Effects of RIRS and Mini-PCNL on liver and kidney function,endocrine changes and trauma in patients with upper ureteral calculi

Objective: To investigate the effects of retrograde intrarenal surgery (RIRS) and Mini-percutaneous nephroscope lithoipsy (Mini-PCNL) on liver and kidney function, endocrine changes and trauma in patients with upper ureteral calculi. Methods: 100 patients with upper ureteral calculi admitted to our hospital from March 2016 to February 2018 were selected. They were randomly divided into observation group (RIRS group) and control group (Mini-PCNL group), with 50 cases in each group. The observation group was given RIRS, while the control group was given Mini-PCNL. At the same time, the expression levels of the liver function [including alanine aminotransferase (ALT), aspartate aminotransferase (AST) and gamma transpeptidase (gamma-GT)], renal function [including urea nitrogen (BUN) and serum creatinine (Scr) levels], endocrine changes [including corticotropin (ACTH), cortisol (Cor), norepinephrine (NE)] and trauma related indexes [Including soluble vascular cell adhesion molecule-1 (sVCAM-1) and erythrocyte sedimentation rate (ESR)] were analyzed and compared between the two groups. Results: There was no significant difference in ALT, AST, γ-GT, BUN and Scr before and after operation between the two groups. The difference was not statistically significant (P>0.05). After operation, the levels of ACTH (27.37±3.29) pg/mL, Cor (150.09±18.47) ng/mL, NE (165.48±26.74) ug/L and sVCAM-1, (596.55±56.24) ng/mL in RIRS group were significantly lower than those in Mini-PCNL group ((38.42±4.33) pg/mL, (222.37±28.70) ng/mL, (287.26±25.29) ug/L and (820.62±72) mL)The differences were statistically significant (P<0.05). While the ESR level in RIRS group (8.29±0.63) mm/h was significantly higher than that in Mini-PCNL group (7.16±0.68) mm/h, and the differences were statistically significant (P<0.05). Conclusions: There is no significant difference in the liver and renal function between RIRS and Mini-PCNL in the treatment of upper ureteral calculi. Objective: To explore the effects of Ticagrelor on oxidative stress, coagulation function, platelet function and related factors in patients with coronary artery disease undergoing interventional therapy. Methods: A total of 140 patients with coronary artery disease who underwent percutaneous coronary intervention in our hospital from October 2016 to March 2018 were selected as the study subjects and were divided into control group (70 cases) and observation group (70 cases) by drawing lots. Both groups were treated with symptomatic routine therapy before operation. On this basis, the control group was treated with clopidogrel before operation, and the observation group was treated with Ticagrelor before operation on the basis of routine treatment. The changes of oxidative stress, coagulation index, platelet function and related factors were compared between the two groups before and after treatment. Results: Before treatment, there were no significant differences in oxidative stress factors, coagulation parameters, platelet parameters and related factors factors between the two groups. After treatment, the levels of MDA, MPAR, VEGF and MMP-9 in the two groups were lower than those before treatment, while the levels of SOD, APTT, TT and PT were higher than those before treatment;and the levels of MDA, MPAR, VEGF, MMP-9, APTT and PT in the observation group were significantly lower than those in the control group after treatment, while the levels fo SOD, TT and IPA in the observation group were significantly higher than those in the control group. Conclusions: Ticagrelor can better reduce oxidative stress injury, improve coagulation function and coronary stenosis, and inhibit platelet aggregation in patients with coronary heart disease undergoing PCI than clopidogrel. Objective: To investigate the effects of Liraglutide on endothelial function, immune function and related factors in patients with type 2 diabetes mellitus. Methods: A total of 160 patients with type 2 diabetes mellitus who received treatment in department of endocrinology of our hospital from December 2015 to June 2018 were collected. They were randomly divided into the control group and the observation group, 80 cases in each group. The patients in the control group were treated with metformin. The patients in the observation group were treated with liraglutide on the basis of the control group. The levels of endothelin (ET-1), nitric oxide (NO), immunoglobulin A (IgA), immunoglobulin M (IgM), immunoglobulin G (IgG), hypersensitive C-reactive protein (hs-CRP), insulin-like growth factor-1 (IGF-1), tumor necrosis factor- (TNF-α), Vaspin and HbAlc were detected and compared. Results: After treatment, compared with the treatment group, the serum levels of ET-1, hs-CRP, IGF-1, TNF-α and HbAlc of patients in the control group and the observation group were significantly decreased, and the levels of NO, IgA, IgG, IgM and Vaspin increased significantly. The change trend of the above indexes of patients in the observation group was more obvious than those in the control group (P<0.05). Conclusion: Liraglutide combined with metformin in the treatment of type 2 diabetes mellitus can significantly enhance endothelial function and immune function of patients, and alleviate inflammation, and it has good clinical efficacy. 展开更多

Objective: To evaluate the effect of tai chi exercise on cardiac and static lung function for older community-dwelling adults at risk of ischemic stroke. Methods: A total of 170 older community-dwelling adults (aged 55–75 years old) at risk of ischemic stroke were allocated to either tai chi training group (85 cases, five 60-min sessions of tai chi training per week for 12 weeks) or control group (85 cases, usual pbysical activity for 12 weeks) using a computer-generated randomization. The echocardiographic parameters of cardiac structure, cardiac function and static lung function were measured at baseline, after 12 weeks of intervention and additional 12-week follow-up period by a blinded professional staff member using a color Doppler ultrasound imaging device or a cardiopulmonary function instrument. The t test and linear mixed model based on the intention-to-treat analysis principle was used to calculate the effect. The adverse effect was observed. Results: Most of echocardiographic parameters on the cardiac structure, cardiac function and static lung function between the tai chi group and control group did not have a significant difference either post 12-week intervention or additional 12-week follow-up period. Only three parameters involving in right ventricular diameter (P=0.024), main pulmonary artery diameter (P=0.002) and vital capacity maximum (P=0.036) were beneficial to be improved in the tai chi group compared to the control group by the analysis of mixed linear model. No adverse effects were found during the intervention period. Conclusions: The 12-week tai chi exercise did not have an obvious beneficial effect on cardiac structure, cardiac function and static lung function in older community-dwelling adults at risk of ischemic stroke.(Trial registration No. ChiCTR-TRC-13003601) 展开更多

Objective:To explore the effects of combining application of diagnosis and treatment technologies of Event Related Potentials(ERP)and Psychic Rehabilitation Intervention(PRI)on rehabilitation of schizophrenia.Methods:PRI and ERP were used in schizophrenia rehabilitation in this research.232 patients were divided into study group(SG)and control group(CG),and the rehabilitation effects were compared.Results:(1)After the implementation of the PRI,more score of the Brief Psychiatric Rating Scale(BPRS)was reduced on SG than the score on CG;more decreased scores of insight and on Social Disability Screening Schedule(SDSS),and more increased scores of treatment compliance questionnaire scores were observed on SG,comparing with CG(P<0.05 or 0.01).(2)After the implementation of the PRI,more decreased factor scores of Symptom Checklist 90 were observed on SG patients family members(P<0.05 or 0.01).(3)PRI significantly improved the patients’social functions,family members’relationship and prevented the recurrence of schizophrenia.(4)Improved P2,P3 amplitudes were also detected on SG compared with CG(P<0.05).Conclusions:(1)PRI interventions may improve schizophrenia patients’social function.(2)ERP may be used to evaluate the schizophrenia patients’rehabilitation,and is worth being popularized. 展开更多

We present optical luminosity functions(LFs) of galaxies in the0.1 g,0.1 r,0.1 i bands, calculated using data in40 deg2 sky area of the LAMOST Complete Spectroscopic Survey of Pointing Area(LaCoSSPAr) in the Southern Galactic Cap. Redshifts for galaxies brighter than r = 18.1 were obtained mainly with LAMOST. In each band, LFs derived using both parametric and non-parametric maximum likelihood methods agree well with each other. In the0.1 r band, our fitting parameters of the Schechter function are φ*=(1.65 ± 0.36) × 10-2 h3 Mpc-3, M*=-20.69 ± 0.06 mag and α =-1.12 ± 0.08,which agree with previous studies. Separate LFs are also derived for emission line galaxies and absorption line galaxies. The LFs of absorption line galaxies show a dip at0.1 r 18.5 and can be fitted well by a double-Gaussian function, suggesting a bimodality in passive galaxies. 展开更多

Objective: Analyze the effect of argatroban on neurological function in patients with acute cerebral infarction and explore its possible mechanisms. Methods: From August 2015 to August 2017, 140 patients with acute cerebral infarction who were admitted to the Department of Neurology in our hospital were divided into a control group and an observation group according to the lottery method, with 70 cases in each group. Patients in the conventional group received routine treatment. Patients in the treatment group received routine treatment plus argatroban. The changes of neurological function, coagulation function and inflammatory factors were observed in the two groups. Result: Before treatment, there were no significant differences in neurological function parameters, coagulation function indexes and inflammatory factors between the two groups. After treatment, the neurological function indexes NT-proBNP, NPY and S-100β levels, coagulation function index FIB level, inflammatory factor indicators MMP-9, Lp-PLA2 and Hcy, vascular endothelial function index ET levels decreased, coagulation Functional indicators PT, TT and APTT are both elevated, and NO and CGRP levels are elevated. The levels of NT-proBNP, NPY and S-100β, FIB level, MMP-9, Lp-PLA2 and Hcy, ET levels in the treatment group were lower than those in the conventional group, while the levels of PT, TT and APTT levels, and NO and CGRP levels were higher than the conventional group. Conclusion: Argatroban treatment can significantly improve neurological function in patients with acute cerebral infarction. The possible mechanism is to improve coagulation function, vascular endothelial function and relieve inflammatory stress response. 展开更多

Quantitative assessment of the recovery of nerve function, especially sensory and autonomic nerve function, remains a challenge in the field of nerve regeneration research. We previously found that neural control of vasomotor activity could be potentially harnessed to evaluate nerve function. In the present study, five different models of left sciatic nerve injury in rats were established: nerve crush injury, nerve transection/ suturing, nerve defect/autografting, nerve defect/conduit repair, and nerve defect/non-regeneration. Laser Doppler perfusion imaging was used to analyze blood perfusion of the hind feet. The toe pinch test and walking track analysis were used to assess sensory and motor functions of the rat hind limb, respectively. Transmission electron microscopy was used to observe the density of unmyelinated axons in the injured sciatic nerve. Our results showed that axonotmesis-evoked vasodilatation in the foot 6 months after nerve injury/repair recovered to normal levels in the nerve crush injury group and partially in the other three repair groups;whereas the nerve defect/non-regeneration group exhibited no recovery in vasodilatation. Furthermore, the recovery index of axonotmesis-evoked vasodilatation was positively correlated with toe pinch reflex scores and the density of unmyelinated nerve fibers in the regenerated nerve. As C-fiber afferents are predominantly responsible for dilatation of the superficial vasculature in the glabrous skin in rats, the present findings indicate that axonotmesis-evoked vasodilatation can be used as a novel way to assess C-afferent function recovery after peripheral nerve injury. The control group was treated with flunarizine hydrochloride, while the observation group was treated with gastrodin based on the control group. The levels of neurotransmitter effect[including 5-serotonin (5-HT) and tumor necrosis factor alpha (TNF-alpha)], stress response[oxidized low density lipoprotein (ox-LDL) and paraoxonase-1 (PON-1)], vascular function[including nitric oxide (NO), endothelin-1 (ET-1)] and neurological function[including high brain-derived neurotrophic factor (BDNF) and calcitonin gene-related peptide (CGRP)] related indexes were observed and compared between the two groups.Results: The two groups showed significant changes in transmitter effect, stress response, blood vessel and nerve function. Before treatment, there was no significant difference in the levels between the two groups. After treatment, the levels of TNF-a, ox-LDL, NO, ET-1, BDNF and CGRP in the two groups were significantly lower than those before treatment, while the levels of 5-HT and PON-1 were significantly higher than those before treatment. And after treatment, the levels of TNF-a, ox-LDL, NO, ET-1, BDNF and CGRP in the observation group were significantly lower than those in the control group, while the levels of 5-HT and PON-1 were significantly higher than those in the control group.Conclusions:Gastrodin treatment can regulate oxidative stress level and improve vascular and neurological function in migraine patients. 展开更多

The probability distributions of wave characteristics from three groups of sampled ocean data with different significant wave heights have been analyzed using two transformation functions estimated by non-parametric and parametric methods. The marginal wave characteristic distribution and the joint density of wave properties have been calculated using the two transformations, with the results and accuracy of both transformations presented here. The two transformations deviate slightly between each other for the calculation of the crest and trough height marginal wave distributions, as well as the joint densities of wave amplitude with other wave properties. The transformation methods for the calculation of the wave crest and trough height distributions are shown to provide good agreement with real ocean data. Our work will help in the determination of the most appropriate transformation procedure for the prediction of extreme values. 展开更多

The expression levels of thyroid function [including thyroid stimulating hormone (TSH), free triiodothyronine (FT3) and free thyroxine (FT4)], immune response [including thyroid peroxidase antibody (TPOAb), thyroglobulin antibody (TGAb), interferon-γ (IFN-γ), interleukin-4 (IL-4)] and oxidative stress [including malondialdehyde (MDA), glutathione peroxidase (GPx) and superoxide dismutase (SOD)] were observed and compared between the two groups.Results: The two groups showed significant changes in auxiliary diagnosis, immune response and oxidative stress. Before treatment, there was no significant difference in the levels between the two groups. After treatment, there was no significant difference in the above indexes between the control group and that before treatment. There was no significant difference in thyroid function related indexes between the treatment group and the control group. The levels of TPOAb, TGAb, IFN-γ and MDA in the treatment group were significantly lower than those before treatment and in the control group, while the levels of IL-4, GPx and SOD were significantly higher than those before treatment and in the control group.Conclusions: Selenium supplementation can stabilize thyroid function, reduce the level of thyroid antibody production, and improve immune response and oxidative stress response in HT patients. 展开更多

Correlation analysis of real-time three-dimensional ultrasound in diagnosis of left ventricular function, portal hemodynamics and severity of liver function in patients with liver cirrhosis

Objective:To analyze the correlation between real-time three-dimensional ultrasound in diagnosis of left ventricular function, portal hemodynamics and severity of liver function in patients with liver cirrhosis.Methods: 90 patients with cirrhosis admitted to our hospital from January 2017 to December 2018 were enrolled in the cirrhosis group, and the cirrhosis components were group A, B and C according to the Child-Pauh classification criteria. During the same period, 90 healthy subjects who underwent physical examination were selected as the control group;real-time three-dimensional ultrasonography was performed to examine left ventricular function and portal hemodynamic parameters;Pearson correlation was used to analyze left ventricular function, portal hemodynamics and liver. Functional severity relationship.Results: The left heart E and E/A levels in the cirrhosis group were significantly lower than those in the control group (P<0.05), and the A level was significantly higher than the control group (P<0.05). The Dpv and Q levels in the cirrhosis group were significantly higher than those in the healthy control group. The difference was statistically significant (P<0.05), and the level of Vpv was significantly lower than that of healthy controls (P<0.05). There were significant differences in E, A and E/A levels between different Child-Paugh patients (P<0.05). There were significant differences in portal hemodynamics Dpv, Vpv and Q between the different Child-Paugh grades, and the difference was statistically significant (P<0.05);left heart function E and E/A and liver function severity There was a significant negative correlation (P<0.05). There was a significant positive correlation between left cardiac function A and liver function severity (P<0.05). Portal vein hemodynamics Dpv, Vpv and QE were significantly associated with liver function severity. Positive correlation (P<0.05). Objective: To explore the value of serum PCT content for evaluating inflammatory factor release and organ function injury in neonatal septicemia. Methods: 48 children who were diagnosed with neonatal septicemia in our hospital between March 2015 and May 2018 were selected as the septicemia group, and 50 healthy neonates delivered in our hospital during the same period were selected as the normal control group. The differences in PCT and inflammatory factor contents in serum as well as liver and kidney function index levels in peripheral blood were compared between the two groups. Pearson test was used to evaluate the correlation of serum PCT content with inflammatory factor release as well as liver and kidney function injury in children with neonatal septicemia. Results: The serum PCT content of septicemia group was significantly higher than that of normal control group;serum hypersensitive C-reactive protein (hs-CRP), interleukin-6 (IL-6), interleukin-8 (IL-8) and soluble intercellular adhesion molecule-1 (sICAM-1) contents were higher than those of normal control group;peripheral blood aspartate aminotransferase (AST), alanine aminotransferase (ALT), γ-glutamyl transpeptidase (γ-GGT) and alkaline phosphatase (ALP) levels were higher than those of normal control group;peripheral blood blood urea nitrogen (BUN), serum creatinine (Scr) and serum β2-microglobulin (β2-M) levels were higher than those of normal control group (P<0.05). Correlation analysis showed that serum PCT content in children with neonatal septicemia was directly correlated with the degree of inflammatory response as well as the degree of liver and kidney function injury (P<0.05). Conclusion: The serum PCT content abnormally increases in children with neonatal septicemia, and the specific content is consistent with the disease severity and has certain clinical value for the judgment of children's condition. 展开更多

This paper proposes an optimization algorithm based on a multi-loop control system with a neural network controller,in which the objective function that is used is the control plant of each sub-control system.To obtain the global optimization solution from a control plant that has many local minimum points,a transformation function is presented.On the one hand,this approach changes a complex objective function into a simple function under the condition of an unchanged globally optimal solution,to find the global optimization solution more easily by using a multi-loop control system.On the other hand,a special neural network(in which the node function can be simply positioned locally)that is composed of multiple transformation functions is used as the controller,which reduces the possibility of falling into local minimum points.At the same time,a filled function is presented as a control law;it can jump out of a local minimum point and move to another local minimum point that has a smaller value of the objective function.Finally,18 simulation examples are provided to show the effectiveness of the proposed method. 展开更多

Traditionally medicinal plants are used for its potential chemotherapeutic action and for safety upon the continued use of these plants.Achillea millefolium(AM)is an ancient herb which is used to treat wounds,headaches,disorder in gastrointestinal and hepatobiliary,pain,and inflammation.This study was aimed to investigate the biochemical parameter of antimutagenic methanolic extract for AM in vivo.Plant extract together with MTX decreased the damage caused by the drug on the concentrations of total cholesterol and triglycerides(160.3±2.26 and 108.7±2.71),significant increase in total protein concentration(8.51±0.17,9.46±0.19 and 10.8±2.71 g\dL)for 100 and 200 mg\kg was observed.Also plant extracts counteract the damage caused by drug through the reduction for both creatinin and albumin concentrations to 1.57±0.03 mg\dL and 4.56±0.09 g\dL,respectively.On enzymatic level,the results of interactions indicated the ability of plant to modulating harmful effects of the drug(57.1±1.3,130±3.1 and 111.1±2.1 U\L)for GOT,GPT and ALP,respectively.On the other hand,the plant possesses the ability to enhance the concentrations of all immunoglobulin(IgM,IgA and IgG)in the two doses tested.Therefore,the results of this study indicate the antimutagenic potential of AM extract and encouraging its consumption with MTX as one of the promising therapies. 展开更多
