With the change of production and life style, the change of dietary structure and the aggravation of population aging, the incidence and fatality rate of diabetes mellitus have been increasing year by year. The pathogenesis of diabetes varies and is closely related to many factors, such as genetic factors, environmental factors, eating habits and so on. Those that endanger the life quality and survival of diabetic patients are mostly complications, including various macrovascular complications and microvascular complications, such as diabetic cardiomyopathy, diabetic nephropathy, diabetic encephalopathy and diabetic foot. The related molecular mechanisms of the pathogenesis of diabetic nephropathy, diabetic retinopathy and diabetic foot are discussed in this paper to provide reference for new drug research and clinical treatment. 展开更多

Diabetes remains an important health issue as more patients with chronic and uncontrolled diabetes develop diabetic nephropathy (DN), which classically presents with proteinuria followed by a progressive decrease in renal function. However, an increasing proportion of DN patients have a decline in kidney function and vascular complications without proteinuria, known as nonproteinuric DN (NP-DN). Despite the increased incidence of NP-DN, few clinical or experimental studies have thoroughly investigated the pathophysiological mechanisms and targeted treatment for this form of DN. In this review, we will examine the differences between conventional DN and NP-DN and consider potential pathophysiological mechanisms, diagnostic markers, and treatment for both DN and NP-DN. The investigation of the pathophysiology of NP-DN should provide additional insight into the cardiovascular factors influencing renal function and disease and provide novel treatments for the vascular complications seen in diabetic patients. 展开更多

Early vs late oral nutrition in patients with diabetic ketoacidosis admitted to a medical intensive care unit 预览

BACKGROUND Diabetic ketoacidosis(DKA)has an associated mortality of 1%to 5%.Upon admission,patients require insulin infusion and close monitoring of electrolyte and blood sugar levels with subsequent transitioning to subcutaneous insulin and oral nutrition.No recommendations exist regarding the appropriate timing for initiation of oral nutrition.AIM To assess short-term outcomes of oral nutrition initiated within 24 h of patients being admitted to a medical intensive care unit(MICU)for DKA.METHODS A retrospective observational cohort study was conducted at a single academic medical center.The patient population consisted of adults admitted to the MICU with the diagnosis of DKA.Baseline characteristics and outcomes were compared between patients receiving oral nutrition within(early nutrition group)and after(late nutrition group)the first 24 h of admission.The primary outcome was 28-d mortality.Secondary outcomes included 90-d mortality,MICU and hospital lengths of stay(LOS),and time to resolution of DKA.RESULTS There were 128 unique admissions to the MICU for DKA with 67 patients receiving early nutrition and 61 receiving late nutrition.The APACHE(Acute Physiology and Chronic Health Evaluation)IV mortality and LOS scores and DKA severity were similar between the groups.No difference in 28-or 90-d mortality was found.Early nutrition was associated with decreased hospital and MICU LOS but not with prolonged DKA resolution,anion gap closure,or greater rate of DKA complications.CONCLUSION In patients with DKA,early nutrition was associated with a shorter MICU and hospital LOS without increasing the rate of DKA complications. 展开更多

Prevalence of diabetic retinopathy, proliferative diabetic retinopathy and non-proliferative diabetic retinopathy in Asian T2DM patients: a systematic review and Metaanalysis

AIM:To investigate the pooled prevalence of diabetic retinopathy(DR), proliferative DR(PDR) and nonproliferative DR(NPDR) in Asian type 2 diabetes mellitus(T2 DM) patients. METHODS:We performed a systematic search online search using PubMed, EMBASE, Web of Science, the Cochrane Library, and China WeiPu Library to identify eligible studies that reported the prevalence of DR, PDR and NPDR in Asian T2 DM patients. Effect size(ES) with 95% confidence interval(CI) was used to evaluate the prevalence of DR, PDR and NPDR in Asian T2 DM patients, respectively. RESULTS:There were 41 references and 48 995 T2 DM patients involved in this study. The prevalence of DR, PDR, and NPDR was 28%, 6%, and 27% in T2 DM patients, respectively;while the prevalence of PDR and NPDR in DR patients was 17% and 83%, respectively. Subgroup analysis showed that prevalence of DR in T2 DM patients from Singaporean, Indian, South Korean, Malaysian, Asian, and Chinese was 33%, 42%, 16%, 35%, 21% and 25%, respectively. In T2 DM patients with NPDR from Indian, South Korean, Malaysian, Asian, Chinese, higher prevalence was found than that in PDR patients(45% vs 17%, 13% vs 3%, 30% vs 5%, 23% vs 2% and 22% vs 3%), as well as in DR patients(74% vs 26%, 81% vs 19%, 86% vs 14%, 92% vs 8% and 85% vs 15%). The prevalence of PDR in T2 DM from India was higher than patients fromother locations of Asia, and the same results were also observed in NPDR patients. CONCLUSION:In either T2 DM Asian patients or DR patients, NPDR is more common than PDR. Based on our results, we should pay more attention to NPDR screening and management in T2 DM patients, and we also recommend suitable interventions to prevent its progression. 展开更多

Epalrestat is an inhibitor of aldose reductase in the polyol pathway and is used for the management of diabetic neuropathy clinically.Our pilot experiments and accumulated evidences showed that epalrestat inhibited polyol pathway and reduced sorbitol production,and suggested the potential renal protection effects of epalrestat on diabetic nephropathy (DN).To evaluate the protective effect of epalrestat,the db/db mice were used and exposed to epalrestat for 8 weeks,both the physiopathological condition and function of kidney were examined.For the first time,we showed that epalrestatmarkedlyreduced albuminuria and alleviated the podocyte foot process fusion and interstitial fibrosis of db/db mice.Metabolomics was employed,and metabolites in the plasma,renal cortex,and urine were profiled using a gas chromatography-mass spectrometry (GC/MS)-based metabolomic platform.We observed an elevation of sorbitol and fructose,and a decrease of myo-inositol in the renal cortex of db/db mice. Epalrestat reversed the renal accumulation of the polyol pathway metabolites of sorbitol and fructose,and increased myo-inositol level.Moreover,the upregulation of aldose reductase,fibronectin,collagen Ⅲ,and TGF-β1 in renal cortex of db/db mice was downregulated by epalrestat.The data suggested that epalrestat has protective effects on DN,and the inhibition ofaldose reductas and the modulation of polyol pathway in nephritic cells be a potentially therapeutic strategy for DN. 展开更多

Objective: It is expected to establish a diabetic ulcer rat model infected with Escherichia coli to provide an appropriate model for the experimental study of diabetic foot ulcer. Method: 32 SD male rats were randomly divided into three groups including the common ulcer group (N), the diabetic ulcer group (DU) and the model group (M) and Befuji group (B). The stability and practicability of the model were evaluated by HE performed on the local skin tissue of the wounds and ELISA detection detecting the content of High sensitive c-reactive protein (HCRP), basic fibroblast growth factor (bFGF) in rat serum after modeling and HCRP, bFGF, tumor necrosis factor-alpha (TNF-α), advanced glycation end products (AGEs), Interleukin-1β (IL-1β) and Interleukin-6 (IL-6) levels after wound healing. Results: After modeling, the HCRP content in N group was lower than that in the other three groups. There was no significant difference in bFGF content between the groups. After wound healing, the HCRP content in serum of rats in each group showed statistical difference. The level of HCRP in group B was lower than that in group M. There was no difference in bFGF content between the groups. The level of bFGF in group B was higher than that in group N. The contents of AGEs in each group were statistically different. The contents of AGEs in group B were lower than those in group DU and group M. The contents of TNF-α, IL-1βand IL-6 in each group were different. The levels of TNF-α, IL-1βand IL-6 in group B were lower than those in group M. Compared with DU group, the levels of TNF-α, IL-1βin M group were higher . Wound healing time in group M was significantly delayed than that in group N, group DU and group B. Conclusion:This model is more in line with the local environmental characteristics of infected diabetic ulcer and provides a reliable tool for the study of diabetic infections. 展开更多

Combination therapy of tripterygium glycosides plus valsartan in diabetic nephropathy treatment: A systematic review and meta-analysis

Objective: To assess the efficacy and safety of the combination therapy of Chinese herbal medicines,tripterygium glycosides, plus valsartan for the treatment of diabetic nephropathy(DN).Methods: A comprehensive research of 12 electronic databases was performed. Data of the included studies were extracted and analyzed independently by two authors, and were synthesized using Review Manager(version 5.2) and Stata(version 12.0).Results: A total of 12 randomized controlled trials(RCTs) involving 829 patients were included. Pooled results showed that the combination therapy significantly increased total efficacy of DN patients [RR = 1.35,95% CI(1.22, 1.50), P < 0.001], the level of serum albumin [Mean difference(MD)= 3.87, 95% CI(3.12,4.62), P < 0.001], and significantly decreased 24 h urinary protein level [MD =-0.97, 95% CI(-1.19,-0.76), P < 0.001], urinary albumin-excretion rate [MD =-145.53, 95% CI(-227.95,-63.11), P < 0.001],and urinary β2-microglobuli level [MD =-11.86, 95% CI(-13.02,-10.69), P < 0.001]. No significant differences were found in levels of serum creatinine [MD =-0.26, 95% CI(-7.52, 7.00), P > 0.05), blood urea nitrogen [MD = 0.25, 95% CI(-0.23, 0.74), P > 0.05], and endogenous creatinine clearance rate[MD =-0.43, 95% CI(-3.48, 2.62), P > 0.05]. However, tripterygium glycosides plus valsartan seemed to exert higher adverse reaction rate than valsartan monotherapy [MD = 3.41, 95% CI(1.34, 8.66), P < 0.05].There were no publication bias for all of the pooled effect sizes.Conclusion: Combination therapy of tripterygium glycosides plus valsartan may be effective for the treatment of DN. However, the safety of the combination therapy need to be further confirmed. 展开更多

Umbelliferone ameliorates renal function in diabetic nephropathy rats through regulating inflammation and TLR/NF-κB pathway

Diabetic nephropathy (DN) is a leading cause of renal failure, contributing to severe morbidity and mortality in diabetic patients. Umbelliferae (Umb) has been well characterized to exert protective effects in diabetes. However, the action and mechanism of Umb in DN remains unclear. In this work, we studied the effect of Umb in a streptozotocin (STZ)-induced DN rat model and explore its underlying mechanism. DN rats were treated with Umb (20, 40 mg·kg^-1) or irbesartan (15 mg·kg^-1) for 4 weeks. Levels of serum glucose, insulin, blood uric acid, creatinine, triglycerides (TG) and total cholesterol (TC) were measured by commercial assay kits, respectively. Histopathological changes and inflammatory cytokine levels including IL-6, IL-1β and TNF-α in the kidney were also evaluated. Alterations in the expression of podocin, CD2AP and TLR/NF-κB were assessed by western blotting. Our results showed that Umb reduced renal injury in DN rat model, as evidenced by the decrease in blood glucose, 24 h urinary protein, serum creatinine, and blood uric acid. Umb also significantly ameliorated the renal histopathological alteration, and down-regulated the expression of epithclial-to-mesenchymal transition-related molecular markers podocin and CD2AP. Moreover, Umb inhibited TLR2, TLR4, MyD88 expressions, NF-κB activation and considerably reduced levels of other downstream inflammatory molecules (TNF-α, IL-6, IL-1β). These findings indicated that Umb improved renal function through regulating inflammation and TLR/NF-κB pathway, suggesting the potential efficacy of Umb in DN treatment. 展开更多

The prevalence of type 2 diabetes(T2D)has increased worldwide and doubled over the last two decades.It features among the top 10 causes of mortality and morbidity in the world.Cardiovascular disease is the leading cause of complications in diabetes and within this,heart failure has been shown to be the leading cause of emergency admissions in the United Kingdom.There are many hypotheses and well-evidenced mechanisms by which diabetic cardiomyopathy as an entity develops.This review aims to give an overview of these mechanisms,with particular emphasis on metabolic inflexibility.T2D is associated with inefficient substrate utilisation,an inability to increase glucose metabolism and dependence on fatty acid oxidation within the diabetic heart resulting in mitochondrial uncoupling,glucotoxicity,lipotoxicity and initially subclinical cardiac dysfunction and finally in overt heart failure.The review also gives a concise update on developments within clinical imaging,specifically cardiac magnetic resonance studies to characterise and phenotype early cardiac dysfunction in T2D.A better understanding of the pathophysiology involved provides a platform for targeted therapy in diabetes to prevent the development of early heart failure with preserved ejection fraction. 展开更多

玻璃体切除联合康柏西普治疗糖尿病视网膜病变

目的评价玻璃体切除术联合康柏西普治疗增生型糖尿病视网膜病变(PDR)的临床效果。方法前瞻性病例对照研究。2016年10月至2017年12月收治的PDR62例(86眼),随机分为两组:单纯玻璃体切除术31例(41眼)为对照组,玻璃体切除术联合康柏西普玻... 目的评价玻璃体切除术联合康柏西普治疗增生型糖尿病视网膜病变(PDR)的临床效果。方法前瞻性病例对照研究。2016年10月至2017年12月收治的PDR62例(86眼),随机分为两组:单纯玻璃体切除术31例(41眼)为对照组,玻璃体切除术联合康柏西普玻璃体内注射31例(45眼)为联合组。术前、术后3个月和6个月检测最佳矫正视力(BCVA)、黄斑视网膜厚度以及黄斑中心区厚度(CMT),并记录并发症情况。结果联合组总有效率优于对照组(95.56%vs75.61%),差异有统计学意义(χ^2=5.5444,P=0.0185)。联合组术前、术后3个月和6个月BCVA(LogMAR)分别为1.13±0.19、0.71±0.22、0.38±0.16;而对照组分别为1.12±0.18、0.73±0.20、0.53±0.18。联合组术后6个月BCVA优于对照组,差异有统计学意义(t=4.0910,P=0.0001)。两组手术前后CMT、黄斑视网膜厚度及术后并发症发生率差异均无统计学意义(P>0.05)。结论采用玻璃体切除术联合康柏西普治疗PDR具有良好的临床效果。 展开更多

电针对糖尿病大鼠造影剂肾损伤的保护作用 预览

目的观察电针对造影剂肾病模型(CIN模型)大鼠肾损伤的影响。方法选取雄性清洁级SD大鼠50只,随机选取10只大鼠为对照组,将剩余40只制备成糖尿病模型,造模成功30只,随机分为模型组,电针组,冬虫夏草组,各10只,糖尿病模型建好后饲养9周,对... 目的观察电针对造影剂肾病模型(CIN模型)大鼠肾损伤的影响。方法选取雄性清洁级SD大鼠50只,随机选取10只大鼠为对照组,将剩余40只制备成糖尿病模型,造模成功30只,随机分为模型组,电针组,冬虫夏草组,各10只,糖尿病模型建好后饲养9周,对照组与模型组给予等量0.9%氯化钠注射液灌胃,电针组给予电针治疗,冬虫夏草组给予冬虫夏草菌粉[3 g/(kg·d)]灌胃,各组干预1周后建立糖尿病CIN模型,处死大鼠,摘取左肾,以备肾脏病理研究,摘取右肾,用于Western blotting定量分析,观察各组大鼠的尿素氮(BUN)、血肌酐(SCr)、氧化应激指标及肾组织Caspase-3、Caspase-9蛋白检测水平,并采用HE染色和TUNEL染色观察各组肾脏组织的病理改变。结果1)与模型组比较,冬虫夏草组、电针组的大鼠的BUN、Scr、Kim-l、NGAL均明显下降(P<0.05)。2)与模型组比较,冬虫夏草组、电针组氧化应激指标均降低,以及Caspase-9、Caspase-3表达水平均升高(P<0.05)。3)其中冬虫夏草组、电针组差异无统计学意义(P>0.05)。结论电针能够减轻糖尿病大鼠CIN的早期肾损害,且与冬虫夏草治疗效果相当。 展开更多

AIM: To estimate the prevalence of diabetic retinopathy(DR) in a diabetic population of the public health system. METHODS: This non-experimental, descriptive and crosssectional study, with no direct control over the variables analysed, was carried out by the Telemedicine Unit of the University of Concepción(TELMED) and the Family Health Centers(CESFAM) of the Health Service Concepción, Chile, among a population of 7382 diabetic patients of the public health system, from October 2014 to June 2015. Two digital images for each patient’s eyes were obtained and uploaded to the TELMED tele-ophthalmology platform. The images were remotely evaluated by a retina expert ophthalmologist for diagnosis. Treatment and a referral(if required) were recommended, and an individualised report for each patient was sent to the place of origin.RESULTS: The prevalence of DR in this study was 14.9%. Of these, 48.7% had mild non-proliferative DR(NPDR), 30.8% moderate NPDR, 15.9% severe NPDR, and 4.6% proliferative DR. Of the patients with DR, 17.8% were referred for fluorescein angiography, 4.3% for panretinal photocoagulation, 1.2% for vitrectomy, and 0.4% for cataract surgery.CONCLUSION: The use of telemedicine allowed an increased screening coverage for DR in diabetic patients. The different types of DR were detected in a timely manner, decreasing waiting times due to the lack of specialists in ophthalmology in the public health system. 展开更多

Comparison of conbercept and ranibizumab for the treatment efficacy of diabetic macular edema: a Metaanalysis and systematic review

AIM: To evaluate the efficacy of intravitreal injection of conbercept(IVC) and ranibizumab(IVR) in patients with diabetic macular edema.METHODS: Reviewers have searched 12 databases, including PubMed, Medline, EMBASE, Web of Science, Springer, ScienceDirect, OVID, Cochrane Library, Clinical Trials.gov, cqVIP, WanFangdata and China National Knowledge Infrastructure(CNKI), up to December 28, 2018. Rev Man 5.3(Cochrane Library Software, Oxford, UK) was employed for statistical analysis. Fixed and random effects models were applied to assess heterogeneity. Odds ratio(OR) was applied for dichotomous variables;weighted mean difference(WMD) was applied for continuous variables. The confidence interval(CI) was set at 95%. Central macular thickness(CMT) and best-corrected visual acuity(BCVA) were employed to analyze the improvement of DME patients. Inclusion criteria for picking out studies were retrospective studies and randomized controlled trials(RCTs) that compared IVC and IVR for the treatment of diabetic macular edema.RESULTS: Four retrospective studies and five RCTs were included with a total of 609 patients. No statistically significant difference was observed in mean CMT and mean BCVA in the baseline parameters [BCVA(WMD:-0.48;95%CI:-1.06 to 0.10;P=0.1), CMT(WMD:-0.83;95%CI:-15.15 to 13.49;P=0.91). No significant difference was found in the improvement of BCVA and adverse event(AE) in IVC group, compared with IVR group after treatment of loading dosage [the 1 st month BCVA(WMD: 0.01;95%CI:-0.26 to 0.27;P=0.96), the 3 rd month BCVA(WMD:-0.04;95%CI:-0.14 to 0.06;P=0.46);the 6 th month BCVA(WMD:-0.24;95%CI:-1.62 to 1.14;P=0.73)], AE(OR: 0.84;95%CI: 0.38 to 1.84;P=0.66)]. Reactive oxygen species mediates a metabolic memory of high glucose stress signaling in bovine retinal pericytes

AIM: To investigate the role of reactive oxygen species (ROS) and antioxidant mechanism underlying the metabolic memory of bovine retinal pericytes (BRPs) induced by high glucose.METHODS: Effects of high glucose levels and culture time on BRPs viability were evaluated by CCK-8. BRPs were grown in high-glucose media (30 mmol/L) for 4d followed by culture in normal glucose condition (5.6 mmol/L) for 4d in an experimental group. In contrast, in negative and positive control groups, BRPs were grown in either normal-glucose media or high-glucose media for 8d, respectively. The ROS levels, apoptosis, the expression and activity of manganese superoxide dismutase (MnSOD) in BRPs, as well as the protective effect of adeno-associated viral (AAV)-mediated over expression of MnSOD were determined separately by DCHFA, ELISA and Western blot.RESULTS: Comparing the result of cells apoptosis, activity and protein expression of MnSOD and caspase-3, the cell culture system that exposed in sequence in 30 mmol/L and normal glucose for 4d was demonstrated as a suitable model of metabolic memory. Furthermore, delivery of antioxidant gene MnSOD can decrease BRPs apoptosis, reduce activated caspase-3, and reverse hyperglycemic memory by reducing the ROS of mitochondria.CONCLUSION: Increased ROS levels and decreased MnSOD levels may play important roles in pericyte loss of diabetic retinopathy. BRPs cultured in high glucose for 4d followed by normal glucose for 4d could be an appropriate model of metabolic memory. rAAV-MnSOD gene therapy provides a promising strategy to inhibit this blinding disease. 展开更多

Effects of adjuvant Qishen Yiqi Drop Pill therapy on renal function changes and prognosis of patients with early diabetic nephropathy

Objective: To study the effects of adjuvant Qishen Yiqi Drop Pill therapy on renal function changes and prognosis of patients with early diabetic nephropathy. Methods: The patients with early diabetic nephropathy treated in our hospital between February 2015 and April 2017 were chosen and divided into two groups by random number table, observation group received Qishen Yiqi Drop Pill combined with conventional therapy and control group accepted conventional therapy. The renal function indexes, cytokine contents and oxygen free radical generation were compared before treatment and 3 months after treatment. Results:Urine UREA levels as well as serum β2 microglobulin (β2-MG), cystatin C (CysC), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), stromal cell-derived factor-1 (SDF-1), interleukin-17 (IL-17), transforming growth factor-β1 (TGF-β1), malondialdehyde (MDA) and 8-iso-prostaglandin F2α (8-iso-PGF2α) contents of both groups were significantly lower while superoxide dismutase (SOD) and total antioxidant capacity (T-AOC) contents were significantly higher after treatment, and urine UREA level as well as serum β2-MG, CysC, TNF-α, IL-6, SDF-1, IL-17, TGF-β1, MDA and 8-iso-PGF2α contents of observation group after treatment was significantly lower than those of control group while SOD and T-AOC contents were significantly higher than those of control group. Conclusion: Adjuvant Qishen Yiqi Drop Pill therapy can improve the renal function and reduce the inflammatory response and oxidative stress response in early diabetic nephropathy. 展开更多
