BACKGROUND Solitary fibrous tumor of the liver (SFTL) is a rare occurrence with a low number of cases reported in literature. SFTL is usually benign but, 10%-20% cases are reported to be malignant with a tendency to metastasize. The majority of malignant SFTL cases are associated with a paraneoplastic hypoglycaemia defined as Doege-Potter syndrome. Surgery is the best therapeutic treatment, however, long- life follow-up is recommended. CASE SUMMARY A 74-year-old man, was admitted to the emergency department after a syncopal episode with detection of hypoglycaemia resistant to medical treatment. The computed tomography revealed a solid mass measuring 15 cm of the left liver. An open left hepatectomy was performed with complete resection of tumor. Histopathological analyses confirmed a malignant SFTL. CONCLUSION Large series with long-term follow-up have not been published neither have clinical trials been undertaken. Consequently, the methodical long-term followup of surgically treated SFTLs is strongly recommended. 展开更多

BACKGROUND Solitary rectal ulcer syndrome(SRUS)is a rare rectal disorder characterized by bloody mucus in the stool,difficulty in defecation,pain,and anal swelling.To date,the etiology of this syndrome remains not well understood and the diagnosis is frequently confused with other disorders,making treatment a clinical challenge.CASE SUMMARY A 50-year-old woman presented to our hospital with a 40-d history of bloody mucus in the stool and anal swelling.SRUS was suspected.Rectoscopy revealed a large,severe ulcerous lesion.Histologically,the lesion was characterized as chronic ulcer without clear tumor cells,and the final diagnosis of SRUS was made.The patient was treated with Chinese medicine therapy,with administration of Tong Xie Yao Fang.After 3 wk of treatment,the symptoms improved significantly.At 2-mo follow-up,rectoscopy in a local hospital showed healed ulcer scars without obvious protrusion 3 cm from the anal verge.CONCLUSION Chinese medicine therapy represents a potential treatment of SRUS with predominant rectal bleeding,mucinous discharge,and anal swelling pain. 展开更多

Experimental investigation on the optical remote sensing images of internal solitary waves with a smooth surface 预览

The parameter inversion of internal solitary waves (ISWs) based on optical remote sensing images is a key work. A new approach is proposed and demonstrated for simulating the optical remote sensing images of ISWs with a smooth surface in the laboratory. An optical remote sensing simulation system used to detect ISWs is constructed by a two-dimensional ISW flume, a LED (light emitting diode) light source and two CCD (charge coupled device) cameras. The optical remote sensing images of the horizontal surface and ISWs propagation images of a vertical side are detected simultaneously, which aims to explore the response of optical remote sensing corresponding to ISWs with the smooth surface. The results show that during the propagation of ISWs, dark pattern images are obtained by CCD 1 camera. The characteristics of the dark patterns vary along with the incident angle of the light source. The characteristic parameters of the optical remote sensing images correspond to the wave factors of vertical profiles. The experiment also shows a positive correlation between the dark pattern width and the half wave width under different amplitudes of ISWs. The system has the advantages of clear phenomenon and high repeatability, which provides the scientific basis for quantitative investigation on imaging mechanism of ISW by optical remote sensing. 展开更多

A novel method for internal wave monitoring based on expansion of the sound speed profile 预览

For acoustic detection of internal waves,the core issue is to obtain the temporal and spatial distribution of the sound speed profile(SSP).In the inversion process,the SSP is usually expressed by a few parameters through expansion.However,information about internal waves may sometimes be hard to read directly from the inversion results.The aim of this paper is to characterize the internal waves directly though expansion coefficients.By deducing the dynamic equations of the internal waves,an orthogonal basis called the hydrodynamic normal modes(HNMs)can be extracted from a certain number of SSP samples.Unlike the existing widely used empirical orthogonal functions(EOFs),the HNMs have a more explicit physical meaning that is directly related to internal wave activity.The HNMs are then used to expand the SSP time series,and the expansion coefficients are derived.Eventually,information about internal waves can be read directly from the time derivative of the expansion coefficients of the first two modes.In this study,this method is applied to thermistor string profiles from the northern shelf of the South China Sea,where the SSP shows evident spatial and temporal variations due to internal waves.The results show that the SSP can be described approximately by the first two modes with adequate precision.The special oscillation structure of the time derivative of the expansion coefficients can be used to detect internal solitary waves.The expansion coefficients can also give information on internal solitary wave amplitude and width.According to theoretical and experimental analysis,it can be concluded that the internal waves monitoring method introduced in this paper is effective.The HNMs method is simple to apply and depends less on sample data than EOFs.It could be used as an efficient alternative to EOFs to expand the use of the SSP in highly variable areas,where internal waves are intensive. 展开更多

A total of 9 experimental conditions are carried out, including different incident wave heights and water depths. Numerical simulations are performed using a high-order finite-difference model solving Navier–Stokes (N–S) equations. The predicted water wave elevation, velocity and pressure show good agreement with experimental data, verifying the accuracy and capacity of the numerical model. Furthermore, parametric studies are conducted by numerical modelling to examine the effects of the geometrical features of submerged dike on hydrodynamic characteristics around the breakwater. 展开更多

Effects of air relief openings on the mitigation of solitary wave forces on bridge decks 预览

For inverted T-type bridge decks, the air entrapped in the chambers between adjacent girders could increase the wave forces and lead to the destruction of the bridge decks. This paper studies the effects of the air relief openings (ARO) on the mitigation of the solitary wave-induced forces on the bridge decks. Hydrodynamic experiments are conducted for three inverted T-type decks with four, five, and six girders with different wave properties and deck clearances. The open source computational fluid dynamics toolbox OpenFOAM is adopted to conduct numerical simulations for the effects of the AROs. Since the numerical results correlate well with the measurements, the mechanism of the wave-structure interaction can be revealed by the numerical flow fields. Furthermore, the relationship between the shape and the volume of the ARO and the wave forces on the bridge decks, as well as the contribution of each ARO to the effect of the wave force mitigation, is also obtained from the numerical results of the OpenFOAM. Experimental and numerical results demonstrate that the AROs could effectively reduce the vertical wave forces on the bridge decks. The effects of the AROs increase with the increase of the volume of the ARO, while the shape of the ARO has no effect on the reduction of the wave forces. 展开更多

A numerical study to a generalized Korteweg-de Vries(KdV)equation is adopted to model the propagation and disintegration of large-amplitude internal solitary waves(ISWs)in the South China Sea(SCS).Based on theoretical analysis and in situ measurements,the drag coefficient of the Chezy friction is regarded as inversely pro-portional to the initial amplitude of an ISW,rather than a constant as assumed in the previous studies.Numerical simulations of ISWs propagating from a deep basin to a continental shelf are performed with the generalized KdV model.It is found that the depression waves are disintegrated into several solitons on the continental shelf due to the variable topography.It turns out that the amplitude of the leading ISW reaches a maximum at the shelf break,which is consistent with the field observation in the SCS.Moreover,a dimensionless parameter defining the relative importance of the variable to-pography and friction is presented. 展开更多

目的探讨CT三维重建(CT-3DR)联合快速现场评价(ROSE)辅助径向超声支气管镜(R-EBUS)对肺外周孤立性结节(SPN)的诊断价值。方法回顾性分析南京市胸科医院2016年3月至2017年3月收治的SPN共176例患者的临床资料,依据不同检查方式分为EG组[R-... 目的探讨CT三维重建(CT-3DR)联合快速现场评价(ROSE)辅助径向超声支气管镜(R-EBUS)对肺外周孤立性结节(SPN)的诊断价值。方法回顾性分析南京市胸科医院2016年3月至2017年3月收治的SPN共176例患者的临床资料,依据不同检查方式分为EG组[R-EBUS+引导鞘(GS)]、CTE组(CT-3DR+R-EBUS)、RE组(ROSE+R-EBUS)和三联组(CT-3DR+R-EBUS+ROSE),比较各组对SPN的诊断率、并发症、检查时间,探讨CT-3DR+ROSE+R-EBUS三联法对SPN的诊断效能及其影响因素,以及ROSE对SPN的诊断意义。结果EG组、CTE组、RE组、三联组的SPN总体诊断率分别为70.5%、70.0%、69.0%、74.0%,各组间差异均无统计学意义(均P>0.05);EG组、CTE组、RE组和三联组的检查时间分别是(34.0±6.3)、(26.6±6.8)、(27.2±7.8)和(19.4±5.4)min,CTE和RE组均显著短于EG组(均P<0.001),三联组均显著短于其他三组(均P<0.001)。CTE组和三联组术前接受CT-3DR导航,其定位结果与镜下病灶所在目标支气管实际位置符合率分别是87.5%和90.0%(P>0.05)。各组病例中,SPN长径≥2cm者诊断率均显著高于<2cm者(均P<0.05);R-EBUS超声探头位于病灶内部者诊断率均显著高于探头位于病灶一侧或偏离病灶者(均P<0.05);在EG组与RE组,SPN与胸膜之间距离≥2cm者诊断率均显著高于<2cm者(均P<0.05),但CTE组和三联组中未见类似现象;SPN病灶密度与诊断率无明显相关(均P>0.05)。RE组和三联组接受ROSE操作,ROSE读片结果与HE组织病理的诊断符合率为82.6%,Κappa值为0.608,ROSE的诊断灵敏度为0.818,特异度为0.846,阳性预测值为0.931,阴性预测值为0.647。结论CT-3DR术前导航和术中ROSE有助于提升R-EBUS对SPN的诊断效能,CT-3DR+R-EBUS+ROSE的三联法对外周型SPN具有较好的诊断价值且能显著缩短检查时间。 展开更多

BACKGROUND Pathological manifestations of hepatic tumours are often associated with prognosis. Although surgical specimens (SS) can provide more information, currently, pre-treatment needle core biopsy (NCB) is increasingly showing important value in understanding the nature of liver tumors and even in diagnosis and treatment decisions. However, the concordance of the clinicopathological characteristics and immunohistochemical (IHC) staining between NCB and SS from patients with hepatic tumours were less concerned. AIM To introduce a more accurate method for interpreting the IHC staining results in order to improve the diagnostic value of hepatic malignancy in NCB samples. METHOD A total of 208 patients who underwent both preoperative NCB and surgical resection for hepatocellular carcinoma (HCC) or intrahepatic cholangiocarcinoma RESULTS Morphologically, the presence of compact tumour nests or a cord-like structure in NCB was considered the primary cause of misdiagnosis of HCC from ICC. The kappa statistic showed a moderate agreement in histomorphology (k = 0.504) and histological grade (k = 0.488) between NCB and SS of the tumours. A 4-tier (+++,++,+, and -) scoring scheme that emphasized the focal neoplastic cell immunoreactivity of tumour cells revealed perfect concordance of CK19, GPC3 and HepPar1 between NCB and SS (k = 0.717;k = 0.768;k = 0.633). Furthermore, with the aid of a binary classification derived from the 4-tier score, a high concordance was achieved in interpreting the IHC staining of the three markers between NCB and final SS (k = 0.931;k = 0.907;k = 0.803), increasing the accuracy of NCB diagnosis C (k = 0.987;area under the curve = 0.997, 95%CI: 0.990-1.000;P < 0.001). CONCLUSION These findings imply that reasonable interpretation of IHC results in NCB is vital for improving the accuracy of tumour diagnosis. Scale effect of ISWs loads on Floating Production Storage and Offloading(FPSO)is studied in this paper.The application conditions of KdV,eKdV and MCC ISWs theories are used in the numerical method.The depthaveraged velocities induced by ISWs are used for the velocity-inlet boundary.Three scale ratio numerical models λ=1,20 and 300 were selected,which the scale ratio is the size ratio of numerical models to the experimental model.The comparisons between the numerical and former experimental results are performed to verify the feasibility of numerical method.The comparisons between the numerical and simplified theoretical results are performed to discuss the applicability of the simplified theoretical model summarized from the load experiments.Firstly,the numerical results of λ=1 numerical model showed a good agreement with former experimental and simplified theoretical results.It is feasible to simulate the ISWs loads on FPSO by the numerical method.Secondly,the comparisons between the results of three scale ratio numerical models and experimental results indicated that the scale ratios have more significant influence on the experimental horizontal forces than the vertical forces.The scale effect of horizontal forces mainly results from the different viscosity effects associated with the model’s dimension.Finally,through the comparisons between the numerical and simplified theoretical results for three scale ratio models,the simplified theoretical model of the pressure difference and friction forces exerted by ISWs on FPSO is applied for large-scale or full-scale FPSO. 展开更多

Internal solitary waves(ISWs)of depression are commonly found in the coastal environment and are believed to re-suspend sediments in coastal regions where the waves break.In this research,the direct numerical simulation is used to study the scalar transport induced by the ISWs of depression propagating over a slope-shelf topography.The scalar in this paper is considered to represent the concentrations of very fine suspended solids or pollutants.Vortices are observed from the numerical results at the bottom boundary layer on the slope during the ISW shoaling process,resulting in a scalar transport.All in cident ISWs of depression are observed to produce a waveform inversion on the shelf.The scalar transport from a slope to a shelf is the consequence of the combined vortices at the bottom boundary layer and the overturning of the ISWs of depression,and the latter was commonly ignored in previous studies.This study shows that the ISW-induced scalar transport consists of the following four stages:the slip transport,the wash transport,the vortex transport,and the secondary transport.A dimensionless time scales of the four stages are calculated,and the beginning times of the wash transport and the secondary transport are found to be uncorrelated with the slope gradients,taking values of 1.26,4,respectively. 展开更多

目的:探讨MRI图像纹理分析在小脑半球原发性脑淋巴瘤和单发转移瘤鉴别诊断中的应用价值。方法:回顾性收集经手术病理证实的位于小脑半球的原发性中枢神经系统淋巴瘤(PCNSL)20例和实性单发转移瘤22例的增强MRI图像。采用纹理分析方法在增... 目的:探讨MRI图像纹理分析在小脑半球原发性脑淋巴瘤和单发转移瘤鉴别诊断中的应用价值。方法:回顾性收集经手术病理证实的位于小脑半球的原发性中枢神经系统淋巴瘤(PCNSL)20例和实性单发转移瘤22例的增强MRI图像。采用纹理分析方法在增强T1WI轴位图像上获取整个瘤体的平均值、中位数、标准差、峰度、熵值、偏度和异质性等定量参数,并进行统计学分析。结果:在纹理分析中,PCNSL组标准差、熵值、偏度及异质性明显低于转移瘤组,而峰度明显高于转移瘤组(均P<0.05);异质性参数鉴别诊断效能明显优于其它参数值,有较高的敏感性、特异性及准确性;对标准差、熵值、偏度、峰度及异质性5个纹理分析定量参数进行多参数联合分析,其鉴别效能较利用单个纹理分析定量参数鉴别的效能高。结论:纹理分析部分定量参数(标准差、熵值、偏度、峰度及异质性)有助于鉴别小脑半球PCNSL和单发转移瘤。 展开更多

An integrated analysis of internal solitary wave(ISW) observations obtained from two moorings over the continental slope in the northern South China Sea(SCS) leads to an assessment of the vertical heat transport of the ISWs. The clusters of ISW packets are phase-locked to the fortnightly cycle of the semidiurnal tide. The ISWs appear during large semidiurnal tides, and there is a period of 5–6 d when no ISWs are observed. The effect of the ISWs on the continental slope heat budget is observed. The ISWs can modify a local temperature field in which the temperature in the upper layer can be changed by O(100) °C after the ISWs passed the mooring. Both ISWinduced diffusion and ISW-induced advection contribute to the temperature variation. The estimates imply an average vertical heat flux of 0.01 to 0.1 MW/m~2 in the ISWs in the upper 500 m of the water column. The vertical heat transport ranges from 0.56 to 2.83 GJ/m~2 with a mean value of 1.63 GJ/m~2. The observations suggest that the vertical heat transport is proportional to the maximum vertical displacement. 展开更多

By using Darboux transformation, the new higher-order rogue wave solutions of the equation are well constructed. These solutions exhibit rogue waves on a multi-soliton background.Moreover, the dynamics of these solutions is graphically discussed. Our results would be of much importance in enriching and predicting rogue wave phenomena arising in nonlinear and complex systems. 展开更多

Propagation of solitary kinetic Alfven waves(KAWs)is investigated in small but finite/3(particle-to-magnetic pressure ratio)collisionless dense plasma whose constituents are non degenerate wann ions,and relativistic degenerate electrons and positrons.Through the use of reductive perturbation technique,Kortweg-de Vries equation is derived to obtain small amplitude localized wave solution of KAWs.The effects of plasma 0,positron concentration,electron relativistic degeneracy parameter,ion thermal temperature and obliqueness parameter on solitary KAWs are studied.The results of this theoretical investigation are aimed at elucidating characteristics of kinetic Alfven solitary waves in relativistic degenerate e-p-i plasmas found in dense astrophysical objects specifically neutron stars and white dwarfs. 展开更多

We herein reported a 27-year-old woman with a right renal mass for two years.She underwent laparoscopic partial nephrectomy.Immunohistochemical examination of the specimen confirmed the diagnosis of solitary fibrous tumor by revealing its positive staining for cluster of differentiation(CD)34,epithelial membrane antigen(EMA),B-cell lymphoma-2(Bcl-2)and CD99 in the tumor cells.No adjuvant treatment was carried out.The patient was in good health without local recurrence or metastasis during 2 years of follow-up.Laparoscopic partial nephrectomy for renal solitary fibrous tumor is an alternative treatment to radical nephrectomy.It can provide a good outcome.However,further follow-up and more cases of renal solitary fibrous tumor treated with laparoscopic partial nephrectomy are necessary to compare the oncological outcome with radical nephrectomy. 展开更多
