimation for the data used in this study.Given that the use of the SRC model is common,the proposed model in this study can increase the efficiency of this regression model. 展开更多

AngiotensinⅡ(All) has been well known to induce cardiomyocyte hypertrophy. Epigallocatechin-3-gallate (EGCG) is the main active comp on ent of green tea and it has bee n show n to exhibit strong cardioprotective potential, although the underlying molecular mechanisms remain unclear. In this study, we investigated the role and mechanism of EGCG in preventing All-induced cardiomyocyte hypertrophy using rat H9c2 cardiomyocytes cells. Reactive oxygen species assay, cell size, and mRNA expression of cardiac hypertrophy markers ANP and BNP were assessed in response to All treatment. In addition, expression of proteins involved in Hippo signaling pathway were determined by western blot analysis. We found that All treatment resulted in significant upregulation of ANP and BNP expression levels and increase in H9c2 cell size, which were markedly attenuated by EGCG treatment. Furthermore, our results suggested that EGCG inhibited Allinduced cardiac hypertrophy via regulating the Hippo signaling pathway. Therefore, EGCG may be an effective agent for preventing cardiac hypertrophy. 展开更多

BACKGROUND Severe acute pancreatitis(SAP)is a common condition in the intensive care unit(ICU)and has a high mortality.Early evaluation of the severity and prognosis is very important for SAP therapy.Recently,red blood cell distribution(RDW)was associated with mortality of sepsis patients and could be used as a predictor of prognosis.Similarly,RDW may be associated with the prognosis of SAP patients and be used as a prognostic indicator for SAP patients.AIM To investigate the prognostic value of RDW for SAP patients.METHODS We retrospectively enrolled SAP patients admitted to the ICU of the First Affiliated Hospital of China Medical University from June 2015 to June 2017.According to the prognosis at 90 d,SAP patients were divided into a survival group and a non-survival group.RDW was extracted from a routine blood test.Demographic parameters and RDW were recorded and compared between the two groups.The receiver operator characteristic(ROC)curve was constructed and Cox regression analysis was performed to investigate the prognostic value of RDW for SAP patients.RESULTS In this retrospective cohort study,42 SAP patients were enrolled,of whom 22 survived(survival group)and 20 died(non-survival group).The baseline parameters were comparable between the two groups.The coefficient of variation of RDW(RDW-CV),standard deviation of RDW(RDW-SD),Acute Physiology and Chronic Health Evaluation II(APACHE II)score,and Sequential Organ Failure Assessment(SOFA)score were significantly higher in the non-survival group than in the survival group(P<0.05).The RDW-CV and RDW-SD were significantly correlated with the APACHE II score and SOFA score,respectively.The areas under the ROC curves(AUCs)of RDW-CV and RDW-SD were all greater than those of the APACHE II score and SOFA score,among which,the AUC of RDW-SD was the greatest.The results demonstrated that RDW had better prognostic value for predicting the mortality of SAP patients.When the RDW-SD was greater than 45.5,the sensitivity for predicting prognosis was 77.8%and the specifi 展开更多

Increased serum urotensin Ⅱ(UⅡ) levels in human cirrhotic populations have been recently shown,but the long-term effects of UⅡ receptor antagonist on the cirrhosis have not been investigated.To investigate the therapeutic effects of urotensin Ⅱ receptor (UT) antagonist palosuran on rats with carbon tetrachloride (CCI4)- induced cirrhosis,the hepatic and systemic hemodynamics,liver fibrosis,the metalloproteinase-13 (MMP-13)/ tissue inhibitor of metalloproteinase-1 (TIMP-1) ratio,hepatic Rho-kinase activity,and the endothelial nitric oxide synthase (eNOS) activity are measured in CCI4-cirrhotic rats treated with palosuran or vehicle for 4 weeks.Primary hepatic stellate cells (HSCs) are used to investigate the changes in UⅡ/UT expression and the in vitro effect of palosuran.Compared with the vehicle-treated cirrhotic rats,treatment with palosuran can reduce the portal pressure (PP),decrease the risk of liver fibrosis and the level of a smooth muscle actin,collagen-l (COL-I),and transforming growth factor β expression.However,treatment with palosuran can increase MMP-13/TIMP-1,pvasodilator- stimulated phosphoprotein (p-VASP),and p-eNOS expression.Moreover,in vitro UⅡ/UT mRNA expression increases during HSC activation.MMP-13/TIMP-1,COL-I,and p-VASP are inhibited after palosuran treatment Our data indicate that long-term administration of palosuran can decrease PP in cirrhosis,which results from decreased hepatic fibrosis and enhanced eNOS-dependent HSC vasodilatation. 展开更多

Objective Silicosis, caused by inhalation of silica dust, is the most serious occupational disease in China and the aim of present study was to explore the protective effect of Ang (1-7) on silicotic fibrosis and myofibroblast differentiation induced by Ang Ⅱ. Methods HOPE-MED 8050 exposure control apparatus was used to establish the rat silicosis model. Pathological changes and collagen deposition of the lung tissue were examined by H.E. and VG staining, respectively. The localizations of ACE2 and a-smooth muscle actin (a-SMA) in the lung were detected by immunohistochemistry. Expression levels of collagen type Ⅰ, a-SMA, ACE2, and Mas in the lung tissue and fibroblasts were examined by western blot. Levels of ACE2, Ang (1-7), and Ang Ⅱ in serum were determined by ELISA. Co-localization of ACE2 and a-SMA in fibroblasts was detected by immunofluorescenee. Results Ang (1-7) induced pathological changes and enhanced collagen deposition in vivo. Ang (1-7) decreased the expressions of collagen type Ⅰ and a-SMA and increased the expressions of ACE2 and Mas in the silicotic rat lung tissue and fibroblasts stimulated by Ang Ⅱ. Ang (1-7) in creased the levels of ACE2 and Ang (1-7) and decreased the level of Ang Ⅱ in silicotic rat serum. A779 enhanced the protective effect of Ang (1-7) in fibroblasts stimulated by Ang Ⅱ. Conclusion Ang (1-7) exerted protective effect on silicotic fibrosis and myofibroblast differentiation induced by Ang Ⅱ by regulating ACE2-Ang (l-7)-Mas axis. 展开更多

Emodin has been used as a natural medicine against several chronic diseases. The objective of this study is to determine the effects of emodin on cardiac fibrosis and the underlying molecular mechanisms. Intragastric administration of emodin markedly decreased left ventricular wall thickness in a mouse model of pathological cardiac hypertrophy with excess fibrosis induced by transaortic constriction(TAC) and suppressed activation of cardiac fibroblasts induced by angiotensin II(AngII). Emodin upregulated expression of metastasis associated protein 3(MTA3) and restored the MTA3 expression in the setting of cardiac fibrosis. Moreover, overexpression of MTA3 promoted cardiac fibrosis;in contrast, silence of MTA3 abrogated the inhibitory effect of emodin on fibroblast activation. Our findings unraveled the potential of emodin to alleviate cardiac fibrosis via upregulating MTA3 and highlight the regulatory role of MTA3 in the development of cardiac fibrosis. 展开更多

Clinical trials and animal experimental studies have demonstrated an association of arterial baroreflex impairment with the prognosis and mortality of cardiovascular diseases and diabetes. As a primary part of the arterial baroreflex arc, the pressure sensitivity of arterial baroreceptors is blunted and involved in arterial baroreflex dysfunction in cardiovascular diseases and diabetes.Changes in the arterial vascular walls, mechanosensitive ion channels, and voltage-gated ion channels contribute to the attenuation of arterial baroreceptor sensitivity. Some endogenous substances(such as angiotensin II and superoxide anion) can modulate these morphological and functional alterations through intracellular signaling pathways in impaired arterial baroreceptors. Arterial baroreceptors can be considered as a potential therapeutic target to improve the prognosis of patients with cardiovascular diseases and diabetes. 展开更多

BACKGROUND Endoscopic retrograde cholangiopancreatography(ERCP)in patients with Billroth Ⅱ gastrectomy has been considered a challenging procedure due to the surgically altered gastrointestinal anatomy.However,there has been a paucity of comparative studies regarding ERCP in Billroth Ⅱ gastrectomy cases because of procedure-related morbidity and mortality and practical and ethical limitations.This systematic and comprehensive review was performed to obtain a recent perspective on ERCP in Billroth Ⅱ gastrectomy patients.AIM To systematically review the literature regarding ERCP in Billroth Ⅱ gastrectomy patients.METHODS A systematic review was performed on the literature published between May 1975 and January 2019.The following electronic databases were searched:PubMed,EMBASE,and Cochrane Library.The outcomes of successful afferent loop intubation and successful selective cannulation and occurrence of adverse events were assessed.RESULTS A total of 43 studies involving 2669 patients were included.The study designs were 36(83.7%)retrospective cohort studies,4(9.3%)retrospective comparative studies,2(4.7%)prospective comparative studies,and 1(2.3%)prospective cohort study.Of a total of 2669 patients,there were 1432 cases(55.6%)of sideviewing endoscopy,664(25.8%)cases of forward-viewing endoscopy,171(6.6%)cases of balloon-assisted enteroscopy,169(6.6%)cases of anterior obliqueviewing endoscopy,64(2.5%)cases of dual-lumen endoscopy,31(1.2%)cases of colonoscopy,and 14(0.5%)cases of multiple bending endoscopy.The overall success rate of afferent loop intubation was 91.3%(2437/2669),and the overall success rate of selective cannulation was 87.9%(2346/2437).A total of 195 cases(7.3%)of adverse events occurred.The success rates of afferent loop intubation and the selective cannulation rate for each type of endoscopy were as follows:side-viewing endoscopy 98.2%and 95.3%;forward-viewing endoscopy 97.4%and 95.2%;balloon-assisted enteroscopy 95.4%and 97.5%;oblique-viewing endoscopy 94.1%and 97.5%;and dual-lumen endosco 展开更多

Effects of Tanshinone Combined with Western Medicine on Clinical Symptoms and Cardiac Function in Patients with Acute Heart Failure 预览

OBJECTIVE:To explore the effects of Tanshinone combined with western medicine on clinical symptoms and cardiac function in patients with acute heart failure. METHODS:The medical records of 96 patients with acute heart failure were randomly divided into 2 groups (48 cases in observation group and 48 cases in control group). The control group was treated with conventional western medicine, and the observation group was treated with Tanshinone IIA sulfonic acid natrium on the basis of western medicine. The traditional Chinese medicine (TCM) syndromes scores before and after treatment (after 28 d of treatment), clinical efficacy, and cardiac echocardiographic indexes and serum biochemical indicators before and after treatment were observed in the 2 groups. RESULTS:After treatment, the scores of TCM syndromes in the 2 groups were significantly decreased (P<0.05), and the change in observation group was significantly greater than that in control group (P<0.05). The total clinical effective rate in observation group was significantly higher than that in control group (91.67% vs 75.00%)(P<0.05). After treatment, the echocardiographic indexes of left ventricular end-diastolic diameter (LVEDD) and left ventricular end-systolic diameter (LVESD) values were significantly reduced in the 2 groups while the left ventricular ejection fraction (LVEF) and stroke volume (SV) value were significantly increased, and the changes in observation group were significantly larger than those in control group (P<0.05). After treatment, the levels of serum nuclear factor-kappaB (NF-KB), interleukin-1β(IL-1β) and N-terminal pro-brain natriuretic peptide (NT-proBNP) were significantly decreased in the 2 groups (P<0.05), while the 6-keto-prostaglandin F1α(6-keto- PGF1α) level was significantly increased. And the changes in observation group were significantly greater than those in control group (P<0.05). CONCLUSION:Tanshinone combined with western medicine can significantly improve the clinical symptoms of patients with acute heart failure, 展开更多

Caspase-11 is a key upstream modulator for activation of inflammatory response under pathological conditions. In this study, we investigated the roles of caspase-11 in the maturation of interleukin-1β (IL-1β) and development of renal interstitial fibrosis in vivo and in vitro. Mice were subjected to unilateral ureteral obstruction (UUO). The mice were treated with either caspase-11 inhibitor wedelolactone (Wed, 30 mg/kg/day, ig) for 7 days or caspase-11 siRNA (10 nmol/20 g body weight per day, iv) for 14 days. The mice were euthanized on day 14, their renal tissue and blood sample were collected. We found that the obstructed kidney had significantly higher caspase-11 levels and obvious tubular injury and interstitial fibrosis. Treatment with Wed or caspase-11 siRNA significantly mitigated renal fibrosis in UUO mice, evidenced by the improved histological changes. Furthermore, caspase-11 inhibition significantly blunted caspase-1 activation, IL-ip maturation, transforming growth factor-p (TGF-P), fibronectin, and collagen I expressions in the obstructed kidney. Renal tubular epithelial NRK-52E cells were treated in vitro with angiotensin (Ang, 1 pmol/L), which stimulated caspase-11 activation and IL-ip maturation. Treatment with IL-1β (20ng/ml) significantly increased the expression of TGF-β, fibronectin, and collagen I in the cells. Ang ll-induced expression of TGF-β, fibronectin, and collagen I were suppressed by caspase-11 siRNA or Wed. Finally, we revealed using co-immunoprecipitation that caspase-11 was able to interact with caspase-1 in NRK-52E cells. These results suggest that caspase-11 is involved in UUO-induced renal fibrosis. Elevation of caspase-11 in the obstructed kidney promotes renal fibrosis by stimulating caspase-1 activation and IL-1β maturation. 展开更多

BACKGROUND Obesity has been associated with hepatic overexpression of the renin-angiotensin system (RAS). AIM To evaluate the action of two angiotensin Ⅱ (ANGⅡ) receptor blockers (losartan or telmisartan) on the modulation of local hepatic RAS and the resulting metabolic effects in a diet-induced obesity murine model. METHODS Twenty C57BL/6 mice were randomly divided into two nutritional groups for 10 wk: control group (C, n = 5, 10% of energy as fat) or high-fat group (HF, n = 15, 50% of energy as fat). After treatment started, the HF group was randomly divided into three groups: untreated HF group (n = 5), HF treated with losartan (HFL, n = 5) and HF treated with telmisartan (HFT, n = 5). The treatments lasted for 5 wk, and the dose was 10 mg/kg body mass. RESULTS HF diet induced body mass gain (+28%, P < 0.0001), insulin resistance (+69%, P = 0.0079), high hepatic triacylglycerol (+127%, P = 0.0004), and overexpression of intrahepatic angiotensin-converting enzyme (ACE) 1/ ANGII type 1 receptor (AT1r)(+569.02% and +141.40%, respectively, P < 0.0001). The HFL and HFT groups showed higher ACE2/rMAS gene expression compared to the HF group (ACE2:+465.57%, P = 0.0002 for HFL and +345.17%, P = 0.0049 for HFT;rMAS:+711.39%, P < 0.0001 for HFL and +539.75%, P < 0.0001 for HFT), followed by reduced insulin/glucose ratio (-30% for HFL and -33% for HFT, P = 0.0181), hepatic triacylglycerol levels (-28%, P = 0.0381 for HFL;and -45%, P = 0.0010 for HFT, and Plin2 expression. CONCLUSION Modulation of the intrahepatic RAS, with favored involvement of the ACE2/rMAS axis over the ACE1/AT1r axis after losartan or telmisartan treatments, caused hepatic and metabolic beneficial effects as demonstrated by reduced hepatic triacylglycerol levels coupled with reduced PLIN 2 expression and improved glycemic control. 展开更多

As one of the highly effective methods to prepare catalysts for photocatalytic reduction of CO2 into valueadded chemicals,using metalloporphyrin as light-harvesting mixed ligand to modify metal-organic framework(MOF)is very valuable since it can greatly improve the prophyrin dispersibility and consequently inhibit its potential agglomeration.Herein,we employed a one-pot synthetic strategy to chemically immobilize Cu(II)tetra(4-carboxylphenyl)porphyrin(CuTCPP)into UiO-66 MOF structure through coordination mode.Meanwhile,in-situ growth of TiO2 nanoparticles onto the MOF is actualized with the generation of CuTCPP c UiO-66/TiO2(CTU/TiO2)composites.Under Xe lamp irradiation(λ>300 nm),the catalytic result presents that an optimal value of 31.32 μmol g^-1 h^-1 CO evolution amount,about 7 times higher than that of pure TiO2 was obtained through the photocatalysis.It is supposed owning to a consistent augment of light absorption derived from chemically implanted porphyrin derivative,which is simultaneously functioning with an efficacious separation of photo-induced carries given by the newly engendered composites between MOF and TiO2,an effective catalytic activity and approving recyclability of CTU/TiO2 can be achieved in the photocatalytic reduction of CO2 into CO. 展开更多
