Macular retinal thickness and flow density change by optical coherence tomography angiography after posterior scleral reinforcement

Optical coherence tomography angiography(Angio-OCT) has introduced a new non-invasive,quantitative method to assess superficial and deep capillary networks of the retina.In this study,we investigated macular retinal thickness and flow density change following posterior scleral reinforcement(PSR) surgery,using an RTVue XR Avanti Angio-OCT,in patients with pathological myopia.A total of 13 patients with pathological myopia were recruited and all patients completed the 6 months follow-up visit.Data from 22 eyes were used in this study.The mean age was 36.23±15.29 years,and 43%(n=6) were men.Spherical equivalent refractive error(SE) ranged from –8.0 to –24.0 D.Postoperative axial length,best-corrected visual acuity and SE did not change significantly at each follow-up,compared with pre-operative measures(P>0.05).Postoperative flow density of superficial and deep retinal layers at each sector did not change significantly at each follow-up,compared with preoperative measures(P>0.05).However,we found significant decrease in retinal thickness of parafovea-inferior sector after PSR surgery(P<0.01),indicating potential relaxation of vitreofoveal traction after PSR surgery. 展开更多

The macular microstructure repair and predictive factors of surgical outcomes after vitrectomy for idiopathic macular hole

To evaluate the macular microstructure repair and explore the factors related to those changes and visual improvement after vitrectomy for idiopathic macular hole(IMH). Totally 19 eyes of 18 IMH patients who underwent macular hole(MH) surgery were evaluated with bestcorrected visual acuity(BCVA) and spectral-domain optical coherence tomography(SD-OCT) images. All 19 eyes closed at 6 mo postoperatively. BCVA was observed gradually improved(P<0.001), with subretinal fluid(SRF) gradually absorbed(P=0.021) and the rate of external limiting membrane(ELM) defects gradually decreased(P=0.011) with follow-up time. Poorer postoperative logMAR BCVA correlated with larger MH minimum diameter(P<0.001), larger MH basal diameter(P=0.008), longer symptom duration(P=0.002) and poorer preoperative logMAR BCVA(P=0.010). More improvement in BCVA correlated only with poorer preoperative in logMAR BCVA(P=0.002). The earlier reconstruction of ELM was associated with smaller MH basal diameter(P=0.022) and shorter symptom duration(P=0.008). In conclusion, smaller basal diameter of MH and shorter symptom duration were key factors in earlier reconstruction of ELM. 展开更多

目的比较雷珠单抗与曲安奈德联合黄斑区格栅样光凝治疗视网膜分支静脉阻塞继发黄斑水肿(branch retinal vein occlusion secondary macular edema, BRVO-ME)的效果方法回顾性研究视网膜分支静脉阻塞继发黄斑水肿40例(40眼)的临床资料随... 目的比较雷珠单抗与曲安奈德联合黄斑区格栅样光凝治疗视网膜分支静脉阻塞继发黄斑水肿(branch retinal vein occlusion secondary macular edema, BRVO-ME)的效果方法回顾性研究视网膜分支静脉阻塞继发黄斑水肿40例(40眼)的临床资料随机分为2组,其中20例(20眼)接受玻璃体内注射雷珠单抗;另20例(20眼)接受玻璃体内注射曲安奈德(2 mg,0.05 ml)c两组均于玻璃体注药后1周行黄斑格栅样激光光凝,结果两组治疗后各时间点的BCVA较术前均明显提升,黄斑中心区厚度(central macular retinal thickness, CMT)下降(P<0.05),治疗后各时间点两组之间比较差别无统计学意义(P〉0.05)。两组治疗后1个月眼压比较:曲安奈德组高于雷珠单抗组(PV0.05);治疗后3个月两组差异无统计学意义(P>0.05)。结论黄斑区格栅样光凝联合雷珠单抗和联合曲安奈德都是治疗BRVO-ME的有效手段,均可提高BCVA及降低CMT且疗效稳定。联合雷珠单抗者术后眼压稳定,较安全。 展开更多

目的评价玻璃体内注射雷珠单抗联合曲安奈德或视网膜光凝治疗视网膜静脉阻塞(RVO)所致黄斑水肿的临床效果。方法回顾性分析2017年10月至2018年4月RVO患者97例(97只眼)的临床资料,其中视网膜分支静脉阻塞(BRVO)组50例(50只眼)、视网膜中... 目的评价玻璃体内注射雷珠单抗联合曲安奈德或视网膜光凝治疗视网膜静脉阻塞(RVO)所致黄斑水肿的临床效果。方法回顾性分析2017年10月至2018年4月RVO患者97例(97只眼)的临床资料,其中视网膜分支静脉阻塞(BRVO)组50例(50只眼)、视网膜中央静脉阻塞(CRVO)组47例(47只眼)。观察比较治疗前后的视力、FFA及中央视网膜厚度(CRT)的改变。治疗后随访6个月。结果BRVO组治疗前视力(LogMAR)为0.636±0.471,治疗后升至0.368±0.235,治疗前CRT为(443.48±156.23)μm,治疗后降至(220.21±20.31)μm。CRVO组治疗前视力(LogMAR)为0.861±0.531,治疗后升至0.522±0.443,治疗前CRT为(549.11±225.30)μm,治疗后降至(249.36±40.13)μm。比较治疗前后,差异均有统计学意义(P<0.05)。两组均未发现眼部或全身严重的不良反应。结论雷珠单抗玻璃体内注射联合曲安奈德或视网膜光凝治疗RVO可促进视网膜水肿消退,视力改善。 展开更多

Dopamine and its receptors have been widely studied in the neurological conditions and in the retina. In this study, we evaluated the possible role of dopamine in rhegmatogenous retinal detachment(RRD) by comparing the amount of 3,4-dihydroxyphenylacetic acid(DOPAC), a surrogate index of retinal dopamin levels, in the vitreous sample of patients affected by RRD with those affected by macular pucker and vitre ous hemorrhage. Our results showed that significantly higher levels of DOPAC were found in the vitreou sample of patients affected by RRD compared with those affected by vitreous hemorrhage and macula pucker(P = 0.002). Specifically, no trace of the substance was found in vitreous hemorrhage and macula pucker samples. A slightly significant positive correlation was found among DOPAC and post-operativ best corrected visual acuity(r = 0.470, P = 0.049). No correlation was found between DOPAC and the day elapsed between diagnosis and surgery(P = 0.317). For the first time our findings suggest that DOPAC i released in RRD, but not in other retinal diseases such as vitreous hemorrhage and macular pucker. More over, we showed a correlation between visual acuity outcome and the amount of DOPAC in the vitreous This might have a potential, although still unknown, implication in the pathogenesis of the disease and/o in the associated photoreceptors loss. Increased serum levels of soluble CD146 and vascular endothelial growth factor receptor 2 in patients with exudative age-related macular degeneration

AIM: To investigate serum levels of soluble CD146(s CD146) and vascular endothelial growth factor receptor 2(VEGFR2) in patients with age-related macular degeneration(AMD). METHODS: Eighty-eight patients with exudative AMD and 45 sex-and age-matched healthy controls were enrolled in this study conducted in China. Serum samples was obtained from the patients with exudative AMD and from the controls. Serum sCD146 and VEGFR2 protein levels were measured using an enzyme-linked immunosorbent assay.RESULTS: We found that serum sCD146 and VEGFR2 protein levels were significantly higher in the patients with exudative AMD group than in the controls(t=3.859, P<0.001 and t=3.829, P<0.001, respectively). Serum sCD146 levels were significantly higher in patients with classic choroidal neovascularization(CNV) than in those with occult CNV(t=9.899, P<0.001). There was a significant difference in the trend for exudative AMD in the highest versus lowest quartile of circulating sCD146 levels(χ2=10.29, P=0.001). The receiver operating characteristic curve analysis showed that the area under the curve was 0.696 for s CD146(95%CI: 0.601-0.791) with an optimum diagnostic cut-off value of 157.16 ng/mL, a sensitivity of 55.7%, and a specificity of 82.2%.CONCLUSION: The serum sCD146 level increases and may be a biomarker for exudative AMD. 展开更多

The aim of this study was to explore the role of autophagy in response to blue light damage in aged mice and in human retinal pigmented epithelium(hRPE) cells. Blue light damage to the retina was induced in 10-month-old(10 mo) C57 mice and hRPE cells. Flash electroretinography was used to assess retinal function. Retinal structure changes were observed by electron microscopy. Western blot was conducted to determine the expression levels of the following proteins: cleaved caspase-3, p38 mitogen-activated protein kinases, protein kinase R-like endoplasmic reticulum kinase(PERK), autophagy marker light chain 3(LC3), P62, and Beclin-1. On day 1 after light damage to the 10 mo mice, retinal function was changed. The latent periods of awave and b-wave were delayed, and amplitude was reduced. The electron microscopy results revealed mitochondria damage in the retinal pigmented epithelium and a disorganized photoreceptor outer segment(OS). PERK, LC3, and Beclin-1 were upregulated, whereas P62 was not. On day 5 after the blue light damage, restoration of electroretinography and OS was observed.PERK, LC3, and Beclin-1 were downregulated, whereas P62 was not. Protein changes in vitro were consistent with in vivo. The present study provided structural and functional evidence that autophagy plays an important role in the response to blue lightinduced retinal damage. 展开更多

Cystoid macular edema(CME) is the abnormal collection of intraretinal fluid in the macular region, especially in the inner nuclear and outer plexiform layers. CME leads to severe visual impairment in patients with various retinal diseases, such as diabetic retinopathy, retinal vascular occlusion, choroidal neovascularization, and uveitis. Although various retinal conditions lead to CME, a shared pathogenesis of CME is involved in these diseases. Accordingly, the pathogenesis of CME based on vasogenic mechanisms is first discussed in this review, including vascular hyperpermeability, leukostasis, and inflammation. We then describe cytotoxic mechanisms based on retinal Müller cell dysfunction. This comprehensive review will provide an understanding of the pathogenesis of CME for potential therapeutic strategies. 展开更多

目的通过分析屈光参差性弱视儿童黄斑厚度与视盘周围视网膜神经纤维层(RNFL)厚度的相关性,探讨屈光参差性弱视儿童视网膜结构变化。方法选取未接受弱视治疗的30例屈光参差性弱视儿童的弱视眼作为弱视组,同期性别、年龄匹配的50名正视儿... 目的通过分析屈光参差性弱视儿童黄斑厚度与视盘周围视网膜神经纤维层(RNFL)厚度的相关性,探讨屈光参差性弱视儿童视网膜结构变化。方法选取未接受弱视治疗的30例屈光参差性弱视儿童的弱视眼作为弱视组,同期性别、年龄匹配的50名正视儿童右眼作为正常对照组。采用光学相干断层成像技术(OCT)检查患儿黄斑各区及视盘周围各区域RNFL厚度,采用Littmann公式对RNFL厚度进行修正。比较两组各区黄斑厚度及视盘周围各区域RNFL厚度的差异,采用Pearson相关分析两组各区黄斑及视盘周围各区域RNFL厚度的相关性。结果两组黄斑各区厚度比较差异均无统计学意义(均P >0.05),校正前后两组视盘周围各区域RNFL厚度比较差异均无统计学意义(均P >0.05)。正常对照组黄斑各区厚度与校正后视盘周围各区域RNFL厚度均无相关性(均P >0.05)。弱视组上方RNFL厚度与黄斑中央区厚度呈负相关(P<0.05),下方和鼻侧RNFL厚度与上方、下方、鼻侧、颞侧黄斑厚度均呈正相关(均P<0.05),颞侧RNFL厚度与鼻侧、颞侧黄斑厚度呈负相关(均P<0.05)。结论屈光参差性弱视儿童黄斑厚度和视盘周围RNFL厚度存在相关性,提示其视网膜可能存在异常重构。分析两者相关性或许能应用于弱视的诊治。 展开更多

目的评价玻璃体切除联合内界膜剥离及空气填充术治疗特发性黄斑孔的临床效果。方法回顾性分析2016年12月至2018年1月收治的特发性黄斑孔38例(38眼)的临床资料,均行25G玻璃体切除切除联合内界膜剥离及空气填充术治疗。术后随访3个月,观... 目的评价玻璃体切除联合内界膜剥离及空气填充术治疗特发性黄斑孔的临床效果。方法回顾性分析2016年12月至2018年1月收治的特发性黄斑孔38例(38眼)的临床资料,均行25G玻璃体切除切除联合内界膜剥离及空气填充术治疗。术后随访3个月,观察患眼术后视力和黄斑孔闭合情况。结果至末次随访,38例中黄斑孔完全闭合28例,暴露性闭合5例,闭合率86.84%(33/38)。术后3个月黄斑孔未闭合5眼均较术前显著缩小,差异有统计学意义(t=3.680,P=0.010)。手术前后视力(BCVA,logMAR)分别为1.15±0.31和1.03±0.36,差异有统计学意义(t=2.076,P=0.041)。结论对特发性黄斑孔采用玻璃体切除切除联合内界膜剥离及消毒空气填充术治疗可获得良好疗效。 展开更多

目的评价25 G玻璃体切除术中空气填充治疗特发性黄斑孔的效果。方法回顾性对照研究。本院2016年1月至2018年3月62例(62眼)特发性黄斑孔,分为对照组和观察组各31例。全部患者均行常规的三通道25 G玻璃体切除术联合内界膜剥除术,进行气液... 目的评价25 G玻璃体切除术中空气填充治疗特发性黄斑孔的效果。方法回顾性对照研究。本院2016年1月至2018年3月62例(62眼)特发性黄斑孔,分为对照组和观察组各31例。全部患者均行常规的三通道25 G玻璃体切除术联合内界膜剥除术,进行气液交换后,对照组填充全氟丙烷(C3F8),观察组填充消毒空气。术后随访观察3个月。结果两组术后裂孔闭合率比较,差异无统计学意义(P=0.895),观察组患者术后的俯卧时间显著短于对照组(P=0.011),两组术后3个月视力及并发症发生率差异无统计学意义(P=0.487,0.688)。结论特发性黄斑孔25 G玻璃体切除术采取消毒空气填充较C3F8吸收快,能够显著减少患者术后的俯卧时间,安全性良好。 展开更多

目的分析特发性黄斑前膜术后黄斑区结构改变对视力的影响。方法采用回顾性病例研究。收集2016年1月至2017年12月本院玻璃体切除联合黄斑前膜剥除术的特发性黄斑前膜49例(49眼)。术后随访6个月以上,观察手术前后最佳矫正视力(BCVA)、黄... 目的分析特发性黄斑前膜术后黄斑区结构改变对视力的影响。方法采用回顾性病例研究。收集2016年1月至2017年12月本院玻璃体切除联合黄斑前膜剥除术的特发性黄斑前膜49例(49眼)。术后随访6个月以上,观察手术前后最佳矫正视力(BCVA)、黄斑中心区厚度(CMT)、中心凹区光感细胞层厚度(CPRT)、椭圆体区(EZ)完整性的变化。对术后6个月时视力与CMT、CPRT及EZ完整性的相关性进行探讨。结果与手术前相比,术后各时间点BCVA均提高(F=8.114,P<0.05),CMT和CPRT变薄,差异均有统计学意义(F=28.657、10.530,P<0.05)。术后BCVA与术后CMT、术后CPRT及术前BCVA呈正相关(r=0.340、0.742、0.566,P<0.05),与术后EZ完整性呈负相关(r=-0.628,P<0.05)。结论玻璃体切除术治疗特发性黄斑前膜疗效确切,术后视力提高、黄斑水肿减轻。术后视力的恢复与术后黄斑区微结构密切相关。 展开更多

We evaluated the clinical efficacy of free internal limiting membrane(ILM) flap transplantation for the treatment of large macular hole over 500 μm in 42 consecutive patients. Quantified evaluation of the post-operative macular anatomy restoration was performed by spectral-domain optical coherence tomography in the 12 mo follow-up. The results showed 41 eyes achieved successful closure(97.6%). Postoperative best corrected visual acuity, ellipsoid layer, and external limiting membrane disruption were significantly improved at all follow-up time points. The central foveal thickness was significantly higher at 1 mo. We concluded that free ILM flap transplantation proves to be effective to achieve anatomical and functional improvement for the treatment of large macular hole. 展开更多

AIM: To study polymorphisms in promotor regions of tumor necrosis factor(TNF)-α TNF-863 A/C(rs1800630), TNF-308 A/G(rs1800629), and TNF-238 A/G(rs361525) in patients with age-related macular degeneration(AMD) and associations of complex TNF-α genotypes with AMD. METHODS: One hundred and two patients(82 women, 20 men;mean age 64.2±1.2 y) with AMD and 100 healthy age-and sex-matched controls(82 women, 18 men;60±1.4 y) were included in the study. All subjects were Caucasian, all subjects and their parents were inhabitants of Russia. Genomic DNA was obtained from EDTA-preserved blood using the standard phenol-chloroform method. Polymorphisms were detected by polymerase chain reaction followed by the restriction fragment length polymorphism method. The following TNF-α genotypes were studied: TNF-α-238 AA, GA, GG, TNF-α-308 AA, GA, GG, TNF-α-863 AA, CA, CC. RESULTS: Differences in TNF-α-863 and TNF-α-238 genotypes frequencies in patients with AMD and healthy controls were not found. The distribution of TNF-α-308 AA and TNF-α-308 GA genotypes was significantly different between the studied group and the controls [odds ratios(OR) =0.22, P=0.0287 and OR=2.91, P=0.0063, respectively]. TNF-863 CC/TNF-308 GA and TNF-308 GA/TNF-238 GG genotypes were associated with the increased risk of AMD(OR=2.48, P=0.0332 and OR=2.51, P=0.0187, respectively). Five genotypes combinations appeared to be protective. CONCLUSION: In the present study, single nucleotide polymorphisms and complex polymorphisms of one of the key inflammatory cytokines TNF-α, and a number of significant associations of these polymorphisms with AMD in Russian population have been shown. Complexanalysis of genotypes could be important in AMD risk factors detection and studying pathogenesis. 展开更多
