Construction of a full-thickness human corneal substitute from anterior acellular porcine corneal matrix and human corneal cells

AIM: To construct functional human full-thickness corneal replacements.METHODS: Acellular porcine corneal matrix(APCM) was developed from porcine cornea by decellulariztion. The biomechanical properties of anterior-APCM(AAPCM) and posterior-APCM(PAPCM) were checked using uniaxial tensile testing. Human corneal cells were obtained by cell culture. Suspending ring was designed by deformation of an acupuncture needle. MTT cytotoxicity assay was used to check the cytotoxicity of suspending ring soaking solutions. A new three-dimensional organ culture system was established by combination of suspending ring, 48-well plate and medium together. A human full-thickness corneal substitute was constructed from human corneal cells with AAPCM in an organ coculture system. Biochemical marker expression of the construct was measured by immunofluorescent staining and morphological structures were observed using scanning electron microscopy. Pump function and biophysical properties were examined by penetrating keratoplasty and follow-up clinical observations.RESULTS: There were no cells in the AAPCM or PAPCM, whereas collagen fibers, Bowman’s membrane, and Descemet’s membrane were retained. The biomechanical property of AAPCM was better than PAPCM. Human corneal cells grew better on the AAPCM than on the PAPCM.There was no cytotoxicity for the suspending ring soaking solutions. For the constructed full-depth human corneal replacements keratocytes scattered uniformly throughout the AAPCM and expressed vimentin. The epithelial layer was located on the surface of Bowman’s membrane and composed of three or four layers of epithelial cells expressing cytokeratin 3. One layer of endothelial cells covered the stromal surface of AAPCM, expressed Na+/K+ATPase and formed the endothelial layer. The construct was similar to normal human corneas, with many microvilli on the epithelial cell surface, stromal cells with a long shuttle shape, and zonula occludens on the interface of endothelial cells. Relationship between early structural changes at cornea incision sites and surgical outcomes after phacoemulsification

AIM: To assess the early structural changes at clear corneal cataract incision sites and surgical outcomes using anterior segment optical coherence tomography(AS-OCT).METHODS: We evaluated 80 eyes of 59 patients who underwent phacoemulsification with a clear corneal incision. All incisions were evaluated 1 wk, 1, and 3 mo postoperatively using AS-OCT and analyzed regarding angle, length of the incision, maximal corneal thickness at the incision, and if present, corneal endothelial gap length and incision gap area. The patients were divided into two groups according to whether or not an endothelial gap was present at 1 wk postoperatively(endothelial gap, group 1;no endothelial gap, group 2). We analyzed the difference in patient and surgical factors between the two groups, and compared the surgical outcome and the refractive outcome.RESULTS: An endothelial gap was observed in 56(70.0%) of 80 eyes at 1 wk postoperatively but not at 3 mo postoperatively. The mean patient age was significantly higher in group 1 than in group 2. The longer the length of the corneal incision and the steeper the incision angle, the greater the length and area of the endothelial gap. In group 1, the mean change in mean keratometry of the anterior cornea was significantly greater than in group 2, and the spherical equivalent(SE) and mean numerical error indicated significant myopic changes at 1 wk postoperatively. CONCLUSION: The risk of an endothelial gap increases with patient age and a long corneal incision and steep incision angle. The presence of an endothelial gap aftersurgery may affect the early postoperative corneal curvature and SE. 展开更多

Influence of preoperative astigmatism on corneal biomechanics and accurate intraocular pressure measurement after micro-incision phacoemulsification

AIM: To define the corneal hysteresis (CH), corneal resistance factor (CRF), Goldmann-correlated intraocular pressure (IOPg) and corneal compensated intraocular pressure (IOPcc) prior to and following coaxial micro-incision phacoemulsification in patients with corneal astigmatism. METHODS: Of 97 patients with cataracts were enrolled in the study. Group 1 included patients with corneal astigmatism (K1-K2) values of K1-K2<+1.0 D, and group 2 with values of K1-K2 ≥+1.0 D and ≤+2.25 D. Coaxial micro-incision phacoemulsification of a corneal incision of 2.0 mm with intraocular lens (IOL) implantation was performed. CH, CRF, IOPg, IOPcc, waveform score (WS) were measured preoperatively and one week, one month postoperatively using an Ocular Response Analyzer. Axial length (AXL) was calculated by Tomey Optical Biometer OA 2000. RESULTS: Group 1 consisted of 51 patients with mean corneal astigmatism value of +0.49±0.25 D. Group 2 included 46 patients with astigmatism of +1.43±0.43 D. In group 1, CRF (t=2.68, P<0.05), CH (t=2.64, P<0.05) and WS (t=3.51, P<0.05) were significantly lower one week postoperatively, when compared to the preoperative values. CRF significantly decreased (t=3.61, P<0.05) when measured one month following the surgery. In group 2 CH (t=5.92, P<0.05), and WS (t=3.96, P<0.05) were significantly lowered one week after cataract surgery. Moreover, we observed a significant decrease in IOPg (t=2.24, P<0.05), CRF (t=5.05, P<0.05) and CH (t=2.31, P<0.05) one month after phacoemulsification. There was no statistically significant (t=-0.83, P=0.41) difference in AXL between study groups. CONCLUSION: CRF, CH and IOPg are reduced in patients with preoperative corneal astigmatism equal or higher than +1.0 D and lower than +2.25 D. Hence, bias of IOPg measurement in these patients may cause underestimation of the real IOP both before and after cataract surgery. The measurement of IOPcc allows the precise assessment of IOP pre- and postoperatively, independently on corneal astigmatism, CH and CRF values. 展开更多

Interchangeability of corneal curvature and asphericity measurements provided by three different devices

AIM: To evaluate the interchangeability of keratometric and asphericity measurements provided by three measurement systems based on different optical principles.METHODS: A total of 40 eyes of 40 patients with a mean age of 34.1 y were included. In all cases, a corneal curvature analysis was performed with IOL-Master(IOLM), iDesign 2(ID2), and Sirius systems(SIR). Differences between instruments for flattest(K1) and steepest(K2) keratometric readings, as well as for magnitude and axis of corneal astigmatism were analyzed. Likewise, differences in asphericity(Q) between SIR and ID2 were also evaluated. RESULTS: Mean differences between devices for K1 were 0.20±0.21(P<0.001),-0.12±0.36(P=0.046) and-0.32±0.36 D(P<0.001) for the comparisons IOLM-SIR, IOLM-ID2 and SIR-ID2, respectively. The ranges of agreement for these comparisons between instruments were 0.41, 0.70, and 0.70 D. For K2, mean differences were 0.31±0.33(P<0.001),-0.08±0.43(P=0.265) and-0.39±0.38 D(P<0.001), with ranges of agreement of 0.65, 0.84, and 0.74 D. Concerning magnitude of astigmatism, ranges of agreement were in the limit of clinical relevance(0.49 D, P=0.011;0.55 D, P=0.386;0.43 D, P=0.05). In contrast, ranges of agreement were clinically relevant for astigmatic axis(26.68o, 33.83o and 18.37o, P≥0.121) and for Q between SIR and ID2(0.16, P<0.001). CONCLUSION: The keratometric corneal power, astigmatic axis and asphericity measurements provideby the three systems evaluated cannot be considered as interchangeable, whereas measurements of corneal astigmatism obtained with SIR and ID2 can be considered as interchangeable for clinical purposes. 展开更多

BACKGROUND The current case report describes successful phacoemulsification with the aid of perioperative topical ascorbic acid (AA) in two patients with corneal endothelial disorders to prevent postoperative corneal endothelial decompensation. CASE SUMMARY Two eyes of two patients underwent phacoemulsification with pre-existing corneal endothelial disorders including Fuchs corneal endothelial dystrophy (Patient 1) and endotheliitis (Patient 2). Topical AA was applied to both patients at least one month before and after with a frequency of four times per day. After the surgery, both eyes improved best-corrected visual acuity (BCVA) and there was limited human corneal endothelial cell loss without signs of corneal endothelial decompensation, such as deteriorated BCVA or persistent corneal edema during the follow-up of at least two years.CONCLUSION Perioperative administration of topical AA may be an alternative therapy to the triple procedure in patients expecting to undergo cataract surgery. 展开更多

BACKGROUND An in vitro injury model mimicking a corneal surface injury was optimised using human corneal epithelial cells (hCEC). AIM To investigate whether corneal-stroma derived stem cells (CSSC) seeded on an amniotic membrane (AM) construct manifests an anti-inflammatory, healing response. METHODS Treatment of hCEC with ethanol and pro-inflammatory cytokines were compared in terms of viability loss, cytotoxicity, and pro-inflammatory cytokine release, in order to generate the in vitro injury. This resulted in an optimal injury of 20%(v/v) ethanol for 30 s with 1 ng/mL interleukin-1 (IL-1) beta. Co-culture experiments were performed with CSSC alone and with CSSC-AM constructs. The effect of injury and co-culture on viability, cytotoxicity, IL-6 and IL-8 production, and IL1B, TNF, IL6, and CXCL8 mRNA expression were assessed. RESULTS Co-culture with CSSC inhibited loss of hCEC viability caused by injury. Enzyme linked immunosorbent assay and polymerase chain reaction showed a significant reduction in the production of IL-6 and IL-8 pro-inflammatory cytokines, and reduction in pro-inflammatory cytokine mRNA expression during co-culture with CSSC alone and with the AM construct. These results confirmed the therapeutic potential of the CSSC and the possible use of AM as a cell carrier for application to the ocular surface.CONCLUSION CSSC were shown to have a potentially therapeutic anti-inflammatory effect when treating injured hCEC, demonstrating an important role in corneal regeneration and wound healing, leading to an improved knowledge of their potential use for research and therapeutic purposes. 展开更多

目的探讨绷带型角膜接触镜在上睑下垂手术治疗中的应用效果。方法对仙桃市第一人民医院2013年3月至2016年3月初发先天性上睑下垂患者62例(73眼),随机抽签法分成A、B、C三组。三组均行提上睑肌缩短术,A组20例(24眼)从术中即开始配戴绷带... 目的探讨绷带型角膜接触镜在上睑下垂手术治疗中的应用效果。方法对仙桃市第一人民医院2013年3月至2016年3月初发先天性上睑下垂患者62例(73眼),随机抽签法分成A、B、C三组。三组均行提上睑肌缩短术,A组20例(24眼)从术中即开始配戴绷带型角膜接触镜,B组22例(25眼)手术结束后即配戴绷带型角膜接触镜,C组20例(24眼)术后不使用绷带型角膜接触镜。术后随访3个月,比较三组术后FL染色评分和眼部疼痛程度评分。结果术后角膜染色评分比较:术后1 d,B组与C组差异无统计学意义(q=0.537,P>0.05),但与A组差异均有统计学意义(q=10.876、11.298,均P<0.05);术后7 d,三组间差异均有统计学意义(q=8.682、4.048、4.722,均P<0.05);术后2周,A、B两组间差异无统计学意义(q=1.635,P>0.05),但与C组差异均有统计学意义(q=8.747、7.201,均P<0.05);术后3个月,三组间差异均无统计学意义(q=1.607、0.186、1.438,均P>0.05)。术后眼部疼痛评分比较:术后1 d,三组间差异均无统计学意义(q=0.986、0.586、0.410,均P>0.05);术后7 d,A、B两组间差异已无统计学意义(q=0.948,P>0.05),但与C组差异均有统计学意义(q=4.743、3.843,均P<0.05);术后1个月,三组间差异均无统计学意义(q=3.181、0.599、2.654,均P>0.05)。所有患者62例(73眼)术后上睑弧度及重睑成形良好,平视时睑裂高度正常且均位于瞳孔上缘以上,16眼轻度眼睑闭合不全,1~2个月后均自然闭合良好,无手术切口感染及角膜感染发生。结论从术中应用绷带型角膜接触镜,可以有效的对角膜进行保护,且术后短期内持续佩戴绷带型角膜接触镜安全性高,能促进角膜上皮的愈合,减少了暴露性角膜炎的发生,同时也能明显缓解患者眼部疼痛症状。 展开更多

BACKGROUND Glaucoma is a group of diseases characterized by a specific pattern of optic nerve neuropathy and retinopathy.Increasing evidence demonstrates glaucoma associated corneal endothelium loss.Direct-compression mechanism due to elevated intraocular pressure(IOP),cell toxicity after long term exposure to preservatives and glaucoma surgery have been reported to be the possible mechanism.Herein,we compare the specular endothelial microscopy in primary open-angle glaucoma(POAG)patients and healthy controls of the same age group to observe the corneal endothelium changes and the correlations to the mean IOP in a Chinese case control study.AIM To investigate corneal endothelial cell density in Chinese patients with POAG.METHODS A case control study was performed on 60 eyes of 60 patients with POAG.Exclusion criteria included history of corneal diseases,intraocular diseases,contact lens use,ocular trauma or surgery(including intraocular surgery and laser treatment),congenital abnormalities or systemic diseases such as diabetes.Intraocular pressure was measured using Goldmann tonometry.Indirect specular microscopy(TOPCON SP-2000P)was performed on central corneas and endothelial images were acquired.Endothelial cell density,area and cell counts were analyzed.RESULTS Endothelial cell density was 2959±236 cells/mm2 in healthy controls and 2757±262 cells/mm2 in patients with POAG.The POAG eyes had significantly lower endothelial cell density compared to healthy control eyes(P<0.001).In the POAG group,endothelial cell density was 2686±233 cells/mm2 in the patients receiving medication and 2856±272 cells/mm2 in the untreated subgroup.The eyes receiving medication had significantly lower endothelial cell density compared to untreated eyes.There was a negative correlation between cell density and mean IOP(r=-0.286,P=0.004),positive correlation between the average cell area and mean IOP(r=0.228,P=0.022),maximum cell area and mean IOP(r=0.218,P=0.029)and minimum cell area and mean IOP(r=0.290,P=0.003).The percentage of 展开更多

目的:研究飞秒激光辅助准分子激光原位角膜磨镶术(laser in situ keratomileusis,LASIK)和准分子激光上皮下角膜磨镶术(lasersubepithelial keratomileusis,LASEK)矫正中度近视对患者角膜厚度、后表面高度及前房深度的影响。方法:选择2... 目的:研究飞秒激光辅助准分子激光原位角膜磨镶术(laser in situ keratomileusis,LASIK)和准分子激光上皮下角膜磨镶术(lasersubepithelial keratomileusis,LASEK)矫正中度近视对患者角膜厚度、后表面高度及前房深度的影响。方法:选择2016年10月-2018年9月于笔者医院眼科手术治疗的中度近视患者94例,A组47例采用飞秒激光仪制角膜瓣辅助LASIK治疗,B组47例采用LASEK治疗。结果:两组术后1个月、3个月裸眼视力及最佳矫正视力均有改变,前房深度均变浅,角膜后表面Diff值、角膜Q值均明显增加,且A组Diff值增加略小于B组,但差异均无统计学意义(P>0.05);A组Q值增加显著大于B组,差异有统计学意义(P<0.05)。A组并发症发生率为4.3%明显低于B组的17.0%,差异有统计学意义(P<0.05)。结论:LASEK对角膜前表面影响较小,飞秒激光辅助LASIK手术创伤小、术后并发症少,临床工作中需要根据具体情况选择适合患者的治疗方案。 展开更多

角膜异物剔除术后应用妥布霉素滴眼液及氧氟沙星眼膏疗效观察

Labrasol, as a non-ionic surfactant, can enhance the permeation and absorption of drugs, and is extensively used in topical, transdermal, and oral pharmaceutical preparations as an emulsifier and absorption enhancer. Recent studies in our laboratory have indicated that labrasol has a strong absorption enhancing effect on different types of drugs in vitro and in vivo. This study was performed to further elucidate the action mechanism of labrasol on the corneal penetration. In this research, the fluorescein sodium, a marker of passive paracellular transport of tight junction, was selected as the model drug to assess the effect of labrasol on in vitro corneal permeability. To investigate the continuous and real-time influence of labrasol on the membrane permeability and integrity, the Ussing chamber system was applied to monitor the electrophysiological parameters. And, furthermore, we elucidated the effect of labrasol on excised cornea at the molecular level by application of RT-PCR, Western blot, and immunohistochemical staining. The results indicated that labrasol obviously enhance the transcorneal permeability of fluorescein sodium, and the enhancement was realized by interacting with and down-regulating the associated proteins, such as Factin, claudin-1 and β-catenin, which were contributed to cell-cell connections, respectively. 展开更多

因角膜异物而漏诊眼内和眶内异物的原因分析

目的分析因角膜表面铁质异物而被漏诊眼内和眶内异物的原因。方法回顾性分析2018年2月至11月我院就诊被漏诊眼内和眶内异物的角膜铁质异物剔除术后6例(6只眼)的临床资料。结果本组初诊时均未接受CT、X线和UBM检查,其中5例未接受B超检查... 目的分析因角膜表面铁质异物而被漏诊眼内和眶内异物的原因。方法回顾性分析2018年2月至11月我院就诊被漏诊眼内和眶内异物的角膜铁质异物剔除术后6例(6只眼)的临床资料。结果本组初诊时均未接受CT、X线和UBM检查,其中5例未接受B超检查,初诊均未发现角膜、虹膜或巩膜的细小穿孔和眼内眶内异物。6例角膜浅层异物接受角膜异物剔除术后出现并发症,通过CT、X线、B超和UBM检查后确诊眼内或眶内异物。结论角膜表面的铁质异物可能并发眼内或眶内异物,漏诊可导致严重的眼部并发症,细致的眼部检查,并辅以CT、X线、B超和UBM检查能减少漏诊。 展开更多

目的探讨保留段氏层角膜内皮移植术治疗角膜内皮失代偿的安全性、有效性。方法回顾性病例系列研究,收集宁波市眼科医院2013年11月至2017年6月,19例20眼行保留段氏层角膜内皮移植术,用虹膜恢复器呈螺旋形钝性机械分离,分离至段氏层后剥... 目的探讨保留段氏层角膜内皮移植术治疗角膜内皮失代偿的安全性、有效性。方法回顾性病例系列研究,收集宁波市眼科医院2013年11月至2017年6月,19例20眼行保留段氏层角膜内皮移植术,用虹膜恢复器呈螺旋形钝性机械分离,分离至段氏层后剥离角膜基质,徒手制作保留段氏层角膜内皮植片,人工晶体植入器将内皮植入前房后,注入无菌空气展开、固定植片,术后平躺4小时。结果徒手制作角膜内皮植片植片成功率19/20,角膜内皮植片脱位6例,再次注气复位全部成功,术后高眼压5例,4例高眼压经放出前房气体,眼压控制,1例ICE术后高眼压,经睫状体光凝控制眼压。术后最佳矫正视力为0.1~0.8,平均视力(0.24±0.17),术前最佳矫正视力与术后最佳矫正视力二者差异有统计学意义(t=-6.023,P<0.001),术后角膜厚度为(600±91.27)mm,术后角膜内皮细胞密度为(1570±130.45)个/mm^2。结论保留段氏层角膜内皮移植术,器材简单,手术成功率高,术后视力明显提高。 展开更多
