新产程标准分娩对产妇及新生儿的影响 预览

目的:探讨新产程标准分娩对产妇及新生儿的影响。方法:选取某院分娩的236例产妇,利用随机数表法分为2组,每组各118例。观察组以新产程标准进行分娩处理,对照组根据传统产程标准进行分娩处理,对比2组分娩方式、阴道分娩产程及母婴结局。... 目的:探讨新产程标准分娩对产妇及新生儿的影响。方法:选取某院分娩的236例产妇,利用随机数表法分为2组,每组各118例。观察组以新产程标准进行分娩处理,对照组根据传统产程标准进行分娩处理,对比2组分娩方式、阴道分娩产程及母婴结局。结果:观察组剖宫产率较对照组低,差异有统计学意义(P<0.05);阴道分娩产妇潜伏期及第一产程时间观察组较对照组长,差异有统计学意义(P<0.05);2组产妇第二产程及第三产程时间比较,差异无统计学意义(P>0.05);2组产妇产后出血、产后感染、胎儿窘迫、新生儿窒息等发生率比较,差异无统计学意义(P>0.05)。结论:新产程标准分娩可有效降低剖宫产率,但在实施过程中应结合相应的干预措施,以降低产妇及新生儿的并发症发生率。 展开更多

目的分析助产士主导分娩疼痛管理对孕产妇的心理状态、疼痛度及分娩结局的影响。方法选取2016年5月-2017年5月在我院分娩的108例孕产妇作为研究对象。按干预方式不同,将其分为两组,每组各54例。对照组采用常规干预,观察组在常规干预的... 目的分析助产士主导分娩疼痛管理对孕产妇的心理状态、疼痛度及分娩结局的影响。方法选取2016年5月-2017年5月在我院分娩的108例孕产妇作为研究对象。按干预方式不同,将其分为两组,每组各54例。对照组采用常规干预,观察组在常规干预的基础上,行助产士主导分娩疼痛管理,比较两组患者的心理状态及疼痛、分娩情况。结果观察组经干预后SAS评分及SDS评分(25.36±7.21)分、(23.41±6.23)分,均比对照组(36.59±6.37)分、(30.21±5.13)分低,组间比较,差异均具有统计学意义(P <0.05);观察组分娩时疼痛评分为(3.56±1.24)分,低于对照组的(7.51±0.34)分,组间比较,差异均具有统计学意义(P <0.05)。观察组干预后自然分娩率83.33%相较于对照组55.56%高,剖宫产率12.96%相较于对照组33.33%低,且新生儿窒息率3.70%相较于对照组20.37%低,组间比较,差异均具有统计学意义(P <0.05),观察组产后2 h出血量为(142.56±48.21)mL,比对照组(196.80±57.96)mL少,差异具有统计学意义(P <0.05)。结论助产士主导分娩疼痛管理能有效缓解孕产妇负面情绪,减轻分娩疼痛并改善分娩结局。 展开更多

Polymeric amines are being studied intensively as components of systems for gene delivery in genetic engineering and gene therapy of genetic disorders, including cancer. Despite remarkable achievements in the field, polymeric amines, such as polyethyleneimine, show some disadvantages. Strong interaction between the amine-containing polymer and nucleic acid hampers the release of nucleic acid in the cell cytoplasm. Amine groups can interact with the cell membrane which results in cell death. These limitations of polymeric amines stimulated an investigation of new structures for gene delivery. Imidazole-containing polymers have attracted attention as lesser basic substances, while they are able to interact with polymeric acids. Further development of imidazole-based gene delivery agents requires knowledge about some fundamental aspects of interaction between nucleic acids, and polymeric imidazoles. In this work, we studied the complexation of poly(1-vinylimidazole) and oligomeric DNA. We found that the number of active sites capable of binding with negatively charged phosphate groups is comparable with the number of protonated imidazole units in the case of high molecular weight polymer. The increase in polymer charge by 1-bromopropane quaternizating 1%?5% imidazole units or by decreasing the pH to 6.5?7 considerably increased the ability of poly(1-vinylimidazole) to interact with oligonucleotides. The pH sensitivity of this interaction is interesting for cancer gene therapy because the tumours have a lowered intercellular pH (stable oligonucleotide complex) and a higher extracellular pH which can lead to complex dissociation. Minimal critical length for complexation of quaternized poly(1-vinylimidazole) and DNA is below eight units which corresponds to polymers with amine groups. Fluorescence-tagged poly(1-vinylimidazole) samples were obtained and their potential for monitoring the polymer and polymer-oligonucleotide complex internalization into living cells was demonstrated. 展开更多

Effective and low toxicity delivery of siRNA is of great importance for clinical gene therapy. Herein, self-assembled DNA nanoparticles (NPs) based on rolling circle amplification (RCA) with a small interfering RNA (siRNA) payload were successfully developed as a facile and efficient siRNA delivery strategy. This intracellular gene silencing strategy exhibits various advantages including low toxicity, high efficiency, and good stability. The synthesized DNA NPs serve as siRNA carriers, protecting the siRNA against nuclease degradation. We demonstrate that the obtained self-assembled siRNA/NP/PEI system can successfully deliver enhanced green fluorescent protein (EGFP)-siRNA into HeLa cells, realizing the same EGFP knockdown efficiency with less toxicity as that of commercial Lipofectamine 2000. 展开更多

目的:探讨有生育要求的子宫腺肌病(AM)患者接受高强度聚焦超声(HIFU)治疗后的妊娠结局。方法:回顾分析2012年9月至2017年9月在中南大学湘雅三医院接受HIFU治疗的有生育要求的76例AM患者的临床资料。分析患者治疗后症状变化及妊娠结局,... 目的:探讨有生育要求的子宫腺肌病(AM)患者接受高强度聚焦超声(HIFU)治疗后的妊娠结局。方法:回顾分析2012年9月至2017年9月在中南大学湘雅三医院接受HIFU治疗的有生育要求的76例AM患者的临床资料。分析患者治疗后症状变化及妊娠结局,并与接受保守手术治疗的研究进行比较。结果:76例患者经HIFU治疗后,56例58次妊娠(妊娠组),中位妊娠时间为治疗后24个月,其中28例(30次)足月分娩,2例早产,20例自然流产,5例人工流产,1例异位妊娠。未妊娠组20例。患者治疗后月经及痛经症状较治疗前均有显著改善(P<0.05)。19例不孕症患者经治疗后,15例成功受孕,妊娠率78.9%。有自然流产史的14例患者经治疗后,11例受孕,7例已足月分娩。与国外学者的保守手术临床研究比较,HIFU治疗后的妊娠率及足月分娩率均显著增高,差异有统计学意义(P均<0.05),可选择自然分娩,无严重并发症发生。结论:HIFU用于治疗有生育要求的AM患者,可改善患者的妊娠结局,降低自然流产率,分娩方式可选择自然分娩,同时不增加妊娠及分娩时并发症发生率,是一种有效的治疗方式。 展开更多

产程的正确处理对促进安全分娩、降低初次剖宫产率有重要影响。2014年中华医学会妇产科学分会产科学组对产程标准及处理进行了更新,其制定的主要依据是2010年Zhang等发表的基于美国19所医院中62 415例单胎、头位、自然临产并阴道分娩.... 产程的正确处理对促进安全分娩、降低初次剖宫产率有重要影响。2014年中华医学会妇产科学分会产科学组对产程标准及处理进行了更新,其制定的主要依据是2010年Zhang等发表的基于美国19所医院中62 415例单胎、头位、自然临产并阴道分娩.且有新生儿结局的正常产妇的产程研究。随着新产程的应用,新旧产程的评价成为了临床热点问题。2018年12月,《柳叶刀》杂志上发表了一项来自挪威的多中心、随机对照研究,即产程进展研究(Labour Progression Study, LaPS ) .该研究将挪威14个产科中心按照1 : 1原则随机分配为WHO产程图组(对照组)及Zhang产程图组(干预组),其主要结果显示,与WHO产程图相比,应用Zhang产程图并没有显著降低产程中剖宫产发生率,但较研究前均呈现降低的趋势。本文将就该研究带给新产程的循证医学证据进行分析。 展开更多

目的分析右美托咪定复合罗哌卡因硬膜外分娩镇痛的临床疗效。方法选取我院2017年1~12月间收治的足月产妇93例,按照随机数表法将所有孕妇分为观察组(n=47)和对照组(n=46)。观察组为分娩过程中硬膜外镇痛,对照组为自然分娩,不使用镇痛剂,... 目的分析右美托咪定复合罗哌卡因硬膜外分娩镇痛的临床疗效。方法选取我院2017年1~12月间收治的足月产妇93例,按照随机数表法将所有孕妇分为观察组(n=47)和对照组(n=46)。观察组为分娩过程中硬膜外镇痛,对照组为自然分娩,不使用镇痛剂,观察右美托咪定复合罗哌卡因硬膜外分娩镇痛的临床疗效。结果镇痛前,两组孕妇的VAS评分比较,差异无统计学意义(P >0.05)。镇痛后,观察组的产程整个过程的VAS评分显著低于对照组,差异具有统计学意义(P<0.05)。观察组的第一产程的时间为(3.82±1.03)h,显著短于对照组,差异具有统计学意义(P<0.05),而剩余2个产程的时间比较,差异无统计学意义(P >0.05)。两组孕妇在产程过程中不良反应发生率比较,差异无统计学意义(P<0.05)。两组产妇在分娩过程中出血量及新生儿的Apgar评分差异不大,而对照组转剖宫产率为8.70%,显著高于对照组,差异具有统计学意义(P<0.05)。结论右美托咪定复合罗哌卡因硬膜外分娩镇痛可以显著抑制产痛,并且有助于缩短第一产程时间,不会对新生儿健康产生影响,安全性良好,推荐临床广泛使用。 展开更多

This paper presented an interesting nanoparticle-based drug delivery system with morphology transition behavior depending on the content of exposed PEG chain on the particle surface, which is adjustable by addition of different amount of cyclodextrin(a-CD). The effect of a-CD inclusion to the self-assembly behavior of methoxy polyethylene glycol(mPEG) grafted chitosan(CS) was studied. The results showed that the mPEG grafted chitosan(mPEG-[9_TD$IF]g-CS) forms self-assembled nanoparticles with either micelle or hollow sphere morphology depending on the ratio of a-CD to mPEG, as characterized by atomic force microscopy(AFM), transmission electron microscopy(TEM), and X-ray diffraction(XRD). Their sizes and zeta potential increased from 257.6 nmto 768.2 nm and from +4.5 mV to +11.6 mV, respectively, with the increasing amount of a-CD. The correlation between zeta potential and particle size of a-CD/mPEG-[9_TD$IF]g-CS nanoparticles indicated varied PEG density on surface of nanoparticles. Based on the above experimental observations, a likely mechanism for the morphological transition of the rod-coil graft copolymer mPEG-[9_TD$IF]g-CS was proposed. 展开更多

The penetration capability of the drug nano-carriers (NCs) in biological hydrogels, such as mucus and tumor interstitial matrix, would typically influence the efficiency of drug delivery. Understanding the effect of the physicochemical property of the drug carriers on their diffusion capability in biological hydrogel becomes important for designing and optimizing the nano-carriers. Here, using a coarse-grained molecular dynamics model, we studied how the rigidity of NCs affected their diffusivity in biological hydrogel. The results showed that semi-elastic NCs have higher diffusivity than the hard and the soft NCs. Furthermore, the affinity between the NCs and biological hydrogels and the size ratio between the hydrogel meshes and NCs have also affected the diffusivity of the NCs. Further analysis revealed the mechanism that the deformation of the NCs dominates their diffusivity. These findings demonstrated that the rigidity of NCs is a key parameter in designing efficient NCs for deep penetration into the biological hydrogels. 展开更多

Despite the success of parenteral liposomes,oral delivery of liposomes is impeded by various barriers such as instability in the gastrointestinal tract,difficulties in crossing biomembranes, and mass production problems. By modulating the compositions of the lipid bilayers and adding polymers or ligands, both the stability and permeability of liposomes can be greatly improved for oral drug delivery. Transporter-targeted nanoparticulate drug delivery systems(nano-DDS) have emerged as promising nanoplatforms for efficient drug delivery. Recently, great progress in transporter-targeted strategies has been made, especially with the rapid developments in nanotherapeutics. In this review, we outline the recent advances in transporter-targeted nano-DDS. First, the emerging transporter-targeted nano-DDS developed to facilitate oral drug delivery are reviewed. These include improvements in the oral absorption of protein and peptide drugs, facilitating the intravenous-to-oral switch in cancer chemotherapy. Secondly, the recent advances in transporter-assisted brain-targeting nano-DDS are discussed,focusing on the specific transporter-based targeting strategies. Recent developments in transportermediated tumor-targeting drug delivery are also discussed. Finally, the possible transport mechanisms involved in transporter-mediated endocytosis are highlighted, with special attention to the latest findings of the interactions between membrane transporters and nano-DDS. 展开更多

Then the basal vital signs, levels of hemoglobin (Hb) and hematocrit (Hct), as well as the oxygen saturation in brain and intestinal tissue were compared among the four groups. Results: The SBP and DBP between the four groups after treatment had no difference. The HR and RR after treatment in the control group and the mild group were significantly higher than those in the severe group. The RR of the mild group was significantly lower than that of the control group, with statistical difference. The Hb in the control group and the mild group was significantly higher than that in the moderate group and the severe group. The Hb in the moderate group was significantly higher than that in the severe group. Hct in the control group and the mild group was significantly higher than that in the severe group. The Hct in the moderate group had no statistic difference with that of the mild group and severe group. The cerebral oxygen saturation after treatment in the control group and the mild group was significantly higher than that in the severe group, meanwhile the intestinal tissue oxygen saturation after treatment of the control group was significantly higher than that in the severe group. Cerebral oxygen saturation after treatment showed no difference between the severe group and the moderate group, and the intestinal tissue oxygen saturation after treatment either had no difference among the severe group, the moderate group and the mild group. Dbait is a small double-stranded DNA molecule that has been utilized as a radiosensitizer to enhance the sensitivity of glioma to radiotherapy(RT). However, there is no effective drug delivery system to effectively overcome the blood–brain barrier(BBB). The aim of this study was to develop a gene delivery system by using the BBB and glioma dual-targeting and microenvironment-responsive micelles(ch-Kn(s-s)R8-An) to deliver Dbait into glioma for RT. Angiopep-2 can target the low-density lipoprotein receptor-related protein-1(LRP1) that is overexpressed on brain capillary endothelial cells(BCECs) and glioma cells. In particular, due to upregulated matrix metalloproteinase 2(MMP-2) in the tumor microenvironment, we utilized MMP-2-responsive peptides as the enzymatically degradable linkers to conjugate angiopep-2. The results showed that ch-Kn(s-s)R8-An micelles maintained a reasonable size(80–160 nm) with a moderate distribution and a decreased mean diameter from the cross-linking as well as exhibited low critical micelle concentration(CMC) with positive surface charge, ranging from 15 to40 mV. The ch-K5(s-s)R8-An/pEGFP showed high gene transfection efficiency in vitro, improved uptake in glioma cells and good biocompatibility in vitro and in vivo. In addition, the combination of ch-K5(s-s)R8-An/Dbait with RT significantly inhibited the growth of U251 cells in vitro. Thus, ch-K5(s-s)R8-An/Dbait may prove to be a promising gene delivery system to target glioma and enhance the efficacy of RT on U251 cells. 展开更多

Poly(lactide-co-glycolide acid)(PLGA) is an extraordinary well-described polymer and has excellent pharmaceutical properties like high biocompatibility and good biodegradability.Hence,it is one of the most used materials for drug delivery and biomedical systems,also being present in several US Food and Drug Administration-approved carrier systems and therapeutic devices.For both applications,the quantification of the polymer is inalienable.During the development of a production process,parameters like yield or loading efficacy are essential to be determined.Although PLGA is a well-defined biomaterial,it still lacks a sensitive and convenient quantification approach for PLGA-based systems.Thus,we present a novel method for the fast and precise quantification of PLGA by RP-HPLC.The polymer is hydrolyzed into its monomers,glycolic acid and lactic acid.Afterwards,the monomers are derivatized with the absorption-enhancing molecule 2,4′-dibromoacetophenone.Furthermore,the wavelength of the derivatized monomers is shifted to higher wavelengths,where the used solvents show a lower absorption,increasing the sensitivity and detectability.The developed method has a detection limit of 0.1 mg/mL,enabling the quantification of low amounts of PLGA.By quantifying both monomers separately,information about the PLGA monomer ratio can be also directly obtained,being relevant for degradation behavior.Compared to existing approaches,like gravimetric or nuclear magnetic resonance measurements,which are tedious or expensive,the developed method is fast,ideal for routine screening,and it is selective since no stabilizer or excipient is interfering.Due to the high sensitivity and rapidity of the method,it is suitable for both laboratory and industrial uses. 展开更多

Hydrogels composed of poly(vinyl alcohol)(PVA) and poly(ethylene glycol)(PEG) were synthesized using glutaraldehyde as crosslinker and investigated for controlled delivery of the common anti-inflammatory drug,ibuprofen (IBF).To regulate the drug delivery,solid inclusion complexes (ICs) of IBF in β–cyclodextrin (β–CD) were prepared and added to the hydrogels.The ICs were prepared by the microwave irradiation method,which is more environmentally benign.The formation of IC was confirmed by various analytical techniques and the synthesized hydrogels were also characterized.Controlled release of drug was achieved from the hydrogels containing the ICs in comparison to the rapid release from hydrogels containing free IBF.The preliminary kinetic analysis emphasized the crucial role of β–CD in the drug release process that influences the polymer relaxation,thereby leading to prolonged release.The cytotoxicity assay validated the hydrogels as non-toxic in nature and hence can be utilized for controlled delivery of IBF. 展开更多
