BACKGROUND Gastric cancer is the world’s third most lethal malignancy.Most gastric cancers develop through precancerous states of atrophic gastritis and intestinal metaplasia.Two staging systems,operative link for gastritis assessment(OLGA)and operative link on gastric intestinal metaplasia assessment(OLGIM),have been developed to detect high gastric cancer risk.European guidelines recommend surveillance for high-risk OLGA/OLGIM patients(stages III–IV),and for those with advanced stage of atrophic gastritis in the whole stomach mucosa.We hypothesize,that by combining atrophy and intestinal metaplasia into one staging named TAIM,more patients with increased gastric cancer risk could be detected.AIM To evaluate the clinical value of the OLGA,OLGIM,and novel TAIM stagings as prognostic indicators for gastric cancer.METHODS In the Helsinki Gastritis Study,22346 elderly male smokers from southwestern Finland were screened for serum pepsinogen I(PGI).Between the years 1989 and 1993,men with low PGI values(PGI<25μg/L),were invited to undergo an oesophagogastroduodenoscopy.In this retrospective cohort study,1147 men that underwent gastroscopy were followed for gastric cancer for a median of 13.7 years,and a maximum of 27.3 years.We developed a new staging system,TAIM,by combining the topography with the severity of atrophy or intestinal metaplasia in gastric biopsies.In TAIM staging,the gastric cancer risk is classified as low or high.RESULTS Twenty-eight gastric cancers were diagnosed during the follow-up,and the incidence rate was 1.72 per 1000 patient-years.The cancer risk associated positively with TAIM[Hazard ratio(HR)2.70,95%CI:1.09–6.69,P=0.03].The risk increased through OLGIM stages 0-IV(0 vs IV:HR 5.72,95%CI:1.03–31.77,P for trend=0.004),but not through OLGA stages 0–IV(0 vs IV:HR 5.77,95%CI:0.67–49.77,P for trend=0.10).The sensitivities of OLGA and OLGIM stages III–IV were low,21%and 32%,respectively,whereas that of TAIM high-risk was good,79%.On the contrary,OLGA and OLGIM had high specificity,8 展开更多

At present, the bearing capacity evaluation is mainly based on load detection, which requires closed traffic and has certain risks. With the increase of service time, the cracks of reinforced concrete beam bridge will gradually develop and the stiffness will reduce, resulting in the decrease of bearing capacity. Therefore, in this paper, the calculation of stiffness reduction coefficient by using crack characteristic parameters, which provides basic data for bearing capacity evaluation, has been studied. In this paper, using regression analysis through fracture characteristics of four model beam observation and test load-displacement curve characteristic parameters, crack flexural rigidity of the beam bridge relationship has been set up. The qualitative assessment based appearance of cracks in the structure of checks has been converted to quantitative assessment. And compared with the test results of a real bridge, comparative results show that the assessment is objective and reliable. It makes the assessment more objective and scientific. A new way of Quantitative assessment of the structural performance has been provided for a large number of existing reinforced concrete beam bridge. 展开更多

Background: Multiple traumatization in childhood is a broad term that includes interpersonal trauma experiences such as physical, sexual, and emotional abuse and neglect. Children with a child protection case referred to counseling are likely to be in high risk of multiple exposure to interpersonal trauma. We aim to demonstrate that systematic assessment with validated measures is feasible in this age group. Method: The Center for Interventions of Children and Adolescents (CIBU), Denmark, provided a new assessment screening procedure where 16 caregivers with children in the age of 4 - 8-years-old participated prior to initiating counseling in a public family treatment facility. We utilized the Diagnostic Infant Preschool Assessment (DIPA) and the Strengths and Difficulties Questionnaire (SDQ). The SDQ was compared with Danish norms. Results: The data suggest that psychopathology was present among all 16 children in this high-risk sample. Distribution of disorders showed 93.8% prevalence of comorbidity. All 16 children had difficulties regarding psychosocial functioning when compared to a Danish norm population. Most profound was emotional symptoms and symptoms of hyperactivity and inattention. Conclusion: This study revealed a higher prevalence of psychiatric disorders and higher rates of psychosocial difficulties in referred children than in children from the general population. Overall, children in this study had complex symptom profiles. Thus, a systematic approach may be helpful in public treatment facilities, and we suggest the implementation of valid evidence-based instruments. 展开更多

To classify this data, two ... This study compares three types of classifications of satellite data to identify the most suitable for making city maps in a semi-arid region. The source of our data was GeoEye 1 satellite. To classify this data, two pro-grammes were used: an Object-Based Classification and a Pixel-Based Classification. The second classification programme was further subdi-vided into two groups. The first group included classes (buildings, streets, vacant land, vegetations) which were treated simultaneously and on a single image basis. The second, however, was where each class was identified individually, and the results of each class produced a single image and were later enhanced. The classification results were then as-sessed and compared before and after enhancement using visual then automatic assessment. The results of the evaluation showed that the pix-el-based individual classification of each class was rated the highest after enhancement, increasing the Overall Classification Accuracy by 2%, from 89% to 91.00%. The results of this classification type were adopted for mapping Jeddah’s buildings, roads, and vegetations. 展开更多

Kenya is classified as... Water quality is one of the main indicators of the quality of service provided to consumers. Quality has an impact on both the public health and aesthetic value of water as a consumable product. Kenya is classified as a water-scarce country with only 647 cubic meters of renewable freshwater per capita. Water distributed in Nairobi is faced with a myriad of challenges leading to a compromise to its quality. This study focused on evaluating quality of drinking water since human health depends on adequate, clean, reliable water. Analyses were carried out at National Environmental Management Authority (NEMA) accredited Jomo Kenyatta University of Agriculture and Technology (JKUAT) laboratories to determine the chemical, bacteriological and physical characteristics of consumed water in Umoja Innercore Estate in Nairobi. In the study area, 7 HH and 6 BH sites were randomly distributed. pH, turbidity and temperature measurements were analyzed in-situ while bacteria and chemicals were analyzed in laboratories. The study found that 100% of boreholes recorded unsatisfactory water with up to 1100 of Escherichia coli (E. coli) showing high contamination with faecal coliforms and 83% of boreholes recording pH of up to 9.53. Dissolved oxygen was 5.08 mg/L below recommended 12.0 mg/L, salinity of 0.47 mg/L and 0.03 mg/L for boreholes and households respectively. The study reveals the deprived quality of water available to the residents of Umoja Innercore, Nairobi. The study recommends the use of biosand filtration methods for septic tanks, digging of deeper boreholes and lining septic tanks with impermeable materials to prevent contamination of ground water with raw water from septic. 展开更多

Model Integration: A Case Study of Integrative Computations of Hybrid Models to Assess Water Quality of River 认领

Process integration method is used to establish a composite model to perform computations of hybrid models for water quality assessment of Haihe River, and a resolution is presented to clear the long-term obscurity about the differences between single-factor assessment(SFA) and multifactor assessment(MFA) of water quality in this paper. Symbolic models were introduced to describe the types and orders of computations involved in SFA and such MFAs as Nemerow comprehensive index(NCI) and fuzzy comprehensive assessment(FCA). Facilitated by paired t-tests, the composite model of absolute distance(AD) was established to test the differences between SFA and MFAs on four water quality indicators(WQI). Matlab(R14) programs for these models were developed to perform integrative computations on 3 217 batches of water data obtained from seven monitoring sites of Haihe River from 2008 to 2017. Paired t-tests show that results of our SFA model(SFA-4) are not significantly different(p=0.926) from that of SFA based on all WQI, however, extremely significantly different from results of NCI(p=0) and FCA(p=0). SFA-4 is proved by AD model to be farther away from FCA(AD-S→F=1.075) than from NCI(AD-S→N=0.634). More than proving the deduction of SFA≥NCI≥FCA in most cases(p=0.885), the results from AD show that MFAs approach SFA when surface water becomes good(SFA=1) or worst(SFA=6), whereas depart to the farthest distance from SFA when surface water becomes worse(SFA=3). To sum up, the integrative computations involved in AD model on the water data are effective and efficient(improved by 44.2%). Furthermore, AD model shows the differences between SFA and MFAs clearly. 展开更多

目的探讨医生与眩晕患者使用数字等级量表(numeric rating scales,NRS)对周围性眩晕严重程度评分的差异。方法研究对象为2019年6月~2019年9月在我院以眩晕为第一主诉就诊的101例成人患者,医生详细采集每位患者病史后对其眩晕度进行NRS评... 目的探讨医生与眩晕患者使用数字等级量表(numeric rating scales,NRS)对周围性眩晕严重程度评分的差异。方法研究对象为2019年6月~2019年9月在我院以眩晕为第一主诉就诊的101例成人患者,医生详细采集每位患者病史后对其眩晕度进行NRS评分,再由患者自己进行眩晕程度NRS评分,分析两种评分结果的差异和相关性。结果21.78%(22/101)患者与医生NRS评分一致;67.33%(68/101)患者自评得分高于医生评分;81.19%(82/101)的医患评分相差在2分以内;医患NRS评分相关系数r=0.66(P<0.05)。医生对患者的NRS评分1~9(4.34±1.87)分,患者自评NRS评分1~10(5.56±2.08)分,二者之间差异有统计学意义(P<0.05)。结论医生对眩晕给患者生活造成影响的严重程度NRS评分通常低于患者自评得分,但这种差异的临床价值及影响因素有待进一步研究。 展开更多

This research was motivated by the problems confronting the traditional institutions of higher learning in Nigeria which include inadequate funding, lack of infrastructures, inadequate teaching and learning facilities, limited slots for admission known as carrying capacity, a limited number of lecturers/instructors in the tertiary institutions in Nigeria. The objective of this research is to develop a multimedia based virtual learning system for Nigerian tertiary institutions to be used in academic processes for teaching and learning. The methodology used is Object Oriented Design Methodology (OODM) with the following web development tools PHP, JavaScript, Bootstrap 3, HTML5, CSS4, Apache, and MYSQL as database. The expected result is a multimedia based virtual learning system for tertiary institutions in Nigeria which is cost effective, innovative, vibrant, individualized, learner-centric, available anytime, any place even with mobile devices. It was concluded that multimedia based virtual learning system is a veritable tool for Nigerian tertiary institutions, which when fully implemented by Nigerian tertiary institutions would help to increase the number of candidates that are given admission into our tertiary institutions and drastically reduce school dropouts as a result of candidates not gaining admission due to insufficient tertiary institutions/carrying capacity. 展开更多

In this study, toxic equivalent factors and sediment quality guidelines were employed for the assessment of toxicity potential of PAHs in 2 sediment cores collected from the center (RC) and shoreline (RS) of Reforme lake, SE Nigeria over the last ~1 century. Boundary cross plots of isomeric ratios such as Ant/Ant + Phe (>0.1) vs. Fl/Fl + Pyr (>0.4) and BaA/BaA + Chry (>0.35) vs. Icdp/Icdp + Bper (>0.2) indicated biomass combustion/domestic coal utilization as the main sources of PAHs input. The result revealed toxicity unit indices (TU1, 1.22 - 1.57) for the two cores were above the thresholds of no effect. The highest TU1 value found in the near-bottom layer (RS5, 20 - 25 cm) of the RS corresponding to geological time-frame ~1930-1947 coincided with the period of inhabitation by the European settlers along the lake’s catchments when coal or coal products utilization for domestic and recreational purposes was at its peak. On the other hand, TU2 values were <1 at all depth intervals, revealing unlikely PAH effects to resident organisms. Total toxicity equivalency factors (TEqFs) for the surface sediment of RS and RC were 9.29 ng/g TEqFs and 9.16 ng/g TEqFs, respectively, and indicated that more attention should be paid to BaPyr accumulation in the lake’s sediment. 展开更多

根据贵州省84个气象观测站的资料,分析2018-2019年夏收粮油作物生长季内的气温、降水、日照3个气象要素的变化特点,结合作物生物学特性对气象的要求,并与历史同期气候条件进行对比,评估2018-2019年夏收粮油作物生长季内气候条件对农业... 根据贵州省84个气象观测站的资料,分析2018-2019年夏收粮油作物生长季内的气温、降水、日照3个气象要素的变化特点,结合作物生物学特性对气象的要求,并与历史同期气候条件进行对比,评估2018-2019年夏收粮油作物生长季内气候条件对农业生产的影响。结论为:小麦主产区大部光温水条件匹配较好。油菜生长季前期光热条件较差,生育期明显推迟,长势差于2018年,但后期天气转好,利于推进油菜生育进程和产量形成。春播马铃薯气候条件明显好于冬播马铃薯,利于全年马铃薯产量的稳产,冬播马铃薯主产区受阴雨寡照天气影响明显。夏收粮油作物全生育期内无大范围重大农业气象灾害,农业气象条件总体有利于夏收粮油作物生长发育和产量形成,属于较好农业气候年景。 展开更多

目的了解目前直肠癌术后低位前切除综合征(LARS)的评估方法以及治疗进展。方法复习近年来关于直肠癌术后LARS相关研究的文献并加以综述。结果 LARS的发生机制与手术前肛门功能状态、手术中对肛门直肠周围神经、肌肉损伤以及新建直肠容... 目的了解目前直肠癌术后低位前切除综合征(LARS)的评估方法以及治疗进展。方法复习近年来关于直肠癌术后LARS相关研究的文献并加以综述。结果 LARS的发生机制与手术前肛门功能状态、手术中对肛门直肠周围神经、肌肉损伤以及新建直肠容积、顺应性等均有密切关系。针对LARS的症状群,临床现有主观、客观等多种评估办法,包括多种生活质量评分量表、大便失禁相关量表、LARS评分量表、肛管直肠测压、肛门括约肌影像学检查等手段。治疗包括经肛门灌洗、综合康复训练、骶神经刺激、中医疗法等多种治疗方法。结论术前、术中、术后多重因素影响LARS的发生,详尽的评估、规范的手术及治疗以及有效的患者教育都可以预防或减轻LARS,提升患者术后生活质量。 展开更多

对某铁路机务段420名一线职工进行心理健康状况的现况研究并探索其影响因素,初步建立心理健康风险评估模型。整群抽取420例样本,采用症状自评量表(SCL-90)进行心理健康水平测评,与全国常模比较,采用二分类Logistic心理健康风险预警模型... 对某铁路机务段420名一线职工进行心理健康状况的现况研究并探索其影响因素,初步建立心理健康风险评估模型。整群抽取420例样本,采用症状自评量表(SCL-90)进行心理健康水平测评,与全国常模比较,采用二分类Logistic心理健康风险预警模型进行回归分析。结果表明,铁路一线职工在躯体化、饮食睡眠情况、敌对、强迫症状方面的得分均高于全国常模。年龄、文化程度、现职工种、劳动班制、工龄等因素影响职工心理健康状况。进而提出应做好职工心理疏导工作,探索一线职工心理健康预警指标,实现风险识别关口前移,保障铁路运输安全。 展开更多

为更好地解决海上船舶溢油应急管理能力评估过程中的不确定性问题,提出1种基于区间二型模糊层次分析法的云模型综合评估方法。针对评估体系的局部性,借鉴PPRR应急管理过程理论,对影响船舶溢油应急管理能力的因素进行分级和分类,构建全... 为更好地解决海上船舶溢油应急管理能力评估过程中的不确定性问题,提出1种基于区间二型模糊层次分析法的云模型综合评估方法。针对评估体系的局部性,借鉴PPRR应急管理过程理论,对影响船舶溢油应急管理能力的因素进行分级和分类,构建全局评估指标体系;分别运用区间二型模糊层次分析法和逆向云发生器确定各指标权重和云模型参数;通过浮动云和综合云算法获得最终评估结果。结果表明:将5个评估等级和评估结果进行仿真得出评估云的“云滴”图可显示出评估结果的能力等级以及评估结果的可信度和稳定性。计算某海区船舶溢油应急管理能力的评估等级为“较好”,即管辖该海区的海事管理部门能较好地解决海上船舶溢油突发事故,验证了所提方法的可行性和有效性。 展开更多
