Combination therapy of tripterygium glycosides plus valsartan in diabetic nephropathy treatment: A systematic review and meta-analysis

Objective: To assess the efficacy and safety of the combination therapy of Chinese herbal medicines,tripterygium glycosides, plus valsartan for the treatment of diabetic nephropathy(DN).Methods: A comprehensive research of 12 electronic databases was performed. Data of the included studies were extracted and analyzed independently by two authors, and were synthesized using Review Manager(version 5.2) and Stata(version 12.0).Results: A total of 12 randomized controlled trials(RCTs) involving 829 patients were included. Pooled results showed that the combination therapy significantly increased total efficacy of DN patients [RR = 1.35,95% CI(1.22, 1.50), P < 0.001], the level of serum albumin [Mean difference(MD)= 3.87, 95% CI(3.12,4.62), P < 0.001], and significantly decreased 24 h urinary protein level [MD =-0.97, 95% CI(-1.19,-0.76), P < 0.001], urinary albumin-excretion rate [MD =-145.53, 95% CI(-227.95,-63.11), P < 0.001],and urinary β2-microglobuli level [MD =-11.86, 95% CI(-13.02,-10.69), P < 0.001]. No significant differences were found in levels of serum creatinine [MD =-0.26, 95% CI(-7.52, 7.00), P > 0.05), blood urea nitrogen [MD = 0.25, 95% CI(-0.23, 0.74), P > 0.05], and endogenous creatinine clearance rate[MD =-0.43, 95% CI(-3.48, 2.62), P > 0.05]. However, tripterygium glycosides plus valsartan seemed to exert higher adverse reaction rate than valsartan monotherapy [MD = 3.41, 95% CI(1.34, 8.66), P < 0.05].There were no publication bias for all of the pooled effect sizes.Conclusion: Combination therapy of tripterygium glycosides plus valsartan may be effective for the treatment of DN. However, the safety of the combination therapy need to be further confirmed. 展开更多

Benefits and Safety of Tripterygium Glycosides and Total Glucosides of Paeony for Rheumatoid Arthritis: An Overview of Systematic Reviews

Objective: To summarize the evidence from systematic reviews(SRs) on the benefits and safety of Tripterygium glycosides(TG) and total glucosides of paeony(TGP), commonly used to treat rheumatoid arthritis(RA) in China... Objective: To summarize the evidence from systematic reviews(SRs) on the benefits and safety of Tripterygium glycosides(TG) and total glucosides of paeony(TGP), commonly used to treat rheumatoid arthritis(RA) in China, for patients with RA. Methods: SRs of randomized controlled trials(RCTs) on TG or TGP in treating RA were included, by searching 8 databases from their inception until December 2017. Two authors extracted data independently. We assessed the quality of SRs using AMSTAR and graded the quality of evidence according to the GRADE approach. Results: Eleven SRs containing an average of 7.6 RCTs, involving a total of 7,012 participants were included in this overview. On the basis of included SRs, TG and TGP could improve the following indexes for RA patients: American College of Rheumatology(ACR) 20 response rate, ACR50 response rate and ACR70 response rate, swollen joint count, tender joint count, erythrocyte sedimentation rate and C-reactive protein. Moreover, TGP could reduce incidence of hepatotoxicity. The most common adverse effects of TG were gastrointestinal discomfort and gonad toxicity, while for TGP was mild to moderate diarrhea. The overall quality of evidence for these findings ranged from "low" to "moderate". Conclusions: TG and TGP might be 2 potentially effective complementary and alternative drugs for patients with RA. Nevertheless, due to gonad toxicity, TG should only be considered in elderly patients or patients without reproductive needs. More evidence from high quality RCTs and SRs is warranted to support the use of TG and TGP for RA patients. 展开更多

Flaxseed cake contains cyanogenic glucosides, which can be metabolized into hydrocyanic acid in an animal’s body, leading to asphyxia poisoning in cells. Beta-glucosidase is highly efficient in degrading cyanogenic glucosides. The Cattle may have b-glucosidase-producing strains in the intestinal tract after eating small amounts of flaxseed cake for a long time. This study aimed to isolate of a strain from cow dung that produces b-glucosidase with high activity and can significantly reduce the amount of cyanogenic glucosides. We used cow dung as the microflora source and an esculin agar as the selective medium. After screening with 0.05% esculin and 0.01% ferric citrate, we isolated 5 strains producing high amounts of b-glucosidase. In vitro flaxseed cake fermentation was fermented by these 5 strains, in which the strain M-2 exerted the best effect(P < 0.05). The strain M-2 was identified as Lichtheimia ramosa and used as the fermentation strain to optimize the fermentation parameters by a single factor analysis and orthogonal experimental design. The optimum condition was as follows: inoculum size3%, water content 60%, time 144 h, and temperature 32℃. Under this condition, the removal rate of cyanogenic glucosides reached 89%, and crude protein increment reached 44%. These results provided a theoretical basis for the removal of cyanogenic glucosides in flaxseed and the comprehensive utilization of flaxseed cake. 展开更多

As Garcinia Glycosides is an effective but insoluble anti-tumor drug, the aim of this study was to obtain a solid dispersion form Garcinia Glycosides by using solvent-melt method so that improve the solubility and dissolution rate. The solid dispersion was characterized by High Performance Liquid Chromatography (HPLC), infrared spectroscopy and evaluated the intestinal absorption of the drug by rat in situ single pass intestinal perfusion. The results showed the increase of solubility, dissolution velocity and absorption compared to other forms. This indicated that solid dispersion could greatly improve the relative bioavailability of Garcinia Glycosides in vivo. 展开更多

Objective: To compare the pharmacokinetics of naringin and neohesperidin after oral administration of Zhishi total flavonoid glycosides(ZSTFG) extracted from Aurantii Fructus Immaturus in normal and gastrointestinal motility disorders(GMD) mice.Methods: ZSTFG was orally given to normal and GMD mice induced by atropine or dopamine. The plasma samples were incubated with β-glucuronidase/sulfatase, the total(free + conjugated) naringenin and hesperitin were extracted with acetonitrile. The validated HPLC-MS/MS method was successfully applied to the pharmacokinetic study.Results: The results showed that, compared with the normal group, AUC0–∞, AUC0–tand Cmaxfor total naringenin and hesperitin were significantly higher(P < 0.01 or P < 0.05), while CLZ/F for total naringenin and hesperitin was significantly lower(P < 0.01) in the GMD group. Tmax, t1/2 z, MRT0-t, and MRT0-∞for naringenin were longer(P < 0.01) in the GMD group than those in the normal group.Conclusion: The results showed that there were significant differences in pharmacokinetic parameters of naringenin and hesperitin between normal and GMD groups. It was suggested that the absorption of naringenin and hesperitin was increased, and the elimination processes of naringenin and hesperitin were slower in the GMD group than the normal group. The data are of value for further pharmacological studies of ZSTFG and would be useful to provide a reference for improving the therapeutic regimen of ZSTFG in clinical trials. 展开更多

目的观察复方倍他米松并白芍总苷治疗糜烂型口腔扁平苔藓的临床效果。方法 2015年12月—2016年12月,于青岛市市立医院口腔内科诊治的糜烂型口腔扁平苔藓的病人96例,采用随机数字表法分为3组,每组32例。A组口服白芍总苷胶囊,B组病损黏膜... 目的观察复方倍他米松并白芍总苷治疗糜烂型口腔扁平苔藓的临床效果。方法 2015年12月—2016年12月,于青岛市市立医院口腔内科诊治的糜烂型口腔扁平苔藓的病人96例,采用随机数字表法分为3组,每组32例。A组口服白芍总苷胶囊,B组病损黏膜下注射复方倍他米松,C组病损黏膜下注射复方倍他米松并口服白芍总苷胶囊,连续治疗4周后,根据临床治疗效果比较3组的临床疗效、不良反应以及复发率。结果 B组与C组临床疗效比较,差异无统计学意义(P>0.05),但均高于A组,差异有显著统计学意义(Z=29.391、23.859,P<0.05)。A、B、C组病人均未见明显不良反应。随访3月后,A、C组的糜烂复发率明显低于B组,差异具有统计学意义(χ^2=9.014、14.034,P<0.05)。结论 复方倍他米松并白芍总苷治疗糜烂型口腔扁平苔藓临床效果较好,可提高临床疗效,降低复发率,值得临床推广应用。 展开更多

Their structures were identified by analysis of spectroscopic data. 目的为系统研究参坤养血颗粒的化学成分,采用超高效液相色谱-四级杆/静电场轨道阱高分辨质谱(UPLCQ-Orbitrap HRMS)法建立一种快速、准确识别中药中复杂化学成分的定性分析方法。方法通过UPLC-Q-Orbitrap HRMS捕捉化合物的精准相对分... 目的为系统研究参坤养血颗粒的化学成分,采用超高效液相色谱-四级杆/静电场轨道阱高分辨质谱(UPLCQ-Orbitrap HRMS)法建立一种快速、准确识别中药中复杂化学成分的定性分析方法。方法通过UPLC-Q-Orbitrap HRMS捕捉化合物的精准相对分子质量以及多级碎片离子信息,同时将其与对照品的保留时间和质谱信息进行比对,并结合相关参考文献或Chemical Book等数据库信息最终实现对未知化合物的快速、准确定性。结果从参坤养血颗粒中共鉴定出51种化学成分,主要包括有机酸类、黄酮类、醌类、苷类和其他类。结论建立的方法可快速、准确、系统地识别参坤养血颗粒中多种化学成分,并为其质量控制、药效物质基础研究及进一步的临床应用提供扎实的理论依据和科学的研究思路。 展开更多

目的建立一测多评法(quantitative analysis single-marker,QAMS)测定复方人参片中8种苷类成分的含量。方法采用Agilent XDB-C(18)色谱柱(4.6 mm×250 mm,5μm),流动相水(A)-乙腈(B),梯度洗脱;体积流量1.0 m L·min^... 目的建立一测多评法(quantitative analysis single-marker,QAMS)测定复方人参片中8种苷类成分的含量。方法采用Agilent XDB-C(18)色谱柱(4.6 mm×250 mm,5μm),流动相水(A)-乙腈(B),梯度洗脱;体积流量1.0 m L·min^-1;检测波长203 nm;柱温:25℃。以人参皂苷Rb1为内标,计算人参皂苷Rg1、人参皂苷Re、淫羊藿苷、人参皂苷Rc、人参皂苷Rb2、人参皂苷Rb3、人参皂苷Rd的相对校正因子,测定其含量。结果8种成分在各自范围内线性关系良好(r≥0.999 0),加样回收率为95.6%~104.4%,RSD为1.22%~2.73%,QAMS测定结果与外标法测定结果无显著性差异。结论该法准确度、灵敏度高、专属性好、操作简单、重复性好。 展开更多

Achyranthes bidentata and Achyranthes aspera are saponin and steroid rich medicinal plants,used extensively for therapeutic treatments in Traditional Chinese Medicine(TCM)and Ayurveda.A.bidentata is reported to be one of the rare and extensively exploited medicinal plant species that face the issue of being endangered.Finding qualitative substitute with identical phyto-constituents contributing to similar composition and pharmacological benefits will help in reducing the burden of exploitation of the natural habitats of such plants.In the present study,a comparative metabolite analysis of the whole drug and specific tissues isolated by laser micro-dissection(LMD)was carried out for both the selected species,by use of ultra-high performance liquid chromatography-quadrupole time-of-flight mass spectrometry(UHPLC-QTOF MS).The results of the study indicate that the cortex and the medullary ray tissues are rich in their content of steroidal and saponin constituents such as(25S)-inokosterone-20,22-acetonide,ginsenoside Ro,bidentatoside II and achyranthoside B.Metabolite profiling of the whole tissues of both the species indicates presence of identical constituents.Thus,it is inferred that A.bidentata and A.aspera can be used as qualitative substitutes for each other. 展开更多

采用紫外-可见分光光度法、红外光谱法和高效液相色谱法,分析了‘菲尔杜德’红树莓多酚粗提液和纯化液的组成成分,同时采用3个体外抗脂质过氧化指标研究了其抗脂质过氧化活性。组分分析结果表明红树莓多酚中可能含有黄酮类、花色苷、糖... 采用紫外-可见分光光度法、红外光谱法和高效液相色谱法,分析了‘菲尔杜德’红树莓多酚粗提液和纯化液的组成成分,同时采用3个体外抗脂质过氧化指标研究了其抗脂质过氧化活性。组分分析结果表明红树莓多酚中可能含有黄酮类、花色苷、糖苷类物质,初步鉴定出红树莓多酚中含有原花青素B2、芦丁、水杨酸和鞣花酸4种酚类物质,且红树莓多酚粗提液和纯化液中多酚组成具有差异性。抗脂质过氧化活性结果表明:在一定质量浓度范围内,红树莓多酚粗提液和纯化液对Fe~(2+)诱导卵黄脂蛋白脂质过氧化、H_2O_2诱导大鼠红细胞氧化溶血和大鼠肝组织匀浆自发脂质过氧化具有抑制作用,且呈现剂量依赖关系;红树莓多酚粗提液的抗脂质过氧化活性强于纯化液。红树莓多酚提取物对脂质过氧化有抑制作用,但其作用机制和途径仍需进一步研究。 展开更多

以乙醇为液化剂,浓硫酸为催化剂,在体积250 mL的高温高压反应釜中对竹粉进行液化,考察了不同反应条件对竹粉液化的影响,并对液化产物进行中和、逐级分离等处理。液化结果表明:在催化剂浓H2SO4用量5%、反应时间30 min、反应温度180℃条... 以乙醇为液化剂,浓硫酸为催化剂,在体积250 mL的高温高压反应釜中对竹粉进行液化,考察了不同反应条件对竹粉液化的影响,并对液化产物进行中和、逐级分离等处理。液化结果表明:在催化剂浓H2SO4用量5%、反应时间30 min、反应温度180℃条件下,竹粉液化率为69.67%,产物中糖苷得率为39.86%,酚类物质得率为20.7%。液化产物逐级分离得到乙基糖苷及3种酚类物质两类平台化合物,其GC-MS分析结果表明:3种酚类物质的主要成分为2,6-二甲氧基苯酚,3,5-二甲氧基-4-邻羟基苯乙酸,4-烯丙基-2,6-二甲氧基苯酚和绵马酚等,约占酚类物质总量的80%;糖苷中主要包含五碳糖苷(呋喃糖苷)和六碳糖苷(吡喃葡萄糖苷和半乳糖苷),占糖苷总量的88.62%。 展开更多
