Heating frequency optimization for artificial feld-aligned scattering

In this study, we present a simulation study of artificial field-aligned irregularities(AFAI) to calculate the scattering coefficient considering a Gaussian autocorrelation function for the wave number spectrum of the density fluctuation. By analyzing variations in the scattering coefficient under different ionospheric backgrounds, the optimal range of the heating frequency was found,which is about 0.9–1 times the critical frequency of the F2 layer. This is especially noticeable as when the heating frequency varies from 0.5 times to 0.9 times of the critical frequency, the scattering coefficient increases by 6.8–16.2 d B. These results should be useful for optimizing the heating frequency in the future artificial field-aligned scattering(AFAS) transmission applications at middle and low latitudes. 展开更多

Submersible electrical motor direct-drive progressing cavity pump (PCP) rodless lifting was studied to solve the traditional rod-drive pump problems, such as rod-tubing wearing, low efficiency and short running time. The theoretical researches and laboratory experiments of key tools such as submersible motor and the construction technology of lifting system were introduced. The field application and economic benefit were analyzed and compared with the traditional rod pumping unit. A new low speed and large torque permanent magnet synchronous motor was developed. This motor was used to drive PCP without gear reducer, which improved the reliability and feasibility. It can run at the speed from 50 to 500 r/min with stepless speed regulation, and it can perform high efficiency and large torque. Besides, other key supporting tools, such as motor protector and flex shaft, were developed. The submersible electrical motor direct- drive PCP technology can be used in a 139.7 mm (5.5 in) casing well, with daily output ranging from 5 to 50 m3. Until now, the technology has been deployed more than 100 wells. The field application results show that it eliminates the rod-tubing wearing and saves electric energy by more than 30% compared with the traditional rod pumping unit. And it also makes the oil produced in a safe and environmental friendly way. 展开更多

目的探讨三通接头在人工气道气囊压力监测中的应用.方法采用便利抽样法,选择2016年8月-2017年7月入住沈阳市第四人民医院重症医学科建立人工气道进行机械通气的患者85例,均采用最小压力封闭技术向气囊注气封闭气道.随机分成两组.观察组(... 目的探讨三通接头在人工气道气囊压力监测中的应用.方法采用便利抽样法,选择2016年8月-2017年7月入住沈阳市第四人民医院重症医学科建立人工气道进行机械通气的患者85例,均采用最小压力封闭技术向气囊注气封闭气道.随机分成两组.观察组(n=43)人工气道气囊连接三通接头,再连接气囊压力表监测气囊压力;对照组(n=42)人工气道直接连接气囊压力表监测气囊压力.监测两组患者充气后4、8、24、48 h气囊压力变化△P(△P=最小封闭压力-监测压力),观察患者返流误吸、气道黏膜损伤、肺部感染发生情况.结果两组充气4 h气囊压力变化差异无统计学意义[△P 4 h:(3.76±1.47)vs.(3.82±1.36)cmH2O,P>0.05],充气8、24、48 h观察组气囊压力变化较对照组小,差异均有统计学意义[△P 8 h:(5.76±2.58)vs.(7.29±2.42)cmH2O,24 h:(6.32±3.08)vs.(9.38±3.32)cmH2O,48 h:(8.79±5.26)vs.(15.32±5.87)cmH2O;P<0.01).观察组患者返流误吸发生率为9.3%,低于对照组的11.9%,差异有统计学意义(P<0.05);气道黏膜损伤及肺部感染发生率两组比较差异无统计学意义(P>0.05).结论使用三通接头可以有效减少人工气道气囊漏气现象,减少返流误吸的发生率. 展开更多

Objective:To explore the best time for progesterone supplementation in AID ovulation induction cycles by Letrozole.Methods: The data analysed in this study were collected from 509 patients who were performed AID (Artificial Insemination by Donor) administrated letrozole (LE) between 2014.8-2015.7. All patients were randomly divided into 4 groups by the time of progesterone administrated, including experimental group and the control group. The experimental group was divided into group 1-72 h after ovulation, group 2-48 h after ovulation, group 3-24 h after ovulation and control group—without administrated LE. The gestation and live birth rate were evaluated by monitoring vaginal ultrasound and HCG blood value 14 d after AID.Results: The pregnancy rate with administrated progesterone added 72 h after ovulation was 31.9%, which was significantly higher than those of other groups, the same situation as groups added progesterone was significantly higher than the control group. However, there was no significant difference in the numbers of abortions among the four groups. The LBR of group 4 was significantly lower than that of group 1.Conclutions: Progesterone administrated 72 h after ovulation can promoted the gestation rate, but did not affect the rate of miscarrage . 展开更多

经右侧腋静脉途径左束支区域起搏单中心经验报道

目的探讨经右侧腋静脉途径行左束支区域起搏的经验及安全性。方法回顾总结2018年4月至2018年8月在上海新华医院经右侧腋静脉行永久性左束支区域起搏患者的腔内标测电图、起搏程控及临床随访资料。结果共15例患者入选,其中二度Ⅱ型房室... 目的探讨经右侧腋静脉途径行左束支区域起搏的经验及安全性。方法回顾总结2018年4月至2018年8月在上海新华医院经右侧腋静脉行永久性左束支区域起搏患者的腔内标测电图、起搏程控及临床随访资料。结果共15例患者入选,其中二度Ⅱ型房室传导阻滞5例,三度房室传导阻滞6例,窦性停搏1例,窦性心动过缓3例。所有患者均同意经右侧腋静脉行左束支区域起搏术,术中单极起搏有2例未达到左束支区域起搏标准,其余13例达到标准,且参数稳定可靠,术中即刻成功率86.7%(13/15)。手术时间(133.0±32.9)min。术后7 d及1个月随访,13例成功患者起搏后继续保持右束支阻滞图形,参数稳定,无1例脱位。术前、术后1个月,左心室射血分数(LVEF)分别为63.9%±4.9%、60.2%±7.1%,左心室舒张末期内径(LVEDD)分别为(50.7±4.9)mm、(51.7±5.8)mm,前后相比差异无统计学意义(P>0.05)。QRS时限分别为,起搏前(92.8±18.6)ms、单极起搏后(112.7±26.2)ms,前后相比明显增加差异有统计学意义(P<0.05),但仍低于120 ms。结论经右侧腋静脉途径行左束支区域起搏安全、可行,随访期间对LVEF无影响。 展开更多

目的比较左束支区域起搏与右心室流入道间隔部起搏的电学参数与短期临床效果。方法入选北京安贞医院2017年4月至2018年9月符合心血管植入型电子器械治疗适应证患者60例,分成两组,各30例,均用3830导线,分别植入左束支区域与右心室流入道... 目的比较左束支区域起搏与右心室流入道间隔部起搏的电学参数与短期临床效果。方法入选北京安贞医院2017年4月至2018年9月符合心血管植入型电子器械治疗适应证患者60例,分成两组,各30例,均用3830导线,分别植入左束支区域与右心室流入道间隔部。测量、随访与比较两组患者的起搏导线参数、心功能指标、心电参数及不良事件。结果比较两组术中植入电学参数差异无统计学意义。短期随访起搏阈值左束支区域起搏者略低[(0.7±0.3)mV对(0.9±0.4)mV,P=0.021],R波感知[(11.2±6.1)mV对(9.4±5.4)mV,P=0.255],阻抗[(724.0±134.9)Ω对(698.5±147.0)Ω,P=0.485],左心室射血分数[56.3%±9.8%对55.7%±9.1%,(P=0.796)]、左心室舒张末期内径[(49.8±8.0)mm对(50.6±6.5)mm,P=0.685)、二尖瓣反流面积减少程度[(0.1±1.1)cm2对(-0.3±1.0)cm2,P=0.202]、QRS时限[(127.4±32.4)ms对(138.3±33.9)ms,P=0.209]、QT间期[(461.7±38.5)ms对(463.2±35.0)ms,P=0.875]、T波同向性[(9.9±2.0/12)对(9.5±1.8/12),P=0.449]、Tp-e间期[(109.5±33.3)ms对(109.7±19.0)ms,P=0.974]两组差异无统计学意义。结论左束支区域起搏与右心室流入道间隔部起搏比较,术中与短期随访电学参数和临床效果未见显著优势,长期效果有待更多研究。 展开更多

强化培训和督导干预对预防ICU呼吸机相关性肺炎的临床研究 预览

目的探索强化培训和督导干预对预防重症监护病房(ICU)呼吸机相关性肺炎(VAP)的临床应用效果。方法选择2017年3月至2018年3月入住驻马店市第一人民医院ICU经口气管插管的机械通气患者241例作为研究对象;将2017年3-8月进行呼吸机的常规治... 目的探索强化培训和督导干预对预防重症监护病房(ICU)呼吸机相关性肺炎(VAP)的临床应用效果。方法选择2017年3月至2018年3月入住驻马店市第一人民医院ICU经口气管插管的机械通气患者241例作为研究对象;将2017年3-8月进行呼吸机的常规治疗和护理的患者作为对照组(119例),将2017年9月至2018年3月被强化培训和督导后的工作人员进行治疗和护理的患者作为观察组(122例);开展VAP预防与控制措施的依从性监测(相关知识知晓率、手卫生、口腔护理、半卧位等依从性),同时监测呼吸机使用时间及平均住院时间和VAP发生率。结果观察组患者半卧位依从性达97.54%(119/122)、工作人员相关知识知晓率达95.90%(117/122)、手卫生依从性达92.62%(113/122)、口腔护理每8小时1次完成率达100.00%(122/122),均优于对照组;观察组患者呼吸机使用时间、平均住院时间明显较对照组缩短;观察组患者VAP发生率[13.11%(16/122)]明显低于对照组[24.37%(29/119)],差异均有统计学意义(P<0.05)。结论强化培训和督导干预对预防VAP具有积极意义,可提高手卫生、口腔护理每8小时1次、半卧位等依从性,缩短了机械通气及ICU住院时间,从而显著降低了VAP发生率。 展开更多

The back pain is the most common injury in human activities where heavy objects must be lifted or must be suspended for a long time.A weight lifting exoskeleton also known as force augmentation exoskeleton is designed to reduce the strain on the back and the limbs and reduce the risk to suffer injuries.On the other hand,different kinds of controllers have been implemented to achieve whit this goal,for example,a conventional PD Control,PD Control with Gravity Compensation,PD Control with Adaptive Desired Gravity Compensation and PD Control with Robust Compensator.This paper aims to evaluate and compare the performance from the previously cited controllers used to reduce the strain in the back,through the implementation of each controller in a three Degrees Of Freedom(DOF)exoskeleton powered by pneumatic muscle actuators;some numerical simulations as well as experimental trials have been conducted and three different performance indices were used in order to determine the effectiveness of each one with respect to the simple PD controller when the mass to be lifted is unknown. 展开更多

目的:观察在不同二氧化碳气胸压力下胸腔镜食管癌根治术麻醉的效果和血流动力学的变化。方法:选均由同一高年资手术医师带领其治疗组筛选出来的早期食管癌患者60例,在全麻下行胸腔镜食管癌根治术,其中男33例,女27例,年龄(60±8)岁,... 目的:观察在不同二氧化碳气胸压力下胸腔镜食管癌根治术麻醉的效果和血流动力学的变化。方法:选均由同一高年资手术医师带领其治疗组筛选出来的早期食管癌患者60例,在全麻下行胸腔镜食管癌根治术,其中男33例,女27例,年龄(60±8)岁,美国麻醉医师协会(ASA)分级Ⅰ级或Ⅱ级,按术侧胸腔缓慢充二氧化碳的压力值为4、6、8 mmHg者随机分为A、B、C三组,每组各20例。观察三组单肺通气时(T 0)、单肺通气注入二氧化碳压力达标后1 min(T 1)、5 min(T 2)、30 min(T 3)、60 min(T 4)、90 min(T 5)、120 min(T 6)时和双肺通气5 min(T 7)的血压(BP)、心率(HR)、脉搏血氧饱和度(SPO2)、潮气量(VT)、气道压力(PaW)和呼气末二氧化碳分压(P ET CO 2)及不良事件发生率。结果:三组T 2时的收缩压(SBP)、舒张压(DBP)、SPO2、VT与T 0时比较均显著降低(P<0.05);B组血压和心率在各时间点与A组比较无明显差异(P>0.05);C组中T 3时的SBP、DBP和T 2时的SPO2、VT与T 0时比较均显著降低(P<0.05),T 1-T 6时的HR与T 0时比较均显著升高(P<0.05),T 2-T 6时PaW、P ET CO 2较T 0时升高(P<0.05);B组呼吸指标在各时间点与A组比较无明显差异(P>0.05);C组在T 3-T 6时HR较A组升高(P<0.05),SBP、DBP在T 3-T 5时较A组降低(P<0.05),在T 2-T 5时PaW、P ET CO 2较A组升高(P<0.05)。结论:在胸腔内缓慢注入二氧化碳压力为6 mmHg下胸腔镜食管癌根治术更为合理可行,但对麻醉中呼吸、循环管理要求高。 展开更多

Extracting implicit anomaly information through deformation monitoring data mining is highly significant to determining dam safety status.As an intelligent singular value diagnostic method for concrete dam deformation monitoring, shallow neural network models result in local optima and overfitting, and require manual feature extraction.To obtain an intelligent singular value diagnosis model that can be used for dam safety monitoring, a convolutional neural network (CNN) model that has advantages of deep learning (DL), such as automatic feature extraction, good model fitting, and strong generalizability, was trained in this study.An engineering example shows that the predicted result of the intelligent singular value diagnostic method based on CNN is highly compatible with the confusion matrix, with a precision of 92.41%, receiver operating characteristic (ROC) coordinates of (0.03, 0.97), an area-under-curve (AUC) value of 0.99, and an F1-score of 0.91.Moreover, the performance of the CNN model is better than those of models based on decision tree (DT) and k-nearest neighbor (KNN) methods.Therefore, the intelligent singular value diagnostic method based on CNN is simple to operate, highly intelligent, and highly reliable, and it has a high potential for application in engineering. 展开更多

A combination strategy of multi robot hunting in dynamic environment based on a fuzzy inference system (FIS) and artificial immune algorithm is proposed. By analyzing relative relation of hunters and escaper, abstract data is gathered to describe the relative location and relative motion state of the robots, which in turn forms the beginning stage of the fuzzy rule. The artificial immune algorithm optimizes and generates the rule data base and adaptive design considers factors in measuring the hunting efficiency. The optimized rules are applied to the hunting task and the results show that the algorithm can effectively actualize hunting of multiple mobile robots. 展开更多

目的探讨儿童经胸植入左心室心外膜永久起搏器对右心室起搏所致起搏器综合征的逆转作用。方法3例完全性房室传导阻滞(CAVB)患儿,于(0.97±0.68)岁,年龄范围0.5~1.75岁时在外院植入右心室永久起搏器,2例为心外膜游离壁起搏,1例为心... 目的探讨儿童经胸植入左心室心外膜永久起搏器对右心室起搏所致起搏器综合征的逆转作用。方法3例完全性房室传导阻滞(CAVB)患儿,于(0.97±0.68)岁,年龄范围0.5~1.75岁时在外院植入右心室永久起搏器,2例为心外膜游离壁起搏,1例为心内膜心尖部起搏。3例患儿术后均发生起搏器综合征,于清华大学第一附属医院心脏中心小儿科更换为左心室心外膜永久起搏器,分析其更换起搏器后左心室舒张末期内径(LVEDD)及左心室射血分数(LVEF)的改变情况。结果3例患儿于右心室起搏后1.75~7.00(4.25±2.63)年发现起搏器综合征,更换为左心室心外膜起搏器,左心室起搏时平均年龄(5.92±2.67)岁,年龄范围4.25~9.00岁,3例患儿更换左心室起搏器后心功能逐渐改善,更换左心室起搏前LVEDD 52~71 mm,LVEF 16%~25%;左心室起搏后5 d~3个月心功能开始改善,LVEDD 52~61 mm,LVEF 30%~33%;左心室起搏后1.0~4.5年心功能恢复正常。术前LVEDD对比术后LVEDD,P=0.975;术前LVEF对比术后LVEF,P=0.035。结论右心室游离壁或心尖部起搏可导致起搏器综合征的发生,左心室心外膜起搏可逆转右心室起搏所致的起搏器综合征。 展开更多

In recent times,there has been progressive development in artificial intelligence(AI)following the introduction of deep learning in the medical field including gastroenterology and endoscopy.Most of the reported studies were based on retrospective data.Several prospective studies of real-time diagnosis of moving images using the AI system are expected to match the real clinical situation and to aid the endoscopists in the detection and diagnosis of neoplasms without missing any lesion.AI can read a large number of endoscopic images in a few minutes and make a diagnosis;therefore,it is expected to cover the lack of support for the screening esophagogastroduodenoscopy in the health check-up and a large number of capsule images,thereby freeing the endoscopists from this burden.AI can help make the diagnosis during the endoscopic procedure and thereby prevent an unnecessary biopsy for patients taking antithrombotic drugs.AI can also be useful for education and training in endoscopy.Trainees can learn to perform endoscopy and the detection and diagnosis of lesions by the support of AI.In the near future,real-time endoscopic diagnosis using AI is expected to lessen the burden of endoscopists,to enhance the quality level of endoscopists,to overcome the miss of lesions and to make optimal diagnosis. 展开更多

An improved artificial bee colony algorithm with MaxTF heuristic rule for two-sided assembly line balancing problem

Two-sided assembly line is usually used for the assembly of large products such as cars,buses,and trucks.With the development of technical progress,the assembly line needs to be reconfigured and the cycle time of the line should be optimized to satisfy the new assembly process.Two-sided assembly line balancing with the objective of minimizing the cycle time is called TALBP-2.This paper proposes an improved artificial bee colony(IABC)algorithm with the MaxTF heuristic rule.In the heuristic initialization process,the MaxTF rule defines a new task's priority weight.On the basis of priority weight,the assignment of tasks is reasonable and the quality of an initial solution is high.In the IABC algorithm,two neighborhood strategies are embedded to balance the exploitation and exploration abilities of the algorithm.The employed bees and onlooker bees produce neighboring solutions in different promising regions to accelerate the convergence rate.Furthermore,a well-designed random strategy of scout bees is developed to escape local optima.The experimental results demonstrate that the proposed MaxTF rule performs better than other heuristic rules,as it can find the best solution for all the 10 test cases.A comparison of the IABC algorithm and other algorithms proves the effectiveness of the proposed IABC algorithm.The results also denote that the IABC algorithm is efficient and stable in minimizing the cycle time for the TALBP-2,and it can find 20 new best solutions among 25 large-sized problem cases. 展开更多

目的 通过对单纯左心室起搏与双心室起搏的临床疗效比较,探讨单纯左心室起搏作为慢性心力衰竭患者心脏再同步化治疗(CRT)的可行性.方法 选择符合中国心力衰竭诊断和治疗指南2014的CRT适应证患者52例,男性32例,女20例,平均年龄(60.7±... 目的 通过对单纯左心室起搏与双心室起搏的临床疗效比较,探讨单纯左心室起搏作为慢性心力衰竭患者心脏再同步化治疗(CRT)的可行性.方法 选择符合中国心力衰竭诊断和治疗指南2014的CRT适应证患者52例,男性32例,女20例,平均年龄(60.7±13.3)岁,患者成功植入CRT后随机分为两组,单纯左心室起搏组(LVP组,26例)采用右心房左心室起搏模式,标准双心室起搏组(BVP组,26例)采用右心房双心室起搏模式,术后12周进行随访并程控,观察患者术前及术后12周6 min步行距离(6MWD)、N末端B型利钠肽前体(NT-proBNP)、纽约心功能(NYHA)分级及心脏超声指标的变化.结果 组内比较显示,两组患者术后12周NYHA分级、NT-proBNP、舒张末期左心室内径(LVEDD)、左心室间隔部与侧壁基底段收缩期达峰时间差(TPSD)、二尖瓣反流面积(MRA)、主肺动脉射血前时间差(IVMD)较术前均有所降低(P均<0.05),6MWD、左心室射血分数(LVEF)较术前均有所升高(P均<0.05);组间比较显示,LVP组术前术后LVEF、MRA、IVMD、TPSD的差值优于BVP组[(△9.2±5.5)%比(△5.6±3.7)%,P=0.021;(△-1.7±0.6)cm2比(△-0.8±+0.3)cm2,P=0.038;(△-15.3±8.5) ms比(△-10.2±8.1)ms,P=0.033;(△-19.2±4.5)ms比(△-9.6±3.7)ms,P=0.035];而NYHA分级、6MWD、NT-proBNP、LVEDD未见明显差别[△-1.44±0.30比△-1.39±0.34,P=0.062;(△188±41)m比(△190±35)m,P=0.051;(△-3011±803)pg/ml比(△-2889±745)pg/ml,P=0.066;(△-7.2±4.5)mm比(△-8.6±3.7)mm,P=0.064];LVP组有8例发生不良事件,BVP组有7例发生不良事件,两组患者不良事件的发生率无统计学差异(P>0.05).结论 单纯左心室起搏通过程控优化能够实现心室收缩同步最大化,可以明显改善心功能指标;单纯左心室起搏成为慢性心力衰竭CRT治疗的一种策略选择有一定的可行性. 展开更多

Light-driven artificial muscles based on electrospun microfiber yarns

Artificial muscles are widely potential in diverse fields including medical treatment, humanoid robotics and industrial automation. Soft materials due to bio-friendly interaction are suitable for those applications. Planar and fiber shape based artificial muscles that use the polymer as substrate materials can be triggered by various stimuli like electric, moisture and heat. Here,artificial muscles based on electrospun microfiber yarns are demonstrated, which can be remotely controlled. The carbon nanotubes embedded in polyurethane microfibers enable the yarns to efficiently absorb near-infrared light and radiate heat,which induces the fast temperature change that leads to the contraction/expansion motions along the axial direction. Microfiber mats are collected by a rotating cylinder that results in highly oriented alignment. The yarns from microfiber mats are highly twisted into the coiled structure, which generates maximum contractive actuation of 6.7% at 70°C. 1000 cycles were demonstrated without an obvious decline of actuating performances. The light-driven artificial muscles can be potentially applied in wireless control, bio-medical devices and so on. 展开更多
