Objective: The current surgical treatment guidelines for early proximal gastric cancer(PGC) still lack agreement. Lymphadenectomy of lymph nodes No. 5 and No. 6 is the major difference between total and proximal gastrectomy. We elucidated the appropriate surgical procedure for PGC by investigating the pathological characteristics and prognostic significance of lymph nodes No. 5 and No. 6.Methods: In total, 333 PGC patients who underwent total gastrectomy were enrolled in this study. We investigated their clinicopathological characteristics and the metastatic patterns of the lymph nodes. Patients with metastasis in lymph nodes No. 5 and No. 6 were combined into one group and we compared the difference in survival between those with and without metastasis in lymph nodes No. 5, 6(lymph nodes No. 5 and No. 6 in any group of metastasis) for different subgroups.Results: The metastatic rates for lymph nodes No. 5 and No. 6 in PGC were 9.91% and 16.11%, respectively.The metastatic rate for both lymph nodes No. 5, 6 was 20.42%. Multivariate analysis showed that positive metastasis in lymph node No. 4, depth of invasion, and tumor size were independently correlated with the presence of metastasis in lymph nodes No. 5, 6.Conclusions: When lymph node No. 4 is positive(intraoperative pathology) or tumor size ≥5 cm or T4 stage,lymphadenectomy should be performed for lymph nodes No. 5 and No. 6, and total gastrectomy is recommended. 展开更多

This paper investigates the finite-time synchronization for a class of linearly coupled dynamical complex networks with both nonidentical nodes and uncertain disturbance. A set of controllers are designed such that the considered system can be finite-timely synchronized onto the target node. Based on the stability of the error equation, the Lyapunov function method and the linear matrix inequality technique, several sufficient conditions are derived to ensure the finite-time synchronization, and applied to the case of identical nodes and tlie one without uncertain disturbance. Also the adaptive finite-time synchronization is discussed. A numerical example is given to show the effectiveness of the main results obtained. 展开更多

BACKGROUND Radical D2 lymphadenectomy for advanced gastric cancer as a standard procedure has gained global consensus. Mounting studies have shown that the number of lymph nodes dissection directly affects the prognosis and recurrence of gastric cancer. Our previous study showed that there was no obvious lymph node around the abnormal hepatic artery derived from the superior mesenteric artery. AIM To investigate the relationship between celiac artery variation and the number of lymph nodes dissection in gastric cancer surgery. METHODS The clinicopathological data of 421 patients treated with radical D2 lymphadenectomy were analyzed retrospectively. The difference of the number of lymph nodes dissection between the celiac artery variation group and the normal vessels group and the relationship with prognosis were analyzed. RESULTS Celiac artery variation was found in 110 patients, with a variation rate of 26.13%. Celiac artery variation, tumor staging, and Borrmann typing were factors that affected lymph node clearance in gastric cancer, and the number of lymph nodes dissection in patients with celiac artery variation was significantly less than that of non-variant groups (P < 0.05). Univariate analysis showed that there was no significant difference in survival time between the two groups (P > 0.05). Univariate and multiple Cox regression analysis showed that celiac artery variation was not a prognostic factor for gastric cancer (P > 0.05). Tumor staging, intraoperative bleeding, and positive lymph node ratio were prognostic factors for gastric cancer patients (all P < 0.05). CONCLUSION The number of lymph nodes dissection in patients with celiac artery variation was reduced, but there was no obvious effect on prognosis. Therefore, lymph nodes around the abnormal hepatic artery may not need to be dissected in radical D2 lymphadenectomy. 展开更多

BACKGROUND Neoadjuvant chemotherapy has been applied worldwide to improve the survival of patients with gastric adenocarcinoma (GAC). The evaluation of histological regression in primary tumors is valuable for predicting prognosis. However, the prognostic effect of regression change in lymph nodes (LNs) remains unclear. AIM To confirm whether the evaluation of regression change in LNs could predict the prognosis of GAC patients who received neoadjuvant chemotherapy followed by surgery. METHODS In this study, we evaluated the histological regression of resected LNs from 192 GAC patients (including those with esophagogastric junction adenocarcinoma) treated with neoadjuvant chemotherapy. We classified regression change and residual tumor in LNs into four groups:(A) true negative LNs with no evidence of a preoperative therapy effect,(B) no residual metastasis but the presence of regression change in LNs,(C) residual metastasis with regression change in LNs, and (D) metastasis with minimal or no regression change in LNs. Correlations between regression change and residual tumor groups in LNs and regression change in the primary tumor, as well as correlations between regression change in LNs and clinicopathological characteristics, were analyzed. The prognostic effect of regression change and residual tumor groups in LNs was also analyzed. RESULTS We found that regression change and residual tumor groups in LNs were significantly correlated with regression change in the primary tumor, tumor differentiation, ypT stage, ypN stage, ypTNM stage, lymph-vascular invasion, perineural invasion and R0 resection status. Regression change and residual tumor groups in LNs were statistically significant using univariate Cox proportional hazards analysis, but were not independent predictors. For patients who had no residual tumor in LNs, the 5-year overall survival (OS) rates were 67.5% in Group A and 67.4% in Group B. Species specialization limits movement ability and shapes ecological networks: the case study of 2 forest mammals

To counteract the negative effects of forest fragmentation on wildlife, it is crucial to maintain functional ecological networks. We identified the ecological networks for 2 mammals with very different degrees of forest specialization, the European badger Meles metes and the Roe deer Capreolus capreolus, by differentiati ng 4 agroforestry eleme nts as either no des or conn ectivity elements, and by defining the distanee that provides the functional connectivity between fragments. Species occurrenee data were collected in a wide agroecosystem in northern Italy. To test the role of hedgerows, traditional poplar cultivations, short rotation coppices, and reforestations as ecological network elements for the 2 species we applied the method of simulated species perceptions of the Iandscape (SSPL), comparing the ability of different SSPLs to explain the observed species distribution. All analyses were repeated considering different seenarios of species movement ability through the matrix. Model outputs seem to show that the specialist and highly mobile Roe deer has the same movement ability throughout the matrix (2 km) as the European badger, a smaller, but generalist species. The ecological network identified for the European badger was widespread throughout the area and was composed of woodlands, poplar cultivations and hedgerows as nodes and short rotation coppices as connectivity elements. Conversely, the ecological network of the Roe deer was mostly limited to the main forest areas and was composed of woodlands, poplar cultivations and ref o restat i ons as no des and short rotation coppices and hedgerows as conn ectivity elements. The degree of forest specialization strongly affects both species perception of habitat and movement ability throughout the matrix, regardless of species size. Background: An artificial intelligence system of Faster Region-based Convolutional Neural Network (Faster R-CNN) is newly developed for the diagnosis of metastatic lymph node (LN) in rectal cancer patients. The primary objective of this study was to comprehensively verify its accuracy in clinical use. Methods: Four hundred fourteen patients with rectal cancer discharged between January 2013 and March 2015 were collected from 6 clinical centers, and the magnetic resonance imaging data for pelvic metastatic LNs of each patient was identified by Faster R-CNN. Faster R-CNN based diagnoses were compared with radiologist based diagnoses and pathologist based diagnoses for methodological verification, using correlation analyses and consistency check. For clinical verification, the patients were retrospectively followed up by telephone for 36 months, with post-operative recurrence of rectal cancer as a clinical outcome;recurrence-free survivals of the patients were compared among different diagnostic groups, by methods of Kaplan-Meier and Cox hazards regression model. Results: Significant correlations were observed between any 2 factors among the numbers of metastatic LNs separately diagnosed by radiologists, Faster R-CNN and pathologists, as evidenced by rradiologist-Faster R-CNN of 0.912, rPathologist-radiologist of 0.134, and rPathologist-Faster R-CNN of 0.448 respectively. The value of kappa coefficient in N staging between Faster R-CNN and pathologists was 0.573, and this value between radiologists and pathologists was 0.473. The 3 groups of Faster R-CNN, radiologists and pathologists showed no significant differences in the recurrence-free survival time for stage N0 and N1 patients, but significant differences were found for stage N2 patients. Conclusion: Faster R-CNN surpasses radiologists in the evaluation of pelvic metastatic LNs of rectal cancer, but is not on par with pathologists. 展开更多

Structural controllability is critical for operating and controlling large-scale complex networks. In real applications, for a given network, it is always desirable to have more selections for driver nodes which make the network structurally controllable. Different from the works in complex network field where structural controllability is often used to explore the emergence properties of complex networks at a macro level,in this paper, we investigate it for control design purpose at the application level and focus on describing and obtaining the solution space for all selections of driver nodes to guarantee structural controllability. In accord with practical applications,we define the complete selection rule set as the solution space which is composed of a series of selection rules expressed by intuitive algebraic forms. It explicitly indicates which nodes must be controlled and how many nodes need to be controlled in a node set and thus is particularly helpful for freely selecting driver nodes. Based on two algebraic criteria of structural controllability, we separately develop an input-connectivity algorithm and a relevancy algorithm to deduce selection rules for driver nodes. In order to reduce the computational complexity,we propose a pretreatment algorithm to reduce the scale of network's structural matrix efficiently, and a rearrangement algorithm to partition the matrix into several smaller ones. A general procedure is proposed to get the complete selection rule set for driver nodes which guarantee network's structural controllability. Simulation tests with efficiency analysis of the proposed algorithms are given and the result of applying the proposed procedure to some real networks is also shown, and these all indicate the validity of the proposed procedure. 展开更多

At present,most underwater positioning algorithms improve the positioning accuracy by increasing the number of anchor nodes which resulting in the increasing energy consumption.To solve this problem,the paper proposes a localization algorithm assisted by mobile anchor node and based on region determination(LMRD),which not only improves the positioning accuracy of nodes positioning but also reduces the energy consumption.This algorithm is divided into two stages:region determination stage and location positioning stage.In the region determination stage,the target region is divided into several sub-regions by the region division strategy with the smallest overlap rate which can reduce the number of virtual anchor nodes and lock the target node to a sub-region,and then through the planning of mobile nodes to optimize the travel path,reduce the moving distance,and reduce system energy consumption.In the location positioning stage,the target node location can be calculated using the HILBERT path planning and trilateration.The simulation results show that the proposed algorithm can improve the positioning accuracy when the energy consumption is reduced. 展开更多

Endoscopic ultrasound-guided fine-needle aspiration biopsy (EUS-FNA) is a useful procedure that enables reliable pathological diagnoses of pancreatobiliary diseases, subepithelial lesions, and swollen lymph nodes. In recent years, a pathological diagnosis based on EUS-FNA has made it possible to provide accurate treatment methods not only in these fields, but also in respiratory organs and otorhinolaryngology. BACKGROUND In recent years,the incidence of fungal infection has been increasing,often invading one or more systems of the body.However,it is rare for lymph nodes to be invaded without the involvement of other organs.CASE SUMMARY A 21-year-old man was admitted to hospital for repeated cough for 2 mo and abdominal pain for 1 mo.Physical examination revealed multiple lymph nodes enlargement,especially those in the left neck and groin.CT scan showed multiple lymph nodes enlargement in the chest,especially left lung,abdominal cavity,and retroperitoneum.The first lymph node biopsy revealed granulomatous lesions of lymph nodes,so intravenous infusion of Cefoperazone tazobactam combined with anti-tuberculosis drugs were given.Because fever and respiratory failure occurred 4 d after admission,mechanical ventilation was given,and Caspofungin and Voriconazole were used successively.However,the disease still could not be controlled.On the 11th day of admission,the body temperature reached 40° C.After mycosis of lymph nodes was confirmed by the second lymph node biopsy,Amphotericin B was given,and the patient recovered and was discharged from the hospital.CONCLUSION No fixed target organ was identified in this case,and only lymph node involvement was found.Caspofungin,a new antifungal drug,and the conventional first choice drug,Voriconazole,were ineffective,while Amphotericin B was effective. 展开更多

The Five-hundred-meter Aperture Spherical radio Telescope(FAST) has an active reflector.During observations, the reflector will be deformed into a paraboloid 300 meters in diameter. To improve its surface accuracy, we propose a scheme for photogrammetry to measure the positions of 2226 nodes on the reflector. The way to detect the nodes in the photos is the key problem in this application of photogrammetry. This paper applies a convolutional neural network(CNN) with candidate regions to detect the nodes in the photos. Experimental results show a high recognition rate of 91.5%, which is much higher than the recognition rate for traditional edge detection. 展开更多
