Blood-Letting Therapy for Hypertension: A Systematic Review and Meta-Analysis of Randomized Controlled Trials

Objective:To evaluate the efficacy and safety of blood-letting therapy(BLT)in treatment of hypertension.Methods:A comprehensive electronic and manual bibliographic searches were performed in Cochrane Central Register of Controlled Trials,Excerpt Medica Database(EMBASE),PubMed,China National Knowledge Infrastructure,Chinese Scientific Journal Database,Chinese Biomedical Literature Database,and Wanfang Database to identify randomized controlled trials(RCTs)in which hypertensive patients were treated with BLT or BLT plus antihypertensive drugs(BPAD)against placebo,no treatment or antihypertensive drugs.The Cochrane Risk Assessment Tool was used to assess the methodological quality of trials.The Review Manager 5.3 software was used for meta-analysis.Results:A total of 7 RCTs with 637 hypertensive patients from 1989 to 2017 were identified.Compared with antihypertensive drugs,blood pressure was significantly reduced by BLT(RR=1.21,95%CI:1.01 to 1.44,P=0.03;heterogeneity:P=0.06,I^2=60%)and BPAD(RR=1.25,95%CI,1.02 to 1.53,P=0.03;heterogeneity:P=0.01,I^2=71%).Moreover,a significant improvement in Chinese medicine syndrome by BLT(RR=1.32;95%CI:1.14 to 1.53,P=0.0002;heterogeneity:P=0.53,I^2=0%)and BPAD(RR=1.47;95%CI:1.06 to 2.04,P=0.02;heterogeneity:P=0.13,I^2=56%)was identified.The reported adverse effects were well tolerated.Conclusions:Although some positive findings were identified,no definite conclusions regarding the efficacy and safety of BLT as complementary and alternative approach for treatment of hypertension could be drew due to the generally poor methodological design,significant heterogeneity,and insufficient clinical data.Further rigorously designed trials are warranted to confirm the results. 展开更多

Optimization modeling method for coal-to-electricity heating load considering differential decisions

Heating by electricity rather than coal is considered one effective way to reduce environmental problems. Thus, the electric heating load is growing rapidly, which may cause undesired problems in distribution grids because of the randomness and dispersed integration of the load. However, the electric heating load may also function as an energy storage system with optimal operational control. Therefore, the optimal modeling of electric heating load characteristics, considering its randomness, is important for grid planning and construction. In this study, the heating loads of distributed residential users in a certain area are modeled based on the Fanger thermal comfort equation and the predicted mean vote thermal comfort index calculation method. Different temperatures are considered while modeling the users’ heating loads. The heat load demand curve is estimated according to the time-varying equation of interior temperature. A multi-objective optimization model for the electric heating load with heat energy storage is then studied considering the demand response(DR), which optimizes economy and the comfort index. A fuzzy decision method is proposed, considering the factors influencing DR behavior. Finally, the validity of the proposed model is verified by simulations. The results show that the proposed model performs better than the traditional method. 展开更多

Objective: To assess the effect and safety of Huannao Yicong Formula (还脑益聪方, HYF) in the treatment of patients with mild-to-moderate Alzheimer's disease (AD). Methods: Sixty patients with mild-to moderate AD were evenly randomized into HYF group and donepezil group with the random number method. Patients in the HYF group took 5 g of HYF granules twice daily and 5 mg placebo of donepezil once daily. Patients in the donepezil group took 5 mg donepezil once daily and 5 g placebo of HYF granules twice daily. The intervention lasted for 6 months. Clinical researchers, participants and statisticians were blinded to the treatment assignment throughout the study. The primary outcomes were scores of Alzheimer's Disease Assessment Scale-Cognitive Subscale (ADAS-Cog) and Chinese Medicine Symptom Scale (CM-SS). The secondary outcomes were scores of Montreal Cognitive Assessment (MoCA) test and Mini-Mental State Exam (MMSE). The serum levels of acetylcholinesterase (AchE) and amyloid-β protein 42 (Aβ 42) were detected with enzyme linked immunosorbent assay kits. The scale assessments were conducted at baseline, the 3rd and 6th months of treatment, respectively. Biochemistry tests were conducted at baseline and the 6th month of treatment. Results: A total of 52 patients completed the trial, 28 in HYF group and 24 in donepezil group. Compared with the baseline, HYF and donepezil significantly decreased the total scores of ADAS-Cog and CM-SS, and significantly increased the scores of MoCA and MMSE after 6-month treatment (all P<0.01). Both treatments remarkably reduced the serum levels of AchE and Aβ 42 (both P<0.05). The CM-SS total effective rate of HYF was significantly higher than donepezil [75.00%(21/28) vs. 54.17%(13/24), P<0.05]. No severe adverse events were observed in both groups. Objective: To evaluate the effect of tai chi exercise on cardiac and static lung function for older community-dwelling adults at risk of ischemic stroke. Methods: A total of 170 older community-dwelling adults (aged 55–75 years old) at risk of ischemic stroke were allocated to either tai chi training group (85 cases, five 60-min sessions of tai chi training per week for 12 weeks) or control group (85 cases, usual pbysical activity for 12 weeks) using a computer-generated randomization. The echocardiographic parameters of cardiac structure, cardiac function and static lung function were measured at baseline, after 12 weeks of intervention and additional 12-week follow-up period by a blinded professional staff member using a color Doppler ultrasound imaging device or a cardiopulmonary function instrument. The t test and linear mixed model based on the intention-to-treat analysis principle was used to calculate the effect. The adverse effect was observed. Results: Most of echocardiographic parameters on the cardiac structure, cardiac function and static lung function between the tai chi group and control group did not have a significant difference either post 12-week intervention or additional 12-week follow-up period. Only three parameters involving in right ventricular diameter (P=0.024), main pulmonary artery diameter (P=0.002) and vital capacity maximum (P=0.036) were beneficial to be improved in the tai chi group compared to the control group by the analysis of mixed linear model. No adverse effects were found during the intervention period. Conclusions: The 12-week tai chi exercise did not have an obvious beneficial effect on cardiac structure, cardiac function and static lung function in older community-dwelling adults at risk of ischemic stroke.(Trial registration No. ChiCTR-TRC-13003601) 展开更多

Effect of Zhizhu Kuanzhong Capsules (枳术宽中胶囊) on Treatment of Functional Dyspepsia: A Meta-Analysis of Randomized Controlled Trials

Objective: To evaluate the effect of Zhizhu Kuanzhong Capsules (枳术宽中胶囊, ZKC) for functional dyspepsia (FD) through meta-analysis. Methods: Online databases, including PubMed, EM base, China National Knowledge Infrastructure, Wanfang Data, VIP database and Cochrane Library, were searched for randomized controlled trials (RCTs) of ZKC for FD from the inception to April, 2016. Trials were selected according to inclusion criteria and were evaluated with quality assessment standards in the Cochrane Handbook for Systematic Reviews of Interventions and Jadad scale. RevMan 5.3 and GRADEprofiler 3.6 were used for statistical analysis and evidence quality assessment. Results: Twenty-three trials with 2,496 patients were included and most of them were of poor methodological quality. ZKC alone or ZKC combined with routine Western medicine (WM) showed a better clinical effect rate compared with the control group of WM [odds ratio (OR)=3.32, 95% confidence interval (2.66, 4.15), P<0.00001]. No serious adverse reactions were reported. Conclusions: ZKC alone or ZKC combined with routine WM could significantly improve the clinical effective rate in the treatment of FD. The quality of the evidence is low, so it is necessary to design multicenter, strictly randomized and double-blind controlled trials with large samples to validate the conclusions. 展开更多

Objective: This study aims to systematically evaluate the efficacy and safety of compound kushen injection (CKI) in combination with chemotherapy in patients with gastric cancer (GC). Methods: A comprehensive electronic search was conducted by searching PubMed, EMBASE, Cochrane Library, Chinese Biological Medical disc, China National Knowledge Infrastructure and Wanfang databases (the last update January 20, 2018). All randomized controlled trials (RCTs) of CKI plus chemotherapy versus chemotherapy alone in GC patients were identified. The quality of each study was evaluated using the Jadad’s scale, and the meta-analysis was performed using Review Manager 5.3 and STATA 14 software. Results: A total of nine studies on 688 cases were included in this study. The results showed that CKI combined with chemotherapy had a better effect on improving patients’ overall response rate (ORR) and life quality. The consequences of Egger’s and Begg’s tests showed there was no significant publication bias. Conclusion: The current evidence showed that CKI may enhance the clinical efficacy of chemotherapy, improve the quality of life and increase the safety in patients with gastric cancer. 展开更多

A simple approach to synthesize ZnO microstructures was reported using dodecylamine,hexadecylamine and oleylamine as template agents.The synthesized ZnO was characterized by XRD,FESEM,TEM,EDS mapping,FTIR,UV-vis DRS,Raman spectroscopy and BET analysis.Hierarchical oriented ZnO microstructures were obtained.The photocatalytic performance of ZnO prepared with different types of alkylamines was evaluated by the degradation of methylene blue(MB)and methyl orange(MO).The results suggested that alkylamine control the nucleation,growth and morphology of ZnO.The photocatalytic properties of ZnO on the degradation of MB and MO decreased with increasing the alkyl chain length in alkylamine. 展开更多

Controlled islanding is considered to be the last countermeasure to prevent a system-wide blackout in case of cascading failures.It splits the system into self-sustained islands to maintain transient stability at the expense of possible loss of load.Generator coherence identification is critical to controlled islanding scheme as it helps identify the optimal cut-set to maintain the transient stability of the post-islanding systems.This paper presents a novel approach for online generator coherency identification using phasor measurement unit(PMU) data and dynamic time warping(DTW).Results from the coherence identification are used to further cluster non-generator buses using spectral clustering with the objective of minimizing power flow disruptions.The proposed approach is validated and compared to existing methods on the IEEE39-bus system and WECC 179-bus system, through which its advantages are demonstrated. 展开更多

Objective: To evaluate the efficacy and safety of essential oil treatment for type Ⅲ chronic prostatitis/chronic pelvic pain syndrome(CP/CPPS). Methods: A randomized controlled trial was conducted from December 2014 to October 2015. Seventy type Ⅲ CP/CPPS patients were assigned to the essential oil group(35 cases) or almond placebo oil control group(35 cases) by a random number table. The oil was smeared by self-massage on the suprapubic and sacral region once a day for 4 weeks. The National Institutes of Health Chronic Prostatitis Syndrome Index(NIH-CPSI) and expressed prostatic secretions(EPS) were examined. The primary outcome was NIH-CPSI pain domain. The secondary outcomes included other NIH-CPSI domains and laboratory examinations of EPS. Adverse events were also observed. Results: Sixty-six subjects completed the full 4-week treatment. There was no significant difference between almond oil control and essential oil groups in terms of the total score of NIH-CPSI, pain, quality of life and urination domain scores of NIH-CPSI and EPS examinations(P>0.05). In the essential oil group, pain between rectum and testicles(perineum) in the domain of pain or discomfort was significantly reduced at week 2 and week 4 compared with almond oil control group(P<0.01). No serious adverse events occurred. Conclusion: The essential oil may reduce the pain or discomfort in the perineum region in patients with CP/CPPS. 展开更多

Background: Many patients with chronic angina experience anginal episodes despite successful recanalization, antianginal and antiischemic medications. Empirical observations suggested that Shenzhu Guanxin Recipe Granules(参术冠心方颗粒, SGR), a Chinese herbal compound, exerted potential impacts on increased treadmill exercise performance and angina relieve. However, there has been no systematic study to clarify the impact of SGR on exercise tolerance in patients with stable angina. The SERIES(ShEnzhu guanxin Recipe for Improving Exercise tolerance in patients with Stable angina) trial is designed to determine the effects of SGR on exercise duration, electrocardiographic(ECG) evidence of myocardial ischemia, and incidence of major adverse cardiac events(MACE) in stable anginal patients. Methods: A total of 184 eligible patients with stable angina will be randomly assigned to receive placebo or SGR(10 g/day for 12 weeks) in a 1:1 ratio. The primary outcome will be the change from baseline in total exercise tolerance duration, time to onset of angina and ECG ischemia during exercise treadmill testing performed over a 12-week study period. The secondary outcome will include ECG measures, the occurrence and composite of MACE and the Seattle Angina Questionnaire score. Moreover, the coronary microcirculation will be evaluated to explore the possible effects in response to treatment of SGR. After the procedure, all participants will be followed up by interview at 3 and 6 months, enquiring about any cardiac events, hospitalizations, cardiac functional level and medication usage. Additionally, the occurrence of adverse events will be evaluated at each follow-up. Discussion: This study may provide novel evidence on the efficacy of SGR in improving exercise tolerance and potentially reducing clinical adverse events. 展开更多

Current deadlock control policies(DCPs) based on mixed integer programming(MIP) cannot detect siphons that cause and cope with livelocks in Petri nets. This study proposes a revised mixed integer programming(RMIP) method to directly solve the new smart siphons(NSSs) associated with livelocks in a system of sequential systems with shared resources(S~4 R), a typical subclass of generalized Petri net models. Accordingly,the solved NSSs are max’-controlled by adding the corresponding control places(CPs). As a result, an original S~4 R system with livelocks can be converted into the live controlled Petri net system. The related theoretical analysis and an example are given to demonstrate the proposed RMIP and the corresponding control algorithm(CA). 展开更多

目的 对疫苗生产车间半成品配制区域的悬浮粒子进行动态监测和数据分析.方法 采用便携式悬浮粒子采样仪,于2016和2017年对乙型脑炎减毒活疫苗生产洁净车间进行日常悬浮粒子检测,其中A、B级洁净区域为每班次检测,C级洁净区域为每周检测.... 目的 对疫苗生产车间半成品配制区域的悬浮粒子进行动态监测和数据分析.方法 采用便携式悬浮粒子采样仪,于2016和2017年对乙型脑炎减毒活疫苗生产洁净车间进行日常悬浮粒子检测,其中A、B级洁净区域为每班次检测,C级洁净区域为每周检测.对2年的检测数据制作单值-移动极差控制图,并进行t检验比较.结果 3个级别洁净区域的悬浮粒子数或均值如下,A级区≥0.5 μm的粒子数均<1,≥5.0μm的粒子数均为0.B级区≥0.5 μm的粒子数均值:2016年为46.82(0~143.21),2017年为38.83(0~125.33);≥5.0μm的粒子数均值:2016年为2.52(0~7.83),2017年为2.41(0~7.15).C级区≥0.5 μm的粒子数均值:2016年为214.64(0~679.16),2017年为209.81(0~619.31);≥5.0 μm的粒子数均值:2016年为15.04(0~47.13),2017年为14.16(0~39.24).3个洁净区域的悬浮粒子数均小于95%置信上限且均符合GMP范围要求.2016和2017年检测数据的差异无统计学意义(t值为0.14~1.02,P值均>0.05).结论 疫苗生产车间半成品配制区的悬浮粒子动态监测结果符合GMP标准. 展开更多

The efficacy and safety of besifloxacin for acute bacterial conjunctivitis: a Meta-analysis

AIM: To evaluate the relative efficacy and safety of besifloxacin for treatment of acute bacterial conjunctivitis.METHODS: A comprehensive search in PubMed, EMBASE Web of Science, Cochrane Central Database and CNKI was undertaken for randomized controlled trials(RCTs) comparing besifloxacin with other treatments or placebo. The primary outcome measures were clinical resolution, rates of bacterial eradication, individual clinical outcomes, cure rates, and bacterial eradication rates of different kinds of pathogens. Safety outcomes were the number of adverse effects(AEs). The final search was performed on August 2018.RESULTS: Eight RCTs were included. Five studies compared the efficacy and safety of besifloxacin with placebo, 2 studies compared besifloxacin with moxifloxacin, and 1 study compared besifloxacin with gatifloxacin. A total of 3105 patients met the inclusion criteria. Besifloxacin presented higher efficacy and safety than did placebo in clinical resolution, rates of bacterial eradication, individual clinical outcomes, cure rates, bacterial eradication rates of different kinds of pathogens and the number of AEs. There was no significant difference between besifloxacin and moxifloxacin or gatifloxacin in the comparison items mentioned above.CONCLUSION: Besifloxacin is highly effective and safe for treatment of acute bacterial conjunctivitis. Further comparative trials regarding the effect of besifloxacin for treatment of acute bacterial conjunctivitis will aid in treatment decisions. 展开更多

OBJECTIVE:To assess the efficacy and safety of Jiawei Xiaoyao (JWXY) capsules on mild to moderate depression with anxiety symptoms,a randomized,double-blind,double-dummy controlled,multicenter,parallel-treatment trial was carried out among 210 outpatients with mild to moderate depression and anxiety symptoms from three hospitals in Beijing China.METHODS:Participants were randomized into the JWXY group or the sertraline group.Each group received JWXY treatment and sertraline placebo,or sertraline and JWXY placebo for 8 weeks.Main outcomes were measured using the Hamilton Depression Rating Scale (HAMD),Hamilton Anxiety Rating Scale (HAMA) and the Clinical Global Impression Scale.RESULTS:JWXY and sertraline had the equivalent effect on HAMD at every interview point.JWXY was more effective at reducing the HAMA scores at the 2nd and 12th week,HAMD sleep disturbance subscale scores at the 8th and 12th week,and HAMA somatic anxiety subscale scores at the 12th week.The rate of adverse events in the two groups was the same.CONCLUSION:For mild to moderate depression with anxiety symptoms,JWXY could be as effective as sertraline in alleviating depressive symptoms.For anxiety symptoms,JWXY may be effective more quickly and with longer lasting effects than sertraline.In particular,it may also improve quality of sleep and somatic anxiety symptoms.JWXY is safe and cheaper than conventional antidepressants,and may be the first alternative choice for depression with anxiety symptoms. 展开更多

A conventional diode-triggered silicon-controlled rectifier(DTSCR) structure with a layout strategy for electrostatic discharge(ESD) protection is presented and confirmed in a 65-nm CMOS technology.The modified device is featured by a scaled-down trigger-diode string, which shortens its turn-on time using a variable scaling factor. To confirm the parasitic resistance adjustments of the modified DTSCR, transmission line pulsing(TLP)/very fast transmission line pulsing(VF-TLP) tests and simulations are performed on the device. Compared to a conventional DTSCR, this structure exhibits an improved turn-on speed and robustness, which are suitable for I/O protection of ESD events in the nanosecond range, particularly the charged-device model(CDM) event. 展开更多

The rat high-impact free weight drop model mimics the diffuse axonal injury caused by severe traumatic brain injury in humans,while severe controlled cortical impact can produce a severe traumatic brain injury model using precise strike parameters.In this study,we compare the pathological mechanisms and pathological changes between two rat severe brain injury models to identify the similarities and differences.The severe controlled cortical impact model was produced by an electronic controlled cortical impact device,while the severe free weight drop model was produced by dropping a 500 g free weight from a height of 1.8 m through a plastic tube.Body temperature and mortality were recorded,and neurological deficits were assessed with the modified neurological severity score.Brain edema and bloodbrain barrier damage were evaluated by assessing brain water content and Evans blue extravasation.In addition,a cytokine array kit was used to detect inflammatory cytokines.Neuronal apoptosis in the brain and brainstem was quantified by immunofluorescence staining.Both the severe controlled cortical impact and severe free weight drop models exhibited significant neurological impairments and body temperature fluctuations.More severe motor dysfunction was observed in the severe controlled cortical impact model,while more severe cognitive dysfunction was observed in the severe free weight drop model.Brain edema,inflammatory cytokine changes and cortical neuronal apoptosis were more substantial and blood-brain barrier damage was more focal in the severe controlled cortical impact group compared with the severe free weight drop group.The severe free weight drop model presented with more significant apoptosis in the brainstem and diffused blood-brain barrier damage,with higher mortality and lower repeatability compared with the severe controlled cortical impact group.Severe brainstem damage was not found in the severe controlled cortical impact model.These results indicate that the severe controlled cortical impact model is relat 展开更多

【目的】探讨控制性降压、急性高容量血液稀释(AHH)及回收式自体输血在颅内肿瘤外科手术中的应用价值。【方法】回顾性分析2016年1月至2018年1月本院收治的82例颅内肿瘤患者的临床资料,根据输血方法的不同将其分为观察组(控制性降压+ AH... 【目的】探讨控制性降压、急性高容量血液稀释(AHH)及回收式自体输血在颅内肿瘤外科手术中的应用价值。【方法】回顾性分析2016年1月至2018年1月本院收治的82例颅内肿瘤患者的临床资料,根据输血方法的不同将其分为观察组(控制性降压+ AHH +回收式自体输血,n= 43)和对照组(控制性降压+ AHH,n = 39)。观察两组AHH前即刻(T0)、AHH后30 min(T1)、AHH后60 min(T2)和手术结束时(T3)患者的心率(HR)、平均动脉压(MAP)、中心静脉压(CVP)等指标。【结果】两组手术时间、术中出血量比较差异无统计学意义(P>0.05);观察组未输异体血比例明显高于对照组,差异有统计学意义(P<0.05)。观察组和对照组T1、T2时MAP明显高于T0时,观察组T1、T2时CVP明显高于对照组,差异均有统计学意义(P<0.05)。两组术后纤维蛋白原(FIB)低于术前,凝血酶原时间(PT)、活化部分凝血酶原时间(APTT)明显较高于术前,差异均有统计学意义(均P<0.05);观察组和对照组术前、术后FIB、PT和APTT比较,差异无统计学意义(P>0.05)。【结论】控制性降压联合AHH及回收式自体输血在颅内肿瘤外科手术中有较好的应用价值,可减少异体输血情况,对血流动力学、凝血功能影响小。 展开更多
