ECHO、DCG及MSCT冠脉成像在无症状心肌缺血临床诊断中的应用 预览

目的分析超声心动图(ECHO)、动态心电图(DCG)、多排螺旋电子计算机断层扫描(MDCT)冠脉成像在无症状心肌缺血(SMI)临床诊断中的应用效果。方法回顾性分析我院84例疑似SMI患者临床资料,所有患者均行ECHO、DCG、MSCT冠脉成像及冠状动脉造影... 目的分析超声心动图(ECHO)、动态心电图(DCG)、多排螺旋电子计算机断层扫描(MDCT)冠脉成像在无症状心肌缺血(SMI)临床诊断中的应用效果。方法回顾性分析我院84例疑似SMI患者临床资料,所有患者均行ECHO、DCG、MSCT冠脉成像及冠状动脉造影(CAG)检查,并以CAG诊断结果作为金标准,比较ECHO、DCG、MSCT冠脉成像对SMI的诊断价值。结果 84例疑似SMI患者中,经CAG确诊SMI有69例(82.14%)。MSCT冠脉成像诊断SMI准确77例(91.67%),漏诊5例(7.25%),误诊2例(13.33%),Kappa值为0.737;DCG诊断SMI准确73例(86.90%),漏诊8例(11.59%),误诊3例(20.00%),Kappa值为0.605;ECHO诊断SMI准确66例(78.57%),漏诊14例(20.29%),误诊4例(26.67%),Kappa值为0.421。3种检查方法Kappa值以MSCT冠脉成像最高;3种检查方法特异度、阳性预测值、阴性预测值比较,差异无统计学意义(P>0.05);MSCT冠脉成像灵敏度、准确率均高于ECHO(P<0.05);但MSCT冠脉成像与DCG灵敏度、准确率比较,差异无统计学意义(P>0.05);ECHO与DCG灵敏度、准确率比较,差异亦无统计学意义(P>0.05)。结论 ECHO、DCG及MSCT冠脉成像在SMI临床诊断中具有一定作用,但MSCT冠脉成像准确性更高,临床实用价值较高。 展开更多

Based on the conventional observation data,satellite,radar,automatic observation and other refined monitoring data,the severe hail weather in Hohhot City on July 29,2015 was analyzed at multiple scales.The results showed that from the perspective of synoptic-scale system,high-altitude cold air invaded Mongol shortwave trough,and the North China small high pressure and the subtropical high formed a warm shear line;the surface convergence line triggered severe convection weather.Unstable vertical structure,continuous increase of low-level humidity,more drier of high altitudes and the wind direction shear at 180° triggered hailstorm and lightning weather in Hohhot.α-scale convective cloud clusters were maintained in Hohhot,and the ground "herringbone" shear line oscillated violently between the north and the south.Strong lightning and big hails appeared on the ground.The "herringbone" shear line lifted slowly to the north,and there was short-time heavy precipitation on the ground.Radar products showed "bow echo",and the echo had a hanging structure;a mesoscale cyclone was formed 3 km away from the ground.On the radar wind profile,it can be seen that low-layer warm advection and strong wind shear triggered strong hailstorm in Hohhot.The disappearance of low- level warm advection and the maintenance of strong vertical shear of wind produced short-time heavy precipitation.There was a small difference between short-time heavy precipitation and hail in echo top height,but the coverage was significantly reduced.In Hohhot City,vertically integrated liquid water content quickly increased during the hail,and decreased during the short-time heavy precipitation. 展开更多

Catheter based ventriculography has evolved over the last century and in many instances has been replaced by use of cardiac ultrasound.Despite the shift,ventricular function remains an important part of assessing myocardial damage and prognosis.There is no doubt that cardiac ultrasound can evaluate LV size,myocardial wall motion and wall thickening,and is non-invasive,readily available,relatively inexpensive and portable.Poor acoustic windows are the major limitation of cardiac ultrasound as are foreshortened imaging planes.In contrast,catheter based left ventriculography,can evaluate similar attributes of LV function but limitations are that it is invasive requiring arterial access,involves radiation exposure,and the use of iodinated radio-opaque contrast which may result in renal dysfunction.The Society of Cardiovascular angiography and Intervention(SCAI)recommendations for catheter based left ventriculography are consensus opinions which need rigorous prospective evaluation.Probably the most important SCAI recommendation is that local criteria should be developed to decrease variation in performance among operators within individual catheterization laboratories.No data are available to determine which patient gets catheter based left ventriculography or echo assessment of ventricular function.Decision making regarding the use of catheter based or ultrasound based angiography is quite complex because of limitation and interaction of the various determinants,e.g.creatinine,diabetes,gender,contrast volume and fl uoro time. 展开更多

Background: The zone of calcified cartilage (ZCC) plays an important role in the pathogenesis of osteoarthritis (OA) but has never been imaged in vivo with magnetic resonance (MR) imaging techniques. We investigated the feasibility of direct imaging of the ZCC in both cadaveric whole knee specimens and in vivo healthy knees using a 3-dimensional ultrashort echo time cones (3D UTE-Cones) sequence on a clinical 3T scanner. Methods: In all, 12 cadaveric knee joints and 10 in vivo healthy were collected. At a 3T MR scanner with an 8-channel knee coil, a fat-saturated 3D dual-echo UTE-Cones sequence was used to image the ZCC, following with a short rectangular pulse excitation and 3D spiral sampling with conical view ordering. The regions of interests (ROIs) were delineated by a blinded observer. Singlecomponent T2* and T2 values were calculated from fat-saturated 3D dual-echo UTE-Cones and a Carr-Purcell-Meiboom-Gill (T2 CPMG) data using a semi-automated MATLAB code. Results: The single-exponential fitting curve of ZCC was accurately obtained with R2 of 0.989. For keen joint samples, the ZCC has a short T2* ranging from 0.62 to 2.55 ms, with the mean ±standard deviation (SD) of 1.49 ±0.66 ms, and with 95% confidence intervals (CI) of 1.20-1.78 ms. For volunteers, the short T2* ranges from 0.93 to 3.52ms, with the mean±SD of 2.09±0.56 ms, and the 95% CI is 1.43 to 2.74ms in ZCC. Conclusions: The high-resolution 3D UTE-Cones sequence might be used to directly image ZCC in the human knee joint on a clinical 3T scanner with a scan time of more than 10 min. Using this non-invasive technique, the T2* relaxation time of the ZCC can be further detected. 展开更多

On May 28,2018,a strong hailstorm accompanying gale(with speed of 17 m/s)and hail(with the maximum diameter of 42 mm)occurred in Yantai in Shandong Peninsula.Based on the conventional weather observation,Doppler weather radar,and automatic weather station data,the evolution characteristics of the strong hailstorm in Shandong Peninsula were analyzed.The results show that this multi-cell strong storm occurred under the background of high-altitude cold vortex.The strong cold advection at the middle and high altitudes and the weak warm advection at the low altitude formed a strong unstable stratification,and also led to cooling at the middle altitudes at the same time.The height of 0 and-20℃dropped significantly,which provided better environmental conditions for the growth of hail in the air.The lower temperature 0℃also ensured less melting of hail during falling,and the middle dry layer was conducive to the formation of thunderstorm gale.The vertical wind shear above medium intensity was the development and maintenance mechanism of thunderstorm cells gradually evolving into a strong hailstorm.From the analysis of radar echo characteristics,it is found that it was a strong hailstorm weather process caused by the linear multi-cell storm.The thunderstorm cells that could produce large hail occurred and developed,and the whole process lasted for 2 h under the triggering of the convergence line formed by the interaction of the sea breeze front and ground cold front.It had obvious characteristics of hail such as three-body scattering spikes,side-lobe echoes,high-hanging strong echoes,as well as the characteristics of thunderstorm gale such as arcuate echoes,middle-level radial convergence,and back side inflow.The storm development process was accompanied by backward propagation,but the long axis of the line storm was basically perpendicular to the movement direction of the storm,so there was no short-term heavy precipitation.The stratification instability conditions,water vapor conditions and ground convergence condit 展开更多

Improved vocal effort modeling by exploiting echo state network and radial basis function network

The independent hypothesis between frames in vocal effect(VE) recognition makes it difficult for frame based spectral features to describe the intrinsic temporal correlation and dynamic change information in speech phenomena. A novel VE detection method based on echo state network(ESN) is proposed. The input sequences are mapped into a fixed-dimensionality vector in high dimensional coding space by reservoir of the ESN. Then, radial basis function(RBF) networks are employed to fit the probability density function(pdf) of each VE mode by using the vectors in the high dimensional coding space. Finally, the minimum error rate Bayesian decision is employed to judge the VE mode. The experiments which are conducted on isolated words test set achieve 79.5% average recognition accuracy, and the results show that the proposed method can overcome the defect of the independent hypothesis between frames effectively. 展开更多

在现有管理系统中,由于出现回声和网络时延情况,导致坐席语音管理效率较差,提出了同于指挥调度的坐席语音管理系统。根据系统坐席语音指挥调度管理系统总体框架,设计了系统硬件模块。使用MCS-8051型号的单片机作为交换机底板,采用双向... 在现有管理系统中,由于出现回声和网络时延情况,导致坐席语音管理效率较差,提出了同于指挥调度的坐席语音管理系统。根据系统坐席语音指挥调度管理系统总体框架,设计了系统硬件模块。使用MCS-8051型号的单片机作为交换机底板,采用双向通信方式,将CTI-LINK作为开放接口,设计了CTI服务器。利用计算机可实现语音信息录入与查询;采用Visuual Studio2018集成开发环境对交换机软件程序进行编写,选取23B+D的信道,根据信道指令设计坐席语音管理信令方式。设立缓冲机制,改善网络时延问题,保证用户能够听到连续语音。利用数字回声消除法来消除本地回声,由此实现无回声影响指挥调度坐席语音管理系统的设计。通过实验结果可知,该系统最高管理效率为86%,使指挥调度工作效率得以有效提高。 展开更多

In this paper, we propose a scheme for photon echo chirped detection process composed of additional modulation pulses to obtain controllable geometric phase. The geometric phases are observed and measured by a beat signal between the photon echo field and the chirped field. The chirped detection model reveals that the period of the beat signal increases as the chirped rate and delay time increase. Additionally, a two-fold relationship between the modulation phase and the echo shift phase is obtained. The numerical simulations accord with the theoretical results obtained by the finite difference time domain(FDTD) method. 展开更多

Traffic flow prediction has been applied into many wireless communication applications(e.g., smart city, Internet of Things). With the development of wireless communication technologies and artificial intelligence, how to design a system for real-time traffic flow prediction and receive high accuracy of prediction are urgent problems for both researchers and equipment suppliers. This paper presents a novel real-time system for traffic flow prediction. Different from the single algorithm for traffic flow prediction, our novel system firstly utilizes dynamic time wrapping to judge whether traffic flow data has regularity,realizing traffic flow data classification. After traffic flow data classification, we respectively make use of XGBoost and wavelet transform-echo state network to predict traffic flow data according to their regularity. Moreover, in order to realize real-time classification and prediction, we apply Spark/Hadoop computing platform to process large amounts of traffic data. Numerical results show that the proposed novel system has better performance and higher accuracy than other schemes. 展开更多
