BACKGROUND Small-cell lung cancer(SCLC)is a type of fatal tumor that is increasing in prevalence.While these are unpleasant facts to consider,it is vitally important to be informed,and it is important to catch the disease early.Typically,lung cancer does not show severe clinical symptoms in the early stage.Once lung cancer has progressed,patients might present with classical symptoms of respiratory system dysfunction.Thus,the prognosis of SCLC is closely related to the early diagnosis of the disease.Ectopic adrenocorticotropic hormone(ACTH)syndrome(EAS)is related to cancer occurrence,especially for SCLC with the presence of Cushing's syndrome,which is dependent on markedly elevated ACTH and cortisol levels.CASE SUMMARY In the current report,we describe two middle-age patients who were originally diagnosed with diabetes mellitus with no classical symptoms of lung cancer.The patients were eventually diagnosed with SCLC,which was confirmed by bronchoscopic biopsy and histopathology.SCLC-associated diabetes was related to EAS,which was an endogenous ACTH-dependent form of Cushing’s syndrome with elevated ACTH and cortisol levels.Multiple organ metastases were found in Patient 1,while Patient 2 retained good health at 2 years follow-up.EAS symptoms including thyroid dysfunction,hypercortisolism and glucose intolerance were all resolved after anticancer treatment.CONCLUSION In conclusion,SCLC might start with diabetes mellitus and increased cortisol and hypokalemia or other EAS symptoms.These complex clinical features were the most significant factors to deteriorate a patient’s condition.Early diagnosis and treatment from clinicians were essential for the anti-cancer treatment for patients with SCLC. 展开更多

Next-generation sequencing(NGS) technology is capable of sequencing millions or billions of DNA molecules simultaneously.Therefore, it represents a promising tool for the analysis of molecular targets for the initial diagnosis of disease, monitoring of disease progression, and identifying the mechanism of drug resistance. On behalf of the Tumor Biomarker Committee of the Chinese Society of Clinical Oncology(CSCO) and the China Actionable Genome Consortium(CAGC), the present expert group hereby proposes advisory guidelines on clinical applications of NGS technology for the analysis of cancer driver genes for precision cancer therapy. This group comprises an assembly of laboratory cancer geneticists, clinical oncologists, bioinformaticians,pathologists, and other professionals. After multiple rounds of discussions and revisions, the expert group has reached a preliminary consensus on the need of NGS in clinical diagnosis, its regulation, and compliance standards in clinical sample collection. Moreover, it has prepared NGS criteria, the sequencing standard operation procedure(SOP), data analysis, report, and NGS platform certification and validation. 展开更多

With the digestive endoscopic tunnel technique(DETT),many diseases that previously would have been treated by surgery are now endoscopically curable by establishing a submucosal tunnel between the mucosa and muscularis propria(MP).Through the tunnel,endoscopic diagnosis or treatment is performed for lesions in the mucosa,in the MP,and even outside the gastrointestinal(GI)tract.At present,the tunnel technique application range covers the following:(1)Treatment of lesions originating from the mucosal layer,e.g.,endoscopic submucosal tunnel dissection for oesophageal large or circular early-stage cancer or precancerosis;(2)treatment of lesions from the MP layer,per-oral endoscopic myotomy,submucosal tunnelling endoscopic resection,etc.;and(3)diagnosis and treatment of lesions outside the GI tract,such as resection of lymph nodes and benign tumour excision in the mediastinum or abdominal cavity.With the increasing number of DETTs performed worldwide,endoscopic tunnel therapeutics,which is based on DETT,has been gradually developed and optimized.However,there is not yet an expert consensus on DETT to regulate its indications,contraindications,surgical procedure,and postoperative treatment.The International DETT Alliance signed up this consensus to standardize the procedures of DETT.In this consensus,we describe the definition,mechanism,and significance of DETT,prevention of infection and concepts of DETTassociated complications,methods to establish a submucosal tunnel,and application of DETT for lesions in the mucosa,in the MP and outside the GI tract(indications and contraindications,procedures,pre-and postoperative treatments,effectiveness,complications and treatments,and a comparison between DETT and other operations). 展开更多

After nerve-root avulsion injury of the brachial plexus, oxidative damage, inflammatory reaction, and glial scar formation can affect nerve regeneration and functional recovery. Melatonin(MT) has been shown to have good anti-inflammatory, antioxidant, and neuroprotective effects. Chondroitin sulfate ABC(ChABC) has been shown to metabolize chondroitin sulfate proteoglycans and can reduce colloidal scar formation. However, the effect of any of these drugs alone in the recovery of nerve function after injury is not completely satisfactory. Therefore, this experiment aimed to explore the effect and mechanism of combined application of melatonin and chondroitin sulfate ABC on nerve regeneration and functional recovery after nerve-root avulsion of the brachial plexus. Fifty-two Sprague-Dawley rats were selected and their C5–7 nerve roots were avulsed. Then, the C6 nerve roots were replanted to construct the brachial plexus nerve-root avulsion model. After successful modeling, the injured rats were randomly divided into four groups. The first group(injury) did not receive any drug treatment, but was treated with a pure gel-sponge carrier nerve-root implantation and an ethanol-saline solution via intraperitoneal(i.p.) injection. The second group(melatonin) was treated with melatonin via i.p. injection. The third group(chondroitin sulfate ABC) was treated with chondroitin sulfate ABC through local administration. The fourth group(melatonin + chondroitin sulfate ABC) was treated with melatonin through i.p. injection and chondroitin sulfate ABC through local administration. The upper limb Terzis grooming test was used 2–6 weeks after injury to evaluate motor function. Inflammation and oxidative damage within 24 hours of injury were evaluated by spectrophotometry. Immunofluorescence and neuroelectrophysiology were used to evaluate glial scar, neuronal protection, and nerve regeneration. The results showed that the Terzis grooming-test scores of the three groups that received treatment were better than those of the injury 展开更多

Objective: To investigate the role and mechanism of autophagy in epithelial-mesenchymal transition (EMT) of oral squamous cell carcinoma cells induced by CQ (chloroquine) and rapamycin (RAPA). Methods: TGF-β (transforming growth factor β) was used to induce EMT in Cal-27 cell line. At the same time, RAPA was used to enhance and CQ was used to inhibit autophagy. The ability of cell migration was detected by scratch distribution test and the ability of cell migration was detected by Transwell chamber test. Western blot was used to detect the changes of ZO-1, vimentin, FN1 and other EMT-related proteins after 3 d induction, and SPSS 22.0 statistical software was used to analyze the data. Results: After 3 d of induction with 5 ng/mL TGF-β, E-cadherin decreased significantly and Vimentin increased significantly. Compared with the control group, the wound healing rate increased significantly (P<0.05) and the number of penetrating cells increased significantly (P<0.05) after 3 d induction with 5 ng/mL TGF-β, and then the cells were co-induced with 100 ng/mL RAPA and 100 ng/mL CQ and 5 ng/mL TGF-β for 3 d. Compared with TGF-β group. The healing rate of the RAPA co-induced with 5 ng/mL TGF-β group decreased significantly (P<0.05) and the number of penetrating cells decreased significantly (P<0.05). Compared with TGF-β group. The healing rate of the CQ co-induced with 5 ng/mL TGF-β group increased significantly (P<0.05) and the number of penetrating cells increased significantly (P<0.05). Compared with the control group, FN1 and Vimentin expression increased and ZO-1 expression decreased 3 d after induction with 5 ng/mL TGF-β. And then induced Cal-27 cells with 100 ng/mL RAPA and 100 ng/mL CQ and 5 ng/mL TGF-β respectively for 3 d. Compared with TGF-β group, FN1 and Vimentin expression decreased in RAPA co-induction group. Compared with TGF-βgroup, the expression of FN1 and Vimentin increased and the expression of ZO-1 decreased in CQ co-induction group. Conclusion: TGF-β can induce Cal-27 cells to establish 展开更多

Objective:To follow up the participants of the randomized clinical trial "Efficacy and Safety of Niaoduqing Particles(尿毒清颗粒) for Delaying Moderate-to-Severe Renal Dysfunction", and assess the long-term effects of Niaoduqing Particles on delaying the progression of renal dysfunction. Methods: Participants, who had previously been randomly assigned to receive Niaoduqing Particles or placebo for 24 weeks(146 cases in each group), were invited to follow-up and all were administered Niaoduqing Particles 5 g thrice daily and 10 g before bedtime for 24 weeks. The primary endpoints were changes in baseline serum creatinine(Scr) and estimated glomerular filtration rate(e GFR) after completion of the open-label treatment period. Results: After the double-blind period, the median(interquartile range) changes in Scr were 1.1(–13.0–24.1) and 11.7(–2.6–42.9) μmol/L for the Niaoduqing Particle and placebo groups, respectively(P=0.008), and the median changes in e GFRs were –0.2(–4.3–2.7) and –2.21(–5.7–0.8) mL·min^-1·1.73 m^-2, respectively(P=0.016). There were significant differences in the double-blind period changes in renal function between groups. After the open-label period, the median changes in Scr were 9.0(–10.0–41.9) and 17.5(–6.0–50.0) μmol/L for the Niaoduqing Particle and placebo groups according to baseline grouping, respectively(P=0.214), and the median changes in eGFRs were –2.3(–6.4–1.9) and –3.7(–7.5–1.1) mL·min^-1·1.73 m^-2, respectively(P=0.134). There were no statistical differences in the open-label period changes in renal function between groups. The eGFR reduction of participants who accepted Niaoduqing Particle treatment for 48 weeks was projected to 2.5 m L·min^-1·1.73 m^-2 per year. Conclusions: Niaoduqing Particles appear to have long-term efficacy for patients with moderate-to-severe renal dysfunction. Although there was no statistical difference, the early use of Niaoduqing Paticles seems to ameliorate the worsening of renal function. 展开更多

We experimentally and numerically investigate CH3I molecular alignment by using a femtosecond laser and a hexapole. The hexapole provides the single |111〉rotational state condition at 4.5-kV hexapole rod voltage. Based on this single rotational state, an enhanced alignment degree of 0.73 is achieved. Our experimental results are in agreement with the simulation results. We experimentally obtain the ion velocity map images and show the influence of the initial rotational-state population. With the I+ion images and angular distributions at different pump-probe delay time, the alignment and anti-alignment phenomena are further demonstrated. The molecules will be under field-free conditions when the laser effect disappears completely at the full revival time. Our work shows that the quantum control and spatial control on CH3I molecules can be realized and molecular coordinate frame can be obtained for further molecular experiment. 展开更多

BACKGROUND The lethal-7(let-7)family members and their targets are involved in the development and progression of tumors.Let-7-related polymorphisms have been reported to be associated with tumorigenesis and prognosis.In gastric cancer,however,the related studies are limited.AIM To investigate the role of let-7-related microRNA polymorphisms in the tumorigenesis and prognosis of gastric cancer in a Chinese population.METHODS A total of 898 gastric cancer patients and 992 tumor-free controls were recruited into this study from 2008 to 2013.Gastric cancer patients were followed periodically.Ten single nucleotide polymorphisms(SNPs)in the let-7 gene region or their target mRNAs were genotyped using the MassARRAY system and their associations with the risk for or overall survival of gastric cancer were analyzed.RESULTS All the ten SNPs were in Hardy-Weinberg equilibrium.The C allele of the rs3811463 polymorphism in the 3’-untranslated region(UTR)of LIN28A was associated with a lower risk of gastric cancer[odds ratio(OR)=0.74,95%confidence interval(CI):0.61-0.88,P=0.001]after adjustment for age and Helicobacter pylori status.Seven hundred and thirty-five gastric cancer patients who had undergone radical tumorectomy were included in the survival analysis and their 5-year survival rate was 53.9%(95%CI:50.1%-57.6%).The rs10889677 in the 3’-UTR of IL23R was corresponded to the prognosis of gastric cancer in a dose-response manner,in which the death risk increased by 25%[hazard ratio(HR)=1.25,95%CI:1.04-1.45,P=0.011]with each increase in the number of C alleles after controlling for other potential clinicopathological parameters.CONCLUSION The let-7-related polymorphism rs3811463 in LIN28A is associated with the susceptibility to gastric cancer and the let-7-related polymorphism rs10889677 in IL23R is associated with the prognosis of gastric cancer. 展开更多

To explore the ability of dihydroxyphenylalanine-insulin-like growth factor-1 (DOPA-IGF-1) coated hydroxyapatite/poly(lactic-co-glycolic acid)(HA/PLGA) microspheres to promote the proliferation and osteoclastic differentiation of rabbit bone mesenchymal stem cells(rBMSCs), HA/PLGA microspheres with different HA content (10%, 30%, 50%, mass fraction) were prepared by electrospinning method and HA/PLGA microspheres with 50% HA were coated with IGF-1 and DOPA-IGF-1, respectively. They were co-cultured with rBMSCs, respctively. Cell counting kit-8(CCK-8) detection, confocal laser scanning microscopy(CLSM), alkaline phosphatase(ALP) detection and osteogenesis related genes COL IA1, Runx2 and bone morphogenetic protein-2(BMP-2) detection were conducted to detect the proliferation activity, cell morphology, differentiation ability and the expression level of osteogenesis-related genes of cells cultured on all microspheres groups. The results showed that rBMSCs proliferation increased in an HA content dependent manner, and cells proliferated more in the IGF-1 coated and DOPA-IGF-1 coated groups, in particular in DOPA-IGF-1 coated group, and the differences were more remarkable over time (P<0.05). HA/PLGA microspheres promoted the proliferation and osteogenic differentiation of rBMSCs, and DOPA-IGF-1 coating enhanced the proliferation and osteogenic differentiation of rBMSCs. 展开更多

In this work, we mainly investigate the NH3 molecular multiphoton ionization process by using the photoelectron velocity map imaging technique. Under the condition of femtosecond laser(wavelength at 800 nm), the photoelectron images are detected. The channel switching and above-threshold ionization(ATI) effect are also confirmed. The kinetic energy spectrum(KES) and the photoelectron angular distributions(PADs) are obtained through the anti-Abel transformation from the original images, and then three ionization channels are confirmed successfully according to the Freeman resonance effect in a relatively low laser intensity region. In the excitation process, the intermediate resonance Rydberg states are C~1 A 1(6 + 2 photons process), B~1 E(6 + 2 photons process) and C~1 A 1(7 + 2 photons process), respectively. At the same time, we also find that the photoelectron angular distributions are independent of laser intensity. In addition, the electrons produced by different processes interfere with each other and they can produce a spider-like structure. We also find ac-Stark movement according to the Stark-shift-induced resonance effect when the laser intensity is relatively high. 展开更多

By a simple one-step reaction, a new fluorescein-based probe, 3'-hydroxy-3-oxo-3H-spiro[isobenzofuran-l,9'-xanthen]-6-yl-4-bromobutanoate(FLB), was designed and synthesized. Relative to other cations, anions or amines, the probe exhibited high sensitivity and specificity towards hydrazine with approximate 40-fold fluorescence enliancement at 515 nm. The detection limit(DL) was calculated to be 2.87×10^-8 mol/L, which was about 10 times lower than maximum allowable level of hydraziiie(3.12 ×1mol/L) in drinking water pennitted by the US Environment and Protection Association(EPA). Simultaneously, tlirough mass spectroscopy and proton nuclear magnetic resonance titration, the detection mechanism was further confirmed. Eventually, due to the low cytotoxicity and excellent water solubility, FLB could be employed to detect hydrazine in living cells. 展开更多

Sensitive and specific bioassays of tumor markers are critical for early cancer detection and treatment.In recent years,lanthanide(Ln3+)doped upconversion nanoparticles(UCNPs)have attracted wide attentions in tumor markers detectio n.Herein,we co mbined NaYF4:Yb,Tm and silver nanoparticles,serving as energy donor and receptor,respectively,to form an up-conversion fluorescence based inhibitory tumor marker biosensor system.The tumor marker CA-125 molecules are labeled with silver NPs,and the energy transfer fluorescent signal can be detected between the UCNPs and the silver NP receptors.The biosensor shows good stability,high sensitivity and selectivity in the tumor marker concentration range from 5 to 100 ng/mL,as well as a detection limit of 120 pg/mL.Due to the advantage of ease of fabrication and operation,low cost and high information capacity,this technology holds great potential for the clinical applications. 展开更多

MXene has shown distinctive advantages as anode materials of lithium-ion batteries.However,local surface chemistry,which was confirmed that can block ion transfer and limit redox reaction,has a significant effect on electrochemical performance.Herein,annealing MXene under hydrogen was employed for removing -F and turning -OH to -0 terminations.We dem on strate that it improves the kinetics of Li-ion transport between the electrolyte and electrode.As a result,a lower interfacial charge transfer impedance was obtained.The electrochemical measurement exhibited that a nearly 2-fold increase of specific capacity was achieved for the annealed MXene. 展开更多

OBJECTIVE: To investigate the effect of Qiangxin Huoli decoction on rats with chronic heart failure(CHF) induced by adriamycin(ADR), and to investigate the underlying mechanism of this effect.METHODS: Ninety-six healthy Wistar rats were divided into six groups: control, CHF model, CHF treated by Shenfu injection, and three CHF groups treated with Qiangxin Huoli decoction at high, medium, and low doses, respectively. Qiangxin Huoli decoction was administered orally to protect the stomach in the three Qiangxin Huoli decoction groups, while the control group and the CHF model group were administered the same volume of 0.9% physiological saline, and the Shenfu group wereadministered the same volume of Shenfu injection. Ten days later, the CHF model was then induced in all groups except the control group by intraperitoneal injection of ADR at gradient dose intervals. The bodyweights were recorded on days 10, 20, 30, and 40. Hemodynamic indices were recorded, including left ventricular systolic pressure(LVSP), left ventricular end-diastolic pressure(LVEDP), maximum increase in left ventricular pressure(+dp/dtmax), maximum decrease in left ventricular pressure(-dp/dtmax), heart rate(HR), and electrocardiogram using an eight-channel physiological recorder with LabChart software monitoring. The plasma brain natriuretic peptide(BNP) concentration was determined by enzyme-linked immunosorbent adsorption. The expressions of B-cell lymphoma-2(Bcl-2) and Bcl-2-associated X protein(Bax) were detected by immunohistochemical methods.RESULTS: The CHF model group were in poor condition, and the mean bodyweight was significantly decreased compared with the control group. Furthermore, compared with the control group, the CHF groups had significantly decreased LVSP, +dp/dtmax, and-dp/dtmax, and significantly increased LVEDP. The CHF groups also showed significant increases in HR, S-T segment elevation, and plasma BNP levels compared with the control group.
