Patients with pancreatic cancer(PCa)have a poor prognosis apart from the few suitable for surgery.Photodynamic therapy(PDT)is a minimally invasive treatment modality whose efficacy and safety in treating unresectable localized PCa have been corroborated in clinic.Yet,it suffers from certain limitations during clinical exploitation,including insufficient photosensitizers(PSs)delivery,tumor-oxygenation dependency,and treatment escape of aggressive tumors.To overcome these obstacles,an increasing number of researchers are currently on a quest to develop photosensitizer nanoparticles(NPs)by the use of a variety of nanocarrier systems to improve cellular uptake and biodistribution of photosensitizers.Encapsulation of PSs with NPs endows them significantly higher accumulation within PCa tumors due to the increased solubility and stability in blood circulation.A number of approaches have been explored to produce NPs co-delivering multi-agents affording PDT-based synergistic therapies for improved response rates and durability of response after treatment.This review provides an overview of available data regarding the design,methodology,and oncological outcome of the innovative NPs-based PDT of PCa. 展开更多

Investigations of the seismic behavior of steel reinforced concrete L-shaped columns under constant axial compression and cycled bending-shear-torsion load were performed.Six specimens,which considered two parameters,i.e.,the moment ratio of torsion to bending(γ)and the aspect ratio(column length-to-depth ratio,φ),were prepared for the experiment.In this study,the failure process,torsion-displacement hysteresis curves,and flexure-displacement hysteresis curves were obtained.The failure characteristics,mechanical behavior of specimens such as the failure patterns,hysteresis curves,rigidity degradation,ductility and energy dissipation,are analyzed.The experimental research indicated that the major failures of the specimens were bending failure,bending-shear failure and bending-torsion failure as the moment ratio of torsion to bending(γ)increased.The torsion-displacement hysteresis curves were pinched in the middle,formed a slip platform,and the phenomenon of“load drop”occurred after the peak load.The bending-displacement hysteresis curves were plump,which showed that bending capacity of the specimen was better than its torsion capacity.Additionally,the energy dissipation of the specimen was dominated by torsion in the early stage and ultimately governed by the bending moment in the later phase.Test results also indicated that the displacement ductility coefficient and interstory rotation angle of the failure point were less than 3.0 and 1/50,respectively,which means the test specimen performance does not meet the requirement of the Chinese Code for Seismic Design of Buildings(GB 50011-2014)in this respect. 展开更多

The overall objective of peri-oper... <strong>Background:</strong> Lower extremity surgeries performed in elderly people usually have high prevalence of peri-operative medical problems related to anaesthesia. The overall objective of peri-operative care of geriatric population is to fast recovery from anaesthesia and avoid functional decline.<strong> Objective: </strong>To compare the peri-operative pulmonary status of combined spinal epidural anaesthesia (CSEA) and spinal anaesthesia (SA) in geriatric patients underwent lower extremity surgeries. Methods: This prospective comparative study was conducted at Department of Anaesthesia, Analgesia and Intensive Care Medicine, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh from July 2016 to June 2018. A total of 70 geriatric cases that underwent lower extremity surgeries were included in this study. Cases were randomly allocated into two groups;35 in Group A (CSEA) and 35 in Group B (SAB). The different outcome variables between the groups like-duration of anaesthesia, respiratory rates (RR), oxygen saturation (SpO2), end tidal CO<sub>2</sub> (EtCO<sub>2</sub>), peak expiratory flow rate (PEFR), breath holding test (BHT), peri-operative side effects of anaesthesia and post-operative visual analogue score (VAS) were analyzed and compared by statistical tests. <strong>Results: </strong>The mean age, weight, BMI of Group A and Group B patients were not significantly different (<em>p</em> > 0.05). No significant differences were observed in duration of surgery, gender and ASA grade between the groups (<em>p</em> > 0.05). Mean duration of anaesthesia, mean time to achieve target level of sensory block and mean time to achieve complete motor block were significantly higher in Group A (<em>p</em> < 0.001). Mean RR, SpO2, EtCO<sub>2</sub>, PEFR and BHT of both groups were not significantly different (<em>p</em> > 0.05). Objective: To explore the clinical diagnostic value of transperineal volume ultrasound combined with two-dimensional high-frequency ultrasound for anal fistula. Methods: A total of 52 patients with anal fistula admitted to the Affiliated Hospital of Shaanxi University of Traditional Chinese Medicine from December 2017 to July 2018 were selected. They were all undergoing transperineal 3D volume ultrasound combined with 2D high-frequency ultrasound examination, and the diagnosis results were analyzed. The results of ultrasonography and surgical pathology were compared. Results: Among 52 patients, 3D volume ultrasound combined with 2D high-frequency ultrasound were used to diagnose 32 cases of anal fistula intersphincteric type, 14 cases of transsphincter type, 5 cases of supra-sphincter type, and 1 case of extra-sphincter type. T supervisor classification accuracy rate is 90%. The detection rate of branch pipes was 92%, and the compliance rate of internal fistula was 95%. Two-dimensional high-frequency ultrasound was used to diagnose 34 cases of anal fistula intersphincteric type, 14 cases of transsphincter type, 4 cases of supra-sphincter type, and 0 cases of extra-sphincter type. The detection rate of branch canals was 42%, and the accuracy of type classification was 90%. The coincidence rate was 95%. There was a statistically significant difference in the detection rate of the anal fistula branch and the coincidence rate of the internal fistula between the two methods (both P Conclusion: 1) The overall coincidence rate of three-dimensional volumetric ultrasound combined with two-dimensional high-frequency ultrasound in the diagnosis of anal fistula is high;2) Three-dimensional volumetric ultrasound technology has great application prospects in infants and anal fistulas. 展开更多

Combined heat and power (CHP) plants (co-generation plants) using biomass as fuel, can be an interesting alternative to the predominant electrical heating in Canada. The biomass-fueled boiler provides heat for the steam cycle which in turn generates electricity from the generator connected to the steam turbine. In addition, heat from the process is supplied to a district heating system. The heat can be extracted from the system in a number of ways, by using a back-pressure steam turbine, an extraction steam turbine or by extracting heat directly from the boiler. The objective of the paper is the design, modeling and simulation of such CHP plant. The plant should be sized for providing electric-ity and heat for the Anticosti Island community in Quebec. 展开更多

The objective of the first part of the investigation was to use Aspen Plus software and the Redlich-Kwong-Soave equation of state in order to simulate an adiabatic methanation reactor for the production of synthetic natural methane (SNG) using 1 kg/hr<span><span><span style="font-family:;" "=""> </span></span></span><span><span><span style="font-family:;" "=""><span style="font-family:Verdana;">of carbon dioxide. In this paper, we define the Synthetic Natural Gas Combined Cycle (SNGCC) as a combined cycle power plant where the fuel is synthetic natural gas (SNG) produced by a methanation reactor. The feed of the methanation reactor is the recycled stream of carbon dioxide of a CO<sub>2</sub> capture unit treating the flue gas of the SNGCC power plant. The objective of the second part of the investigation is the utilization of Aspen plus software with SRK equation of state for the simulation of the SNGCC power plant. The metallurgical limitation of the gas turbine was fixed at 1300<sup><span style="font-family:Verdana, Helvetica, Arial;white-space:normal;background-color:#FFFFFF;">°<span style="font-family:Verdana;white-space:normal;"></span></span></sup></span></span>C in this investigation. For effective absorption by amine solutions, the molar percentage of CO<sub>2</sub> in the flue gas should be higher than 10%. Moreover, in order to reduce technical problems linked to oxidative degradation of amine in the CO<sub>2</sub> capture plant, the percentage of O<sub>2</sub> in the flue gas should also be lower than 5%. 目的探讨哮喘-慢阻肺重叠综合征(ACOS)患者联用吸入激素与噻托溴铵的临床效果。方法该研究方便选取2015年12月—2018年6月期间就诊的66例ACOS患者,以随机数字表法将其分入研究组、对照组两组,两组患者例数相同。对照组仅用吸入激素进行... 目的探讨哮喘-慢阻肺重叠综合征(ACOS)患者联用吸入激素与噻托溴铵的临床效果。方法该研究方便选取2015年12月—2018年6月期间就诊的66例ACOS患者,以随机数字表法将其分入研究组、对照组两组,两组患者例数相同。对照组仅用吸入激素进行治疗,研究组加用噻托溴铵。对比两组的发作周期、治疗后的肺功能指标、并发症发生率、临床疗效、用药不良反应等指标。结果研究组具有93.94%的治疗总有效率,比对照组的75.76%高,差异有统计学意义(χ^2=4.243,P=0.039<0.05)。研究组治疗后的发作周期为(9.43±2.46)d,比对照组的(5.23±0.45)d长,差异有统计学意义(t=9.648,P=0.000<0.05)。研究组并发症发生率为6.06%,低于对照组的30.30%,差异有统计学意义(χ^2=6.519,P=0.011<0.05)。两组用药不良反应率相近,差异无统计学意义(χ^2=1.015,P=0.314>0.05)。此外,研究组治疗后的肺功能指标吸气分数、1秒用力呼气量分别为(49.22±4.43)%和(1.98±0.65)L,均比对照组的(39.19±3.37)%和(1.19±0.35)L高,而残总比为(28.08±2.66)%,则比对照组的(39.56±3.34)%低,差异有统计学意义(t=10.352、6.147、15.485,P<0.05)。结论为ACOS患者联用吸入激素、噻托溴铵可比单用吸入激素更有效地改善肺功能、延后发作,同时不必担心严重并发症发生。 展开更多

目的分析三个部位接种无细胞百白破b型流感嗜血杆菌联合疫苗(DTaP‑Hib)不良反应发生情况。方法选取2015年5月至2019年4月北京市朝阳区所有接种门诊自愿接种DTaP‑Hib疫苗的婴幼儿为研究对象,共12241名。DTaP‑Hib疫苗推荐免疫接种程序为3... 目的分析三个部位接种无细胞百白破b型流感嗜血杆菌联合疫苗(DTaP‑Hib)不良反应发生情况。方法选取2015年5月至2019年4月北京市朝阳区所有接种门诊自愿接种DTaP‑Hib疫苗的婴幼儿为研究对象,共12241名。DTaP‑Hib疫苗推荐免疫接种程序为3、4、5月龄进行基础免疫,18~24月龄加强免疫。将研究对象按照接种部位分为3组,分别为股外侧肌组、上臂三角肌组和臀部肌肉组,收集研究对象每剂次疫苗接种后30 min、7 d内的不良反应情况,比较不同部位不良反应发生率。结果股外侧肌、上臂三角肌和臀部肌肉组分别为3461、2659和6121名;共接种35027剂次,股外侧肌、上臂三角肌和臀部肌肉组分别接种11129、7957和15941剂次;共观察到不良反应2489例次,总不良反应发生率为7.11%,其中上臂三角肌组总不良反应发生率为9.69%(771例次),高于臀部肌肉组(7.60%,1211例次)和股外侧肌组(4.56%,507例次)(P<0.001)。上臂三角肌组轻度、中度和严重的不良反应发生率均高于其他两组,发生率分别为4.85%(386例次)、3.77%(300例次)和1.07%(85例次),组间差异有统计学意义(P值均<0.001)。基础免疫3剂次和加强免疫第4剂次在不同部位的总不良反应趋势一致,发生率由高到低依次为上臂三角肌注射、臀部肌肉注射和股外侧肌注射,组间差异有统计学意义(P值均<0.001)。结论研究对象3个部位接种DTaP‑Hib不良反应发生率均较低,均具有较好的安全性,其中股外侧肌组不良反应发生率最低,建议推荐为DTaP‑Hib疫苗的优先接种部位。 展开更多

This study investigated coal-mine drainage(AMD)and municipal wastewater(MWW)contaminant concentrations and conducted the combined treatment in phases I and II:phase I,evaluating the effects of mixing the two based on the extent of acid neutralization and metals removal;phase II:conducting anaerobic batch reactor treatment of AMD and MWW under varying COD/sulfate ratios(0.04-5.0).In phase I,acid mine drainage water quality conditions are as follows:pH 4.5,acidity 467.5 mg/L as CaCO3,alkalinity 96.0 mg/L as CaCO3,Cl-11.8 mg/L,SO42-1722 mg/L,TDS 2757.5 mg/L,TSS 9.8 mg/L,BOD 14.7 mg/L,Fe 138.1 mg/L,Mg 110.8 mg/L.Mn 7.5 mg/L,Al 8.1 mg/L,Na 114.2 mg/L,and Ca 233.5 mg/L.Results of the mixing experiments indicated significant removal of selected metals(Fe 85~98%,Mg 0~65%,Mn 63~89%,Al 98~99%,Na 0~30%),acidity(77~95%)from the mine water and pH was raised to above 6.3.The Phase II results suggested under the wide range of COD/sulfate ratios,COD and sulfate removal varied from 37.4%-100%and 0%-93.5%respectively.During biological treatment,alkalinity was generated which leads to pH increase to around 7.6-8.5.The results suggested feasibility of the proposed technology for co-treatment of AMD and MWW.A conceptual design of co-treatment system which is expected to remove a matrix of pollutants has been provided to utilize all the locally available water resources to achieve the optimum treatment efficiency.The technology also offers an opportunity to significantly reduce capital and operating costs compared to the existing treatment methodologies used.Featured Application:In this study,we have measured the concentrations of contaminants in acid mine drainage(AMD)and municipal wastewater(MWW)and conducted the combined treatment in phases I and II.This is significant because in previous years there was a massive fish kill that is linked to the high total dissolved solids(TDS)and salinity content in the water.With the current proposed combined treatment technology,it shows high potential in reducing TDS and salinity content in the 展开更多

Using the FengYun-3C(FY-3C)onboard BeiDou Navigation Satellite System(BDS)and Global Positioning System(GPS)data from 2013 to 2017,this study investigates the performance and contribution of BDS to precise orbit determination(POD)for a low-Earth orbit(LEO).The overlap comparison result indicates that code bias correction of BDS can improve the POD accuracy by 12.4%.The multi-year averaged one-dimensional(1D)root mean square(RMS)of the overlapping orbit differences(OODs)for the GPS-only solution is 2.0,1.7,and 1.5 cm,respectively,during the 2013,2015,and 2017 periods.The 1D RMS for the BDS-only solution is 150.9,115.0,and 47.4 cm,respectively,during the 2013,2015,and 2017 periods,which is much worse than the GPS-only solution due to the regional system of BDS and the few BDS channels of the FY-3C receiver.For the BDS and GPS combined solution(also known as the GC combined solution),the averaged 1D RMS is 2.5,2.3,and 1.6 cm,respectively,in 2013,2015,and 2017,while the GC combined POD presents a significant accuracy improvement after the exclusion of geostationary Earth orbit(GEO)satellites.The main reason for the improvement seen after this exclusion is the unfavorable satellite tracking geometry and poor orbit accuracy of GEO satellites.The accuracy of BDS-only and GC combined solutions have gradually improved from 2013 to 2017,thanks to improvements in the accuracy of International GNSS Service(IGS)orbit and clock products in recent years,especially the availability of a high-frequency satellite clock product(30 s sampling interval)since 2015.Moreover,the GC POD(without GEO)was able to achieve slightly better accuracy than the GPS-only POD in 2017,indicating that the fusion of BDS and GPS observations can improve the accuracy of LEO POD.GC combined POD can significantly improve the reliability of LEO POD,simply due to system redundancy.An increased contribution of BDS to LEO POD can be expected with the launch of more BDS satellites and with further improvements in the accuracy of BDS satellite products in the ne 展开更多

The fluctuations of renewable energy and various energy demands are crucial issues for the optimal design and operation of combined cooling,heating and power(CCHP)system.In this paper,a novel CCHP system is simulated with advanced adiabatic compressed air energy storage(AA-CAES)technology as a join to connect with wind energy generation and an internal-combustion engine(ICE).The capital cost of utilities,energy cost,environmental protection cost and primary energy savings ratio(P E S R)are used as system performance indicators.To fulfill the cooling,heating and power requirements of a district and consider the thermal-electric coupling of ICE and AA-CAES in CCHP system,three operation strategies are established to schedule the dispatch of AA-CAES and ICE:ICE priority operation strategy,CAES priority operation strategy and simultaneous operation strategy.Each strategy leads the operation load of AA-CAES or ICE to improve the energy supply efficiency of the system.Moreover,to minimize comprehensive costs and maximize the P E S R,a novel optimization algorithm based on intelligent updating multi-objective differential evolution(MODE)is proposed to solve the optimization model.Considering the multi-interface characteristic and active management ability of the ICE and AA-CAES,the economic benefits and energy efficiency of the three operation strategies are compared by the simulation with the same system configuration.On a typical summer day,the simultaneous strategy is the best solution as the total cost is 3643 USD and the P E S R is 66.1%,while on a typical winter day,the ICE priority strategy is the best solution as the total cost is 4529 USD and the P E S R is 64.4%.The proposed methodology provides the CCHP based AA-CAES system with a better optimized operation. 展开更多

Pseudomonas aeruginosa(P:aeruginosa)is a major opportunistic pathogen in hospital-acquired infections.Thus,carly diagnosis is the best strategy for fighting against these infections.In this report,we incorporated multiple crOsS displacement amplification(MCDA)combined with the malachite green(MG)for rapid,sensitive,specific and visual detection of P.aeruginosa by targeting the oprl gene.The MCDA-MG assay was conducted at 67°C for only 40 min during the amplification stage,and then products were directly detected by using colorimetric indicators(MG),eliminating the use of an electrophoresis instrument or amplicon analysis equipment.The entire process,including specimen processing(35 min),amplification(40 min)and detection(5 min),can be finished within 80 min.All 30 non-P.aeruginosa strains tested negative,indicating the high specificity of the MCDA primers.The analytical sensitivity of the MCDA-MG assay was 100 fg of genomic templates per reaction in pure culture,which was in complete accordance with MCDA by gel clectrophoresis and real-time turbidity.The assay was also successfully applied to detecting P.aeruginosa in stool samples.Therefore,the rapidity,simplicity,and nearly equipment free platform of the MCDA-MG technique make it possible for clinical diagnosis,and more. 展开更多

Background:Intravertebral and general anesthesia(GA)are two main anesthesia approaches but both have defects.This study was aimed to evaluate the effect of subarachnoid anesthesia combined with propofol target-controlled infusion(TCI)on blood loss and transfusion for total hip arthroplasty(THA)in elderly patients in comparison with combined spinal-epidural anesthesia(CSEA)or GA.Methods:Totally,240 patients(aged>65 years,American Society of Anesthesiologists[ASA]I-III)scheduled for posterior THA were enrolled from September 1st,2017 to March 1st,2018.All cases were randomly divided into three groups to receive CSEA(group C,w=80),GA(group G,n=80),or subarachnoid anesthesia and propofol TCI(group T,w=80),respectively.Primary outcomes measured were intra-operative blood loss,autologous and allogeneic blood transfusion,mean arterial pressure at different time points,length of stay in post-anesthesia care unit(PACU),length of hospital stay,and patient satisfaction degree.Furthermore,post-operative pain scores and complications were also observed.The difference of quantitative index between groups were analyzed by one-way analysis of variance,repeated measurement generalized linear model,Student-Newman-Keuls test or rank-sum test,while ratio index was analyzed by Chi-square test or Fisher exact test.Results:Basic characteristics were comparable among the three groups.Intra-operative blood loss in group T(331.53±64.33 mL)and group G(308.03±64.90 mL)were significantly less than group C(455.40±120.48 mL,F=65.80,P<0.001).Similarly,the autologous transfusion of group T(130.99±30.36 mL)and group G(124.09±24.34 mL)were also markedly less than group C(178.31±48.68 mL,F=52.99,P<0.001).The allogenetic blood transfusion of group C(0[0,100.00])was also significantly larger than group T(0)and group G(0)(Z=2.47,P=0.047).Except for the baseline,there were significant differences in mean arterial blood pressures before operation(F=496.84,P<0.001),10-min after the beginning of operation(F=351.43,P<0.001),30-min after the beginning 展开更多

Combined liver-kidney transplantation(CLKT)is a rarely performed complex surgical procedure in children and involves transplantation of kidney and either whole or part of liver donated by the same individual(usually a cadaver)to the same recipient during a single surgical procedure.Most common indications for CLKT in children are autosomal recessive polycystic kidney disease and primary hyperoxaluria type 1.Atypical haemolytic uremic syndrome,methylmalonic academia,and conditions where liver and renal failure co-exists may be indications for CLKT.CLKT is often preferred over sequential liver-kidney transplantation due to immunoprotective effects of transplanted liver on renal allograft;however,liver survival has no significant impact.Since CLKT is a major surgical procedure which involves multiple and complex anastomosis surgeries,acute complications are not uncommon.Bleeding,thrombosis,haemodynamic instability,infections,acute cellular rejections,renal and liver dysfunction are acute complications.The long-term outlook is promising with over 80%5-year survival rates among those children who survive the initial six-month postoperative period. 展开更多

A combined dam structure using different concrete materials offers many practical benefits.There are several real-world cases where largevolume heterogeneous concrete materials have been used together.From the engineering design standpoint,it is crucial to understand the deformation coordination characteristics and mechanical properties of large-volume heterogeneous concrete,which affect dam safety and stability.In this study,a large dam facility was selected for a case study,and various design schemes of the combined dam structure were developed by changing the configurations of material zoning and material types for a given dam shape.Elastoplastic analysis of the damfoundation-reservoir system for six schemes was carried out under dynamic conditions,in which the concrete damaged plasticity(CDP)model,the Lagrangian finite element formulation,and a surface-to-surface contact model were utilized.To evaluate the mechanical properties of zoning interfaces and coordination characteristics,the vertical distribution of the first principal stress at the longitudinal joint was used as the critical index of deformation coordination control,and the overall deformation and damage characteristics of the dam were also investigated.Through a comparative study of the design schemes,an optimal scheme of the combined dam structure was identified:large-volume roller-compacted concrete(RCC)is recommended for the dam body upstream of the longitudinal joint,and high-volume fly ash conventional concrete(CC)for the dam body downstream of the longitudinal joint.This study provides engineers with a reference basis for combined dam structure design. 展开更多

Combined liver and kidney transplantation(CLKT)is indicated in patients with failure of both organs,or for the treatment of end-stage chronic kidney disease(ESKD)caused by a genetic defect in the liver.The aim of the present review is to provide the most up-to-date overview of the rare conditions as indications for CLKT.They are major indications for CLKT in children.However,in some of them(e.g.,atypical hemolytic uremic syndrome or primary hyperoxaluria),CLKT may be required in adults as well.Primary hyperoxaluria is divided into three types,of which type 1 and 2 lead to ESKD.CLKT has been proven effective in renal function replacement,at the same time preventing recurrence of the disease.Nephronophthisis is associated with liver fibrosis in 5%of cases and these patients are candidates for CLKT.In alpha 1-antitrypsin deficiency,hereditary C3 deficiency,lecithin cholesterol acyltransferase deficiency and glycogen storage diseases,glomerular or tubulointerstitial disease can lead to chronic kidney disease.Liver transplantation as a part of CLKT corrects underlying genetic and consequent metabolic abnormality.In atypical hemolytic uremic syndrome caused by mutations in the genes for factor H,successful CLKT has been reported in a small number of patients.However,for this indication,CLKT has been largely replaced by eculizumab,an anti-C5 antibody.CLKT has been well established to provide immune protection of the transplanted kidney against donor-specific antibodies against class I HLA,facilitating transplantation in a highly sensitized recipient. 展开更多
