为了实现交换机外壳上钉类安装正确与否的精确检测,文章提出一种基于改进卷积神经网络的图像处理识别算法。首先对交换机外壳图像进行Canny算子提取边缘,用透视变换算法与模板形成统一尺寸,定位截取出样本,然后把不同环境下的样本输入Ca... 为了实现交换机外壳上钉类安装正确与否的精确检测,文章提出一种基于改进卷积神经网络的图像处理识别算法。首先对交换机外壳图像进行Canny算子提取边缘,用透视变换算法与模板形成统一尺寸,定位截取出样本,然后把不同环境下的样本输入Caffe框架下构建的深度卷积神经网络,CPU模式下训练识别模型,最后运用于实际工程中。测试分析结果表明,随着不同钉类样本数据量增加,网络结构不能完全满足检测需求,进而采用一种基于产品类型的不同模型训练优化策略。实验结果表明,改进的算法可以快速训练模型并且结合策略可使不同产品检测准确度达99%以上,有效提高了交换机外壳上钉类安装的检测精度。 展开更多

Purpose:Open tibia fracture is prone to infection,consequently causing significant morbidity and increasing the hospital stay,occupational loss and onset of chronic osteomyelitis.Intramedullary nailing is one choice for treating tibia shaft fractures.To improve the delivery of antibiotics at the tissue-implant interface,many methods have been proposed as a part of prophylaxis against infection.This study was conducted to study the role of gentamicin-impregnated intramedullary interlocking(IMIL)nail in the prevention of infection in Gustilo typeⅠandⅡopen tibia fractures and to compare the results with regular intramedullary nail.Methods:The study included 28 patients with open tibia fractures(Gustilo type 1 or type 2);of them 14 underwent regular IMIL nailing and the other 14 were treated with gentamicin-coated nailing.Randomization was done by alternate allocation of the patients.Follow-up was done postoperatively(day 1),1 week,6 weeks,and 6 months for bone union,erythrocyte sedimentation rate(ESR),hemoglobin and C?reactive protein(CRP).Statistical significance was tested using unpaired t-test.A p value less than 0.05 was considered significant.Results:There were 4 cases of infection in controls(regular IMIL nail)and no infection among patients treated with gentamicin-coated nail during the follow up(Х^2=4.66,p=0.031).At 6 months postoperatively,CRP(p=0.031),ESR(p=0.046)and hemoglobin level(p=0.016)showed significant difference between two groups.The bone healing rate was better with gentamicin-coated nail in comparison to regular IMIL nail at 6 months follow-up(p=0.016).Conclusion:Gentamicin-coated IMIL nail has a positive role in preventing infection in Gustilo typeⅠandⅡopen tibia fractures. 展开更多

However, over the last two decades, there has been an increasing trend ... Tibial shaft fractures are common injuries among the pediatric and adolescent population. Conservative treatment remains the preferred treatment. However, over the last two decades, there has been an increasing trend of operative fixation in pediatric tibia fractures, commonly with intramedullary nail fixation (IMN). Elastic stable intramedullary nails (ESIN) are heavily used especially in skeletally immature patients as they are physeal respecting and the technique for insertion is familiar. Alternatively, reamed locked intramedullary nails (RIMN) have gained traction in adolescents and skeletally immature pre-adolescents. When identifying publications germane to intramedullary fixation of pediatric tibia fractures, the majority investigated clinical and radiographic outcomes associated with ESIN. We were able to identify only one study specifically examining RIMN in this population, albeit other studies included patients treated with RIMN. In parallel, there has been considerable progress in the field of skeletal maturity estimation with criteria based on different anatomic regions. However, little data exists for trauma purposes as no gold standard system had been accepted and proven to be precise for determination of potential growth remaining around the knee or for quantifying the risk of damage to the proximal tibial physis. Systems devised have been either unvalidated or unnecessarily complex or both. In order to achieve more informed treatment choices and optimal patient outcomes when using IMN fixation in pediatrics, simple to use, validated plain film-based methodology is needed to define skeletal maturity for the proximal tibia. Additionally, further examination of outcomes and the role of RIMN in this population are warranted. 展开更多

为研究爆燃驱动式射钉侵彻靶板深度的影响因素及规律,对直径7mm的射钉垂直侵彻20mm厚Q235靶板进行数值模拟.基于LS-DYNA有限元软件,选择Johnson-Cook本构模型和Gruneison状态方程对射钉侵彻靶板过程进行仿真模拟,并通过侵彻试验验证模... 为研究爆燃驱动式射钉侵彻靶板深度的影响因素及规律,对直径7mm的射钉垂直侵彻20mm厚Q235靶板进行数值模拟.基于LS-DYNA有限元软件,选择Johnson-Cook本构模型和Gruneison状态方程对射钉侵彻靶板过程进行仿真模拟,并通过侵彻试验验证模型的合理性.采用所建立的仿真模型,模拟分析射钉的头部锥角和材料强度、炸药驱动力、射钉与靶板初始间隙等因素对Q235靶板侵彻深度的影响规律.结果表明:随着射钉头部锥角增加,侵彻阻力不断增大,射钉最大速度逐渐减小,侵彻深度及有效侵彻深度不断减小;射钉材料的静屈服强度低于700MPa时,由于钉体发生明显的墩粗变形,侵彻深度较浅,而高于700MPa时,射钉材料的静屈服强度对侵彻深度影响较小;炸药爆燃产生的气体压强低于150MPa时,侵彻深度随气体压强增大近似呈线性增加,而高于150MPa时,靶板背面变形使应力得到释放,侵彻深度显著提高;随射钉与靶板初始间隙增加,射钉在接触靶板前可获得更高的速度,侵彻深度不断增大,但超过20mm后,初始间隙对侵彻深度基本没有影响. 展开更多

针对我国棉花覆膜种植模式所造成的大面积农田残膜污染问题,在实施耐候地膜新的国家标准基础上,基于耐候地膜在回收时的完整性,设计了一种可将膜面翻转而清除杂质的随动式残膜捡拾装置。通过测试耐候地膜的力学特性,计算捡拾时残膜的受... 针对我国棉花覆膜种植模式所造成的大面积农田残膜污染问题,在实施耐候地膜新的国家标准基础上,基于耐候地膜在回收时的完整性,设计了一种可将膜面翻转而清除杂质的随动式残膜捡拾装置。通过测试耐候地膜的力学特性,计算捡拾时残膜的受力情况,表明捡拾时残膜的受力小于其平均纵向拉断力;对样机关键作业部件进行设计和分析,运用Matlab软件绘制捡拾钉齿端点在土壤中的运动轨迹,确定了捡拾钉齿长度及捡拾钉齿在链板上的排列尺寸;通过捡拾滚筒上链板间距与起膜率、清杂率关系的单因素试验,得到链板间距的最优值;对捡拾滚筒、捡拾钉齿装置运动过程的受力情况进行了分析计算,并对样机的作业性能进行了试验研究。田间试验结果表明,当随动式残膜回收机的前进速度为4.0~4.4km/h时,残膜回收率为89.54%,表明其捡拾装置满足田间作业要求。 展开更多

Previous numerous animal, biomechanical and clinical studies have documented the short-term performance of the intramedullary nails;however, the long-term influence of the nails on the change of bone density distribution is still not fully understood.According to bone remodeling theory, a set of computational algorithms was implemented in the finite element model to predict the bone density distribution for retained intramedullary nail. The bone density was recorded to observe the progression of remodeling. The results show that the effect of intramedullary nail on the Gruen zone 4 and 5 of femur is small, while the effect on the Gruen zone 1, 2 and 6 is greater than other zones.This simulation indicates that intramedullary nail plays a significant role in the long-term bone density distribution after surgery. 展开更多

目的探讨肌骨超声在银屑病性关节炎(PsA)诊断中的应用价值。方法方便选择该院2016年6月—2017年12月收治的确诊为关节炎型银屑病患者35例及寻常型银屑病患者36例为研究对象,对所有受检者行体格检查,观察指甲受累情况。超声扫查双手指甲... 目的探讨肌骨超声在银屑病性关节炎(PsA)诊断中的应用价值。方法方便选择该院2016年6月—2017年12月收治的确诊为关节炎型银屑病患者35例及寻常型银屑病患者36例为研究对象,对所有受检者行体格检查,观察指甲受累情况。超声扫查双手指甲及双侧下肢主要附着点(包括股四头肌腱、髌腱起止端、跟腱末端及跖底筋膜附着端等),统计两组别附着点炎发生部位、百分率及声像特点,进行统计学分析。结果关节炎型银屑病组指甲病变发生率明显高于寻常型银屑病组(χ^2=19.73,P<0.01);超声检查发现关节炎型银屑病组30例(30/35,85.7%)及寻常型银屑病组15例(15/36,41.7%)发生附着点炎。附着点炎累及率关节炎型银屑病组[28.3%(99/350)]明显高于寻常型银屑病组[13.3%(48/360)](χ^2=24.33,P<0.01)。关节炎型银屑病组附着点炎最易累及跟腱、股四头肌腱及跖底筋膜附着端。附着处肌腱内钙化、附着处骨侵蚀及肌腱内血流信号增多发生率关节炎型银屑病组均高于寻常型银屑病组(χ^2=6.98,10.23,7.57,P<0.01)。结论超声能检出PsA患者附着点及指甲病变,对PsA的诊断具有重要参考价值。 展开更多

Objective:To explore the effect of magnetic-guided intramedullary nail fixation on bone metabolism and trauma degree in patients with femoral shaft fracture.Methods: A total of 128 patients with femoral shaft fractures who received surgical treatment in the hospital between April 2016 and November 2017 were divided into control group (n=64) and study group (n=64) according to the random number table method. Control group received the traditional intramedullary nail treatment, and study group received magnetic-guided intramedullary nail treatment. The differences in serum levels of bone metabolism indexes and inflammatory factors were compared between the two groups 48 h after surgery.Results: 48 h after surgery, serum bone formation indexes BGP, PⅠNP, PⅠCP and BAP levels of study group were higher than those of control group whereas bone resorption indexesβ-CTX, TRACP5b and NTX levels were lower than those of control group;serum inflammatory factors TGF-β, hs-CRP, IL-1β, IL-6 and IL-17 levels were lower than those of control group.Conclusion:Compared with traditional intramedullary nail therapy, magnetic-guided intramedullary nail fixation can more effectively balance the bone metabolism status and reduce the fracture end trauma in patients with femoral shaft fracture. 展开更多

Objective:To analyze the trauma and bone metabolism of magnetic navigation intramedullary nail and traditional intramedullary nail fixation treatment of femoral shaft fracture. Methods:58 patients with femoral shaft fracture treated in our hospital between December 2011 and December 2015 were divided into observation group and control group by random number table (n=29). Control group received conventional intramedullary nail fixation treatment, and observation group received magnetic navigation intramedullary nail fixation treatment. 24 h after surgery, blood coagulation indexes, enzymology indexes, bone metabolism indexes and angiogenesis indexes were determined;6 months after surgery, bone mineral density levels were determined. Results:24 h after surgery, peripheral blood thrombin time (TT), prothrombin time (PT), and activated partial thromboplastin time (APTT) levels of observation group were significantly higher than those of control group, and serum fibrinogen (FIB), D-Dimer (D-D), lactate dehydrogenase (LDH), creatine kinase (CK), creatine kinase isoenzyme (CKMB), glutamic oxalacetic transaminase (GOT), sex hormone-binding globulin type I (SHBG), collagen cross-linked carboxyl-terminal telopeptide (CTX) and deoxypyridinoline (DPD) content were lower than those of control group while bone gla protein (BGP), insulin-like growth factor (IGF-1), hypoxia-inducible factor-1α (HIF-α), angiogenin 1 (Ang-1), recombinant basic fibroblast growth factor (bFGF) and vascular endothelial growth factor (VEGF) content were higher than those of control group;6 months after surgery, fracture end bone mineral density (BMD) value of observation group was higher than that of control group. Conclusions:Magnetic navigation intramedullary nail treatment of femoral shaft fracture can more effectively reduce the surgical trauma, improve bone metabolism and increase bone mineral density. 展开更多

近十几年来,奎屯垦区高砷地下水被广泛用于农业灌溉,会在一定程度上降低改水措施对防治砷地方病的效果。为了解高砷地下水灌溉对居民体内砷累积的影响,选择头发和指甲作为评价指标,通过随机采集样品并用氢化物原子荧光光谱法测定样品中... 近十几年来,奎屯垦区高砷地下水被广泛用于农业灌溉,会在一定程度上降低改水措施对防治砷地方病的效果。为了解高砷地下水灌溉对居民体内砷累积的影响,选择头发和指甲作为评价指标,通过随机采集样品并用氢化物原子荧光光谱法测定样品中总As及As(Ⅲ)质量比的方式,对奎屯高砷地下水灌溉区居民砷污染健康影响现况进行了研究。结果表明,头发样品中总砷质量比为0.20-1.47μg/g(n=37),其中As(Ⅲ)质量比为0.06-0.67μg/g(n=32);51.35%的样品中总砷质量比高于卫生部标准(0.60μg/g),18.92%高于中国居民头发总砷正常值上限建议值(1.00μg/g),头发总砷质量比随居民居住年限和年龄增大而增大。指甲样品中总砷质量比为0.08-0.35μg/g(n=13),且与头发中总砷质量比呈正相关,当地居民体内呈现缓慢的As蓄积特征。改水30年后,奎屯垦区居民体内仍存在较高的砷水平,高砷地下水灌溉造成的人体健康风险不容忽视。 展开更多

Objective:To evaluate the bone metabolism balance and traumatic reaction of minimally invasive mippo intramedullary nail internal fixation treatment of femoral shaft fractures. Methods:80 patients with femoral shaft fractures who were treated in our hospital between May 2011 and December 2016 were collected and divided into control group (n=40) and observation group (n=40) according to random number table, control group received conventional steel plate internal fixation treatment, and observation group received minimally invasive mippo intramedullary nail internal fixation treatment. Differences in serum levels of bone formation indexes, bone resorption indexes, inflammatory factors, and pain mediators and so on were compared between two groups of patients before operation and 1 week after treatment.Results: Before operation, differences in serum levels of bone formation indexes, bone resorption indexes, inflammatory factors and pain mediators were not statistically significant between two groups of patients. After operation, serum bone formation indexes P ICP, BGP, BALP and ALP levels in observation group were higher than those in control group;serum bone resorption indexesβ-CTX and OPG levels were lower than those in control group;serum inflammatory factors IL-1β, IL-6, IL-8 and CRP levels were lower than those in control group;serum pain mediators SP, PGE2 and 5-HT levels were lower than those in control group.Conclusion:Minimally invasive mippo intramedullary nail internal fixation treatment of femoral shaft fractures can promote the bone formation, relatively inhibit bone resorption and cause less traumatic reaction. 展开更多

目的:探讨矢状位不稳定的转子间骨折的形态特征和复位技术。方法:回顾性分析北京大学人民医院创伤骨科2009年3月至2016年6月,通过股骨近端防旋转髓内钉(proximal femoral nail anti-rotation,PFNA)内固定手术治疗的矢状位不稳定转... 目的:探讨矢状位不稳定的转子间骨折的形态特征和复位技术。方法:回顾性分析北京大学人民医院创伤骨科2009年3月至2016年6月,通过股骨近端防旋转髓内钉(proximal femoral nail anti-rotation,PFNA)内固定手术治疗的矢状位不稳定转子间骨折,获得完整随访36例,其中切开复位组17例,微创复位组19例,分析手术时间、术中出血量、术中透视时间、尖顶距、螺旋刀片滑动距离、骨折愈合时间和髋关节Harris评分等指标,以及骨折的影像学形态特征。结果:平均随访15个月,微创复位组在手术出血量方面明显少于切开复位组(P〈0.001),而在其他评价指标等方面,两组之间差异无统计学意义。从骨折形态特征看,有分离移位的近端前向不稳定多于有嵌插的近端后向不稳定,且前者的突出特征是股骨近端前内侧面的长斜行骨折,其在远骨折端的对应区域形成V形缺损,在外旋位X线片上较明显。结论:矢状位不稳定骨折分为有嵌插的近端后向不稳定和有分离的近端前向不稳定两种类型,两种类型各有特征,手术应尽可能用微创技术进行复位。 展开更多
