Association between obstructive sleep apnea and left ventricular remodeling:Geometry pattern and a gender effect

Background Previous studies have suggested an association between obstructive sleep apnea(OSA)and left ventricular(LV)remodeling,but it remains unclear whether there's a specific remodeling pattern related to the OSA.Methods This study was a cross-sectional study of 1377 cardiovascular disease(CVD)patients.Respiratory events during sleep were recorded by portable monitors and apnea hypopnea index(AHI)was used to quantify OSA severity.Echocardiography was used to assess left ventricular structure.The left ventricular geometry was divided into four groups according to left ventricular mass index(LVMI)and relative wall thickness(RWT):normal geometry,concentric remodeling,concentric hypertrophy and eccentric hypertrophy.Results The prevalence of OSA was significantly higher in patients with abnormal left ventricular geometry(75.2%vs.68.9%,P=0.012).Compared to patients with AHI<5,those with AHI M 30 had an odds ratio of 1.53(95%CI:1.03-2.28)for LV hypertrophy in male patients,after adjustment for age,body mass index,hypertension,diabetes,coronary artery disease and metabolism syndrome.Furthermore,after adjustment for confounding factor,severe OSA(compared with non-OSA)was significantly associated with the increased risk of eccentric hypertrophy in men(OR:2.34,95%CI:1.23-4.44).No association was observed between OSA and other types of LV remodeling in neither men or women.Conclusions OSA is implicated in LV remodeling and severe OSA is independently associated with a higher risk of eccentric LV hypertrophy in male patients. 展开更多

目的探讨采用耳软骨移植重塑鼻尖外形的疗效。方法自2015年5月至2017年10月,直视下采用耳软骨移植重塑25例患者的鼻尖外形。鼻尖低平18例,鼻尖轻度上旋伴鼻孔外露4例,鼻尖低垂伴鼻根鼻背低平3例。20例为首次手术,5例为二次修复手术。结... 目的探讨采用耳软骨移植重塑鼻尖外形的疗效。方法自2015年5月至2017年10月,直视下采用耳软骨移植重塑25例患者的鼻尖外形。鼻尖低平18例,鼻尖轻度上旋伴鼻孔外露4例,鼻尖低垂伴鼻根鼻背低平3例。20例为首次手术,5例为二次修复手术。结果患者术后切口均一期愈合,耳软骨供区未变形。所有患者获随访3~12个月,术后瘢痕不明显,鼻尖外形精巧,圆润自然,结构清晰,颜色及手感满意,无鼻尖皮肤发红等并发症。结论采用耳软骨移植重塑鼻尖,可改善缺陷的鼻尖外形,提升了面部的整体美感,使鼻部的形态、结构和功能完美结合。 展开更多

Quantitative computed tomography measurement of cross-sectional area of small pulmonary vessels in asthmatic patients

Background: Cross-sectional area (CSA) for small pulmonary vessels is considered a parameter of pulmonary vessel alterations in patients with chronic obstructive pulmonary disease. This study was to evaluate the correlation of CSA with airflow obstruction parameters in asthma. Furthermore, we aimed to measure the difference in vascular alteration between asthma phenotypes and evaluate its relation with cytokine levels. Methods: We consecutively enrolled 20 adult asthmatic patients (13 women: age range, 26–80 years) and 20 healthy controls (8 women: age range, 23–61 years) from Peking University Third Hospital. Total CSA <5 mm2 (CSA<5) was measured with 64-slice spiral computed tomography, and the percentage CSA <5 for the lung area (%CSA<5) was calculated. Data were corrected for body surface area to obtain sixth-generation airway luminal diameter (LDcor), luminal area (Aicor), and airway wall thickness, and airway wall area percentage (WA%) was calculated. Enzyme-linked immunosorbent assay was used to detect the expression of leptin, total immunoglobulin E, periostin, and transforming growth factor β1 in serum and matrix metalloproteinase 9 in induced sputum supernatant of asthmatic patients. The differences in %CSA<5 between subgroups were assessed by independent samples Student's t test, and Spearman correlation analysis was used to analyze the correlation of %CSA<5 with clinical indexes and inflammatory cytokine levels. Results: Patients with asthma and controls did not differ in %CSA<5. In asthma patients,%CSA<5 was lower with initial onset age ≤12 years old, airflow restriction and uncontrolled Global Initiative for Asthma classification (all P < 0.05). Moreover, it was positively correlated with forced vital capacity ratio in 1 s (FEV1)/forced expiratory volume ratio, FEV1%, LDcor, Aicor, and serum leptin level (all P < 0.05) and negatively with total lung WA%(P = 0.007). The detrimental effects of constant hyperglycemia on neural function have been quantitatively and qualitatively evaluated in the setting of diabetes mellitus. Some of the hallmark features of diabetic encephalopathy (DE) are impaired synaptic adaptation and diminished spatial learning capacity. Chronic and progressive cognitive dysfunction, perpetuated by several positive feedback mechanisms in diabetic subjects, facilitates the development of early-onset dementia and Alzheimer’s disease. Despite the numerous clinical manifestations of DE having been described in detail and their pathophysiological substrate having been elucidated in both type 1 and type 2 diabetes mellitus, an effective therapeutic approach is yet to be proposed. Therefore, the aim of this review is to summarize the growing body of evidence concerning the effect of current antidiabetic treatment options on diabetic and non-DE. 展开更多

Vascular remodeling is the essential pathogenic process of various cardiovascular disorders,including hypertension,atherosclerosis,stroke,and restenosis after vein graft.The main characterization of vascular remodeling is abnormal variations of vascular cell phenotype,morphological structure and functions such as migration,hypertrophy,proliferation and apoptosis.Numerous researches revealed that mechanical stress,including shear stress and cyclic stretch,participates in physiological vascular homeostasis,or pathophysiological vascular remodeling.The understanding of mechanobiological mechanism in vascular remodeling will play a unique role in understanding human physiology and disease,and will generate important theoretical and clinical significance [2].Non-coding RNAs are newly recognized RNAs which cannot be translated into proteins but are involved in epigenetic modification of gene regulation.The studies revealed that non-coding RNAs,such as microRNAs(miRNAs)and long noncoding RNAs(long ncRNAs,IncRNA),as well as small interfering RNAs(siRNAs),piwi-interacting RNAs(piRNAs),small nucleolar RNAs(snoRNAs),play essential roles in the regulation of various processes,such as metabolism,development,cell proliferation,cell apoptosis,cell differentiation,oncogenesis and vascular homeostasis[5].However,the roles of non-coding RNAs in the cardiovascular system under mechanical stresses are still not clarified.Our recent researches detected the mechanical regulation of IncRNAs and miRNAs in vascular remodeling.LncRNAs are non-protein-coding transcripts that are longer than 200 nucleotides(nt),which is an arbitrary cut-off value that distinguishes these transcripts from other small RNAs.Unlike the well-established mechanism of microRNA action,the functional mode of IncRNAs is not fully understood.Increasing evidence shows that IncRNAs modulate gene expression via a multilevel-regulated pathway.Given their large number and complicated functional modes,lncRNAs are emerging as important regulators of a variety of cellular res 展开更多

Traumatic brain injury is an important global public health problem.Traumatic brain injury not only causes neural cell death,but also induces dendritic spine degeneration.Spared neurons from cell death in the injured brain may exhibit dendrite damage,dendritic spine degeneration,mature spine loss,synapse loss,and impairment of activity.Dendritic degeneration and synapse loss may significantly contribute to functional impairments and neurological disorders following traumatic brain injury.Normal function of the nervous system depends on maintenance of the functionally intact synaptic connections between the presynaptic and postsynaptic spines from neurons and their target cells.During synaptic plasticity,the numbers and shapes of dendritic spines undergo dynamic reorganization.Enlargement of spine heads and the formation and stabilization of new spines are associated with long-term potentiation,while spine shrinkage and retraction are associated with long-term depression.Consolidation of memory is associated with remodeling and growth of preexisting synapses and the formation of new synapses.To date,there is no effective treatment to prevent dendritic degeneration and synapse loss.This review outlines the current data related to treatments targeting dendritic spines that propose to enhance spine remodeling and improve functional recovery after traumatic brain injury.The mechanisms underlying proposed beneficial effects of therapy targeting dendritic spines remain elusive,possibly including blocking activation of Cofilin induced by beta amyloid,Ras activation,and inhibition of GSK-3 signaling pathway.Further understanding of the molecular and cellular mechanisms underlying synaptic degeneration/loss following traumatic brain injury will advance the understanding of the pathophysiology induced by traumatic brain injury and may lead to the development of novel treatments for traumatic brain injury. 展开更多

Objective: To investigate the analysis of atrial remodeling, fibrosis, oxidative stress and insulin resistance levels in patients with atrial fibrillation. Methods: 60 patients with atrial fibrillation treated in our hospital from December 2016 to January 2018 were selected as observation group, and 60 healthy persons in the same period were selected as control group. The expression levels of indexes in atrial remodeling [including osteoprotegerin (OPG), nuclear factor kappa beta receptor activating factor ligand (RANKL)], fibrosis [including transforming growth factor beta (TGF-beta) and tumor necrosis factor alpha (TNF-alpha)], oxidative stress [including reactive oxygen species (ROS) and advanced oxidation protein products (AOPP)] and insulin resistance [including human fibroblast growth factor -21 (FGF-21) and omentin-1 (Omentin-1)] were observed and compared between the two groups. Results: The level of Omentin-1 in observation group [(36.75±4.60) ug/L] was significantly lower than that in control group [(87.98±4.43) ug/L] (P<0.05). And there was no significant difference in AOPP level between the observation group and the control group [(13.75±2.13) micromol/L] (P>0.05). While the levels of atrial remodeling and fibrosis related indexes, ROS and FGF-21 in the observation group were significantly higher than those in the control group (P<0.05). Conclusions: Understanding the mechanism of atrial remodeling in patients with atrial fibrillation contributes to the prevention and treatment of atrial fibrillation. Patients with atrial fibrillation are not only prone to myocardial fibrosis, but also to oxidative stress and insulin resistance. The relevant indicators should be strengthened in clinical practice, so as to provide evidence for early diagnosis and treatment of the disease. 展开更多

Hyperglycemia reduces the number of circulating endothelial progenitor cells,accelerates their senescence and impairs their function.However,the relationship between blood glucose levels and endothelial progenitor cells in peripheral blood of patients with traumatic brain injury is unclear.In this study,101 traumatic brain injury patients admitted to the Department of Neurosurgery,Tianjin Medical University General Hospital or the Department of Neurosurgery,Tianjin Huanhu Hospital,China,were enrolled from April 2005 to March 2007.The number of circulating endothelial progenitor cells and blood glucose levels were measured at 1,4,7,14 and 21 days after traumatic brain injury by flow cytometry and automatic biochemical analysis,respectively.The number of circulating endothelial progenitor cells and blood sugar levels in 37 healthy control subjects were also examined.Compared with controls,the number of circulating endothelial progenitor cells in traumatic brain injury patients was decreased at 1 day after injury,and then increased at 4 days after injury,and reached a peak at 7 days after injury.Compared with controls,blood glucose levels in traumatic brain injury patients peaked at 1 day and then decreased until 7 days and then remained stable.At 1,4,and 7 days after injury,the number of circulating endothelial progenitor cells was negatively correlated with blood sugar levels(r=?0.147,P<0.05).Our results verify that hyperglycemia in patients with traumatic brain injury is associated with decreased numbers of circulating endothelial progenitor cells.This study was approved by the Ethical Committee of Tianjin Medical University General Hospital,China(approval No.200501)in January 2015. 展开更多

The muscle RING-finger protein-1 (MuRFl) is an E3 ubiquitin ligase expressed in skeletal and cardiac muscle tissues and it plays important roles in muscle remodeling. Upregulation of MuRFl gene transcription participates in skeletal muscle atrophy, on contrary downregulation of protein expression leads to cardiac hypertrophy. MuRFl gene point mutations have been found to generate protein aggregate myopathies defined as muscle disorder characterized by protein accumulation in muscle fibers. We have discovered that MuRFl turned out to be also a target for a new post-translational modification arbitrated by conjugation of SUM01 and it is mediated by the SUMO ligases E2 UBC9 and the E3 PIASy/4. SUMOylation takes place at lysine 238 localized at the second coiled-coil protein domain that is required for efficient substrate interaction for polyubiquitination. We provided evidence that SUMOylation is essential for MuRFl nuclear translocation and its mitochondria accumulation is enhanced in hyper? glycemic conditions delivering a stabilization of the overall SUMOylated proteins in cultured myocytes. Thus, our findings add this SUM01 post-translational modification as a new concept to understand muscle disorders related to the defect in MuRFl activity. 展开更多

目的探讨大腿肌群等速训练对前交叉韧带(anterior cruciate ligament,ACL)重建术后移植物塑形的影响,总结规律以指导临床康复训练。方法选取2016年8月—12月40例接受关节镜下自体腘绳肌腱单束重建ACL手术患者,根据术后功能锻炼方案分为... 目的探讨大腿肌群等速训练对前交叉韧带(anterior cruciate ligament,ACL)重建术后移植物塑形的影响,总结规律以指导临床康复训练。方法选取2016年8月—12月40例接受关节镜下自体腘绳肌腱单束重建ACL手术患者,根据术后功能锻炼方案分为试验组及对照组,每组20例。对照组常规康复锻炼,试验组在此基础上于术后3~6个月替换为大腿肌群等速训练。最终试验组12例、对照组12例获完整随访纳入研究。两组患者性别、年龄、体质量指数、损伤侧别、受伤至手术时间、术前国际膝关节文献委员会(IKDC)评分等一般资料比较,差异均无统计学意义(P>0.05)。术后3、6、12个月及行二次关节镜手术(以下简称,二次镜检)时,行膝关节等速肌力测定,包括伸屈膝关节力矩峰值(peak torque,PT)以及腘绳肌与股四头肌肌力比值(hamstring quadriceps ratio,H/Q);MRI检查并行移植物评分。二次镜检时观察移植物外形、张力、血管化程度并评分,取样本行组织学观察并评分(血管分布、细胞形态)。结果试验组ACL重建术至二次镜检时间为(23.57±3.23)个月,对照组为(23.22±3.56)个月,差异无统计学意义(P>0.05)。二次镜检时,试验组IKDC评分为(90.45±4.73)分,对照组为(89.32±4.54)分,均较术前明显提高(P<0.05);组间比较差异无统计学意义(t=0.868,P=0.404)。膝关节等速肌力测定显示,术后3个月两组屈伸膝关节PT差异无统计学意义(P>0.05),术后6、12个月以及二次镜检时试验组屈伸膝关节PT均大于对照组(P<0.05)。两组术后3个月及二次镜检时H/Q差异无统计学意义(P>0.05),试验组术后6、12个月时H/Q均大于对照组(P<0.05)。术后3、6个月及二次镜检时两组MRI评分差异无统计学意义(P>0.05),术后12个月试验组MRI评分明显高于对照组(P<0.05)。二次镜下观测,试验组移植物评分与对照组比较差异无统计学意义(P>0.05),但血管分布及细胞形态评分均高于对� 展开更多

Dendritic shrinkage after injury: a cellular killer or a necessity for axonal regeneration? 预览

Dendrites form an essential component of the neuronal circuit have been largely overlooked in regenerative research. Nevertheless, subtle changes in the dendritic arbors of neurons are one of the first stages of various neurodegenerative diseases, leading to dysfunctional neuronal networks and ultimately cellular death. Maintaining dendrites is therefore considered an essential neuroprotective strategy. This mini-review aims to discuss an intriguing hypothesis, which postulates that dendritic shrinkage is an important stimulant to boost axonal regeneration, and thus that preserving dendrites might not be the ideal therapeutic method to regain a full functional network upon central nervous system damage. Indeed, our study in zebrafish, a versatile animal model with robust regenerative capacity recently unraveled that dendritic retraction is evoked prior to axonal regrowth after optic nerve injury. Strikingly, inhibiting dendritic pruning upon damage perturbed axonal regeneration. This constraining effect of dendrites on axonal regrowth has sporadically been proposed in literature, as summarized in this short narrative. In addition, the review discusses a plausible underlying mechanism for the observed antagonistic axon-dendrite interplay, which is based on energy restriction inside neurons. Axonal injury indeed leads to a high local energy demand in which efficient axonal energy supply is fundamental to ensure regrowth. At the same time, axonal lesion is known to induce m让ochondrial depolarization, causing energy depletion in the axonal compartment of damaged neurons. Mitochondria, however, become mostly stationary after development, which has been proposed as a potential underlying reason for the low regenerative capacity of adult mammals. Per contra, upon reduced neuronal activity, mitochondrial mobility enhances. In this view, dendritic shrinkage after axonal injury in zebrafish could result in less synaptic input and hence, a release of mitochondria within the soma-dendrite compartment that then translocate 展开更多
